Self-induced penomegaly.

A 56-year-old Caucasian man applied elastic bands to the penile base to overcome flaccidity of the glans during erection. He continued the practice over 25 years, which lead to lymphoedema, fibrosis and consequently penomegaly. Erection was not compromised otherwise.

The concordance of Chlamydia trachomatis genital infection between sexual partners, in the era of nucleic acid testing.

INTRODUCTION: The rate of transmission of Chlamydia trachomatis, by infected males and females to their sexual partners, has been a matter of continued scientific interest and exploration. METHODS: We examined the correlation of C. trachomatis infection in sexual partnerships, using Strand Displacement Amplification. RESULTS: During July-November 2003, 97 male patients were reported positive for C. trachomatis. Fifty of the female sexual consorts were amenable for contact tracing, 38 of whom were identified as C. trachomatis positive. Within the same period, 93 female patients were C. trachomatis positive, and 56 male consorts were traceable, of whom 43 were positive for C. trachomatis. CONCLUSIONS: The concordance of C. trachomatis between sexual partners is in the region of 75%. This strengthens the case for epidemiological treatment for all consorts.

Alcohol and sexual behaviour.

Alcohol, the oldest drug known to the human race, is widely perceived to affect sexual behaviour. The general public, scientific, medical and judicial establishments share and express interest in the potential effects of alcohol on the sexual behaviour of individuals. The understanding of the current state of medical knowledge, regarding the effect of alcohol on sexual behaviour, is of particular interest for the development of sexually transmitted diseases control strategies, and may be called upon for investigating alleged assault and rape. The perceived effects are influenced by historical, mythological, cultural, religious, physiological, pharmacological, expectancy, personal and legislative factors. The potential effects are investigated through surveys, global/situational association studies and placebo-controlled experiments. This article provides a review of literature and a summary of the current state of knowledge regarding the correlation between alcohol and sexual behaviour.

The management of penile intraepithelial neoplasia in genitourinary medicine.

The incidence of reporting of penile intraepithelial neoplasia (PIN) by genitourinary medicine clinics in the UK is rare. The reporting of higher incidence, in most international studies, is suggestive of under-diagnosis of PIN lesions in the UK, which leads to improper therapy and follow-up. A systematic literature review is undertaken, regarding the current state of management of PIN and the possible avenues of improvement in the diagnosis and follow-up. The role of genitourinary medicine clinics is examined, in view of the high number of patients that present with genital dermatological conditions, in whom there are patients with non-identified PIN lesions. The notable increase in reporting of vulval intraepithelial neoplasia has been associated with an increase in vulval cancer, especially in younger patients. The possibility of a similar trend in penile lesions needs to be observed and the association between high risk human papillomavirus (HPV) types, and genital intraepithelial neoplasia and cancer, is recognized and therefore an increased incidence in the diagnosis of HPV associated lesions must be considered.

The male genital skin burning syndrome (Dysaesthetic Peno/Scroto-dynia).

Men may complain of penile and/or scrotal skin burning with no evidence of positive physical signs or investigations. The condition is cumbersome and leads to stress and disruption in social and sexual relationships. The patients report no response to previous medications (including antibiotics, antifungals and topical corticosteroids); and identify improvement in symptoms and quality of life on selective serotonin re-uptake inhibitors (SSRI). A similar condition has been recognized in the female patients (dysaesthetic vulvodynia). We report the occurrence of this condition in three men and suggest it being recognized as 'the male genital skin burning syndrome' (Dysaesthetic Peno/Scroto-dynia).

The diagnosis of hepatitis C in a semi-rural genitourinary medicine clinic.

A retrospective casenote study was conducted to examine the risk factors for patients who were diagnosed as hepatitis C (HCV)-positive (between 1999 and 2001) in a semi-rural genitourinary medicine (GUM) setting in Staffordshire, UK. There was a remarkable escalation in the number of reported HCV-positive cases, year on year, in our study. The majority of the positive cases (20/21) gave a history of previous intravenous drug use (IVDU). The incidence of other sexually transmitted infections were reportedly high in our HCV-positive patients. The increasing number of reported HCV-positive cases in the GUM clinic of the semi-rural Staffordshire setting, may reflect a national pattern that needs further investigations. We advise that HCV serology should be offered to GUM clients (who have a history of IVDU), and to their sex partners. They should also be advised to take tests to exclude other STDs. The case for offering HCV serology as a routine test for patients who request 'the exclusion of STDs' is still undecided.
