Simplified endometrial testing by the Milan-Markley technic.

Recent articles have urged physicians to give increased attention to the use of cytologic and histopathologic technics in the detection of precancerous and cancerous endometrial lesions, since there is an increasing incidence of malignant disease at that site. A routine cervicovaginal smear is inadequate to diagnose cancer of the endometrium. This paper describes a technic for rapid smearing and fixation of endometrical cells. The method of obtaining endometrial cytologic and histologic material is by the use of a helix. A report on 1,748 endometrial cytologic smears is given.

Surveillance of hydatidiform mole with the Milan-Markley helix.

A case of complicated trophoblastic disease followed by quantitative radioimmunoassay and endometrial cytology is presented. Despite the fall of the postevacuation hCG titer to zero, trophoblastic elements were still detected cytologically with the helix technic. A second curettage performed 60 days postevacuation because of a rising serum hCG titer and persistent trophoblastic elements on the endometrial smears was histologically negative. Subsequent helix cytologic studies continued to reveal trophoblastic cells. Chemotherapy with methotrexate and hysterectomy were carried out. A solitary 7-mm lesion was found deep in the myometrium. Titers dropped abruptly postoperatively. Photomicrographs of the cytologic findings are presented.

Endometrial cytoloty. Using the Milan-Markley Technic.

Increased attention has been given to cytologic and histopathologic technic in the detection of endometrial lesions because of an increasing incidence of endometrial malignancy. The cervical-vaginal smear as performed by most clinicians is inadequate for the detection of endometrial neoplasia. Existing technics for the detection of endometrial lesions are costly and time consuming. An effective, reliable and economical technic has been developed for rapid smearing and fixation of endometrial cells. The routine endometrial screening of 739 patients (1143 specimens) using the Milan-Markley technic detected 4 cases of endometrial carcinoma, 1 case of carcinoma of the tube, 22 cases of endometrial hyperplasia, and 9 of endometrial polypi. It is hoped that this cytologic method will complement existing histologic detection procedures.

A safer uterine curette.

A new uterine curette is described, with features designed to help eliminate some of the instrumentation hazards in the operation of uterine curettage. An analysis of the disadvantages of the traditional curette is presented together with a description of how these have been minimized or eliminated in the new instrument. The fundamental difference in the new instrument is its tip. It has features to minimize the likelihood of perforation, deep gouging, and unnecessary injury to the basilar endometrium. Prototypes of the new instrument have gone through an evolutionary and testing period of 25 years, during which the instrument has been used by many operators. There have been no serious accidents or complications reported, and there has been no need for repeat curettage.
