A British experience of surgical voice restoration as a primary procedure.

It has been the aim of surgeons since Billroth first described laryngectomy to restore speech to the patient. However, a substantial percentage of patients fail to develop esophageal speech. Many authors have confirmed the success of the tracheoesophageal fistula procedure as a means of surgical voice restoration as a secondary procedure. We have performed tracheoesophageal puncture as a primary procedure at the time of laryngectomy in 36 patients, with a success rate of 89%. We believe that primary puncture is the method of choice for surgical voice restoration following laryngectomy.

Type A behavior pattern and motor vehicle drivers' behavior.

2 major components of the Type A coronary-prone behavior pattern are said to be a chronic sense of time urgency and impatience. The present study was done to determine whether these characteristics are associated with the driving performance of Type A individuals. 38 women and 32 men completed the Jenkins Activity Survey and a questionnaire concerning their driving. Those subjects exhibiting more Type A behavior tended to be more impatient, reported being involved in more accidents, and received more tickets for driving violations than those scoring lower on the Type A scale.

Communication aids in patients with motor neurone disease.

The study aimed at determining the main factors responsible for the speech and communication problems in motor neurone disease to try to assess the suitability of commercially available aids. Sixteen patients suffering from motor neurone disease with communication problems had their speech assessed by using the Frenchay dysarthria assessment. It was found essential to offer a choice of aids to the patient since suitability of a particular aid could not be evaluated without the patient practising with it.

The accident at Chernobyl: radiation doses and effects.

Four years ago next month, on April 26, 1986, a nuclear power plant in Chernobyl, Russia, exploded releasing tremendous amounts of radioactive substances into the atmosphere. This paper describes the types of radiation released, the levels of exposure, the number of people exposed and short-term effects observed.
