Mammography in a mobile setting: remaining barriers.

This study examined the barriers to undergoing mammography perceived by a group of women with ready access to reduced-cost screening mammography in a mobile van at the workplace. The subject sample comprised women who made appointments for mammography at the Susan G. Komen Mobile Breast Center in Dallas. Women who underwent mammography and women who made appointments but did not undergo the examination were asked to complete a survey examining potential barriers to undergoing mammography. The group of patients surveyed were, as a whole, well educated, affluent, less than 50 years of age, and knowledgeable about breast cancer. Compliant women were more likely to have previously undergone mammography (P less than .001), to have been influenced by their physician's advice to undergo mammography (P less than .005), and to be influenced by the media than were noncompliant women (P less than .005). These findings may be useful in developing strategies to increase mammographic screening in a self-referred population.

Pulmonary disease in AIDS patients.

Pulmonary disease remains a major complication of the human immunodeficiency virus (HIV). Over the past decade several changes in the pattern of disease have occurred. Pneumocystis carinii pneumonia (PCP) remains the most common opportunistic pathogen in AIDS patients, though its incidence on bronchoscopy has declined and empiric therapy often occurs without a specific diagnosis. Changes in the management of patients with PCP have included different dosages and routes of administration for chemotherapy, improved overall survival, and a recent increase in the number of patients surviving episodes of respiratory failure. In addition, infection with mycobacteria tuberculosis (M.Tb.) has emerged as a major public health problem. The pattern of M.Tb. is distinct from non-immunocompromised patients though response to therapy usually occurs.

Apparatus seen on chest radiographs after cardiac surgery in adults.

The authors review the purpose, radiographic appearance, and possible complications of various tubes, catheters, and wires seen on chest radiographs after cardiac surgery. Drainage tubes, temporary epicardial wire electrodes, surgical clips, intraaortic counterpulsation balloon, atrial pressure monitor catheters, and sternal wires are reviewed. Because recent articles have discussed the Swan-Ganz catheter and automatic implantable cardioverter defibrillator, these are not covered in depth.

Retrocardiac phantom image on hypocycloidal chest tomograms.

A phantom image produced by displaced blur margins of the cardiac silhouette and mimicking a paraspinous retrocardiac mass was noted in 50% of 125 hypocycloidal full chest tomograms. It was reproduced on tomograms of a chest phantom. Careful comparison with routine posteroanterior and lateral chest radiographs prevents misinterpretation.
