The peripheral antinociceptive effect induced by morphine is associated with ATP-sensitive K(+) channels.

The effect of several K(+) channel blockers such as glibenclamide, tolbutamide, charybdotoxin (ChTX), apamin, tetraethylammonium (TEA), 4-aminopyridine (4-AP) and cesium on the peripheral antinociceptive effect of morphine was evaluated by the paw pressure test in Wistar rats. The intraplantar administration of a carrageenan suspension (250 microg) resulted in an acute inflammatory response and a decreased threshold to noxious pressure. Morphine administered locally into the paw (25, 50, 100 and 200 microg) elicited a dose-dependent antinociceptive effect which was demonstrated to be mediated by a peripheral site up to the 100 microg dose. The selective blockers of ATP-sensitive K(+) channels glibenclamide (20, 40 and 80 microg paw(-1)) and tolbutamide (40, 80 and 160 microg paw(-1)) antagonized the peripheral antinociception induced by morphine (100 microg paw(-1)). This effect was unaffected by ChTX (0. 5, 1.0 and 2.0 microg paw(-1)), a large conductance Ca(2+)-activated K(+) channel blocker, or by apamin (2.5, 5.0 and 10.0 microg paw(-1)), a selective blocker of a small conductance Ca(2+)-activated K(+) channel. Intraplantar administration of the non-specific K(+) channel blockers TEA (160, 320 and 640 microg), 4-AP (10, 50 and 100 microg) and cesium (125, 250 and 500 microg) also did not modify the peripheral antinociceptive effect of morphine. These results suggest that the peripheral antinociceptive effect of morphine may result from activation of ATP-sensitive K(+) channels, which may cause a hyperpolarization of peripheral terminals of primary afferents, leading to a decrease in action potential generation. In contrast, large conductance Ca(2+)-activated K(+) channels, small conductance Ca(2+)-activated K(+) channels as well as voltage-dependent K(+) channels appear not to be involved in this transduction pathway. A narrow QRS does not predict a normal left ventricular function in Chagas' disease.

Left ventricular (LV) systolic dysfunction is a major prognostic determinant in Chagas' disease (ChD), a potentially fatal disorder which affects nearly 20 million individuals in Latin America. Therefore, reliable screening methods are needed to identify patients in need of a detailed evaluation of LV function. Since previous reports have suggested that a prolonged QRS duration may be an accurate predictor of LV dysfunction, the relationship between QRS duration and LV function was evaluated in ChD patients, in order to determine whether a narrow QRS on surface electrocardiogram (ECG) predicts a normal LV function. Ninety-eight patients with ChD and no other cardiac or systemic illness were underwent standard 12-lead ECG and Doppler echocardiogram (echo) examination. An investigator (blinded to the echo data) measured manually the QRS duration. LV dysfunction was defined by the presence of LV dilatation (LV diastolic dimension > 55 mm), depressed LV ejection fraction (LVEF < 0.50), segmental contractile abnormalities (SCA) or LV aneurysm. QRS duration was significantly correlated with LV diastolic dimension (r = 0.44, P = 0.000) and LVEF (r = 0.24, P = 0.016). A significant percentage of patients with QRS < 100 ms had abnormal systolic LV parameters, including LV dilatation (8.1%), reduced EF (10.2%), SCA (42.8%) and LV aneurysm (14.2%). A narrow QRS does not exclude the presence of significant global and segmental LV dysfunction in ChD patients. Since these abnormalities have prognostic and therapeutic implications, further evaluation of LV systolic performance is desirable in such patients.

[Understanding the sexuality of HVI-1 carriers].

The objective of the present investigation was to identify the sexual difficulties experienced by persons in an HIV-1 crisis. This is a study of a descriptive nature based on the Theory of Crisis. The sample consisted of 5 patients with HIV-1 followed up clinically in a town in the hinterland of the state of São Paulo. Semistructured interviews were tape recorded and transcribed. Data were analyzed according to MEIHY (1996) and BARDIN (1994). We conclude that the subjects presented a negative adaptation with respect to sexual, social and emotional difficulties. The results indicate the need for effective nursing interventions with persons in an HIV-1 crisis.

[Sexual problems experienced by women in HIV-1 crisis].

This descriptive study was based on the assumptions of qualitative investigation methods and on the Crisis Theory formulated by CAPLAN (1966). The objective of the study was to identify the sexual problems experienced by women undergoing an HIV-1 crisis. Data were collected through a semi-structured interview recorded by the method of FREITAS et al. (1992) and analyzed by the method of MEIHY (1996). Authors concluded that these women maintained themselves unbalanced using negative coping mechanisms. Thus, authors believe that it would be possible to help them using adequate nursing actions such as educational and guidance actions, but mainly by actions directed to the needs of patients undergoing an HIV-1 crisis.

[Nurses' professional satisfaction].

We carried out a study with 91 nurses, trying to find out about the feelings of these professionals regarding their satisfaction at work. We used the Work Satisfaction Assessment Questionnaire (WSAQ), drawn up and validated by Siqueira (1978) and adapted with the analysis of seven factors: General Satisfaction; Physical and Psychological Stress; "Status" of the Job; Location of the Company; Compensating Benefits; Recognition and Personal Development. Data showed nurses satisfied with their work, in its intrinsic aspects (Accomplishment, Recognition and Autonomy). The psychiatric nurses were the most mature, most experienced, showing a higher satisfaction level, whereas the pediatric nurses were the youngest, most inexperienced and presenting the highest level of dissatisfaction at work.

[Therapeutic relationship between a nurse and a depressed patient].

This is the report about a therapeutic relationship developed with a depressed patient using the principles of non directive techniques centralized on the patient, the nurse tried to understand the problem brought up by him and his alternative of solution establishing the help relationship with the least interference. The analysis point out the patient problems such as the positive and negative aspects of the nurse performance.

[Nurses who quit nursing: why?].

Professional evasion of nurses is a concern because there was an investment from State, family and from the individual himself and there is a lack of nurses at work field. We have collected, in an intentional sample, the opinion of nurses about the reasons that made them quit nursing, the nursing knowledge they use in their actual activity and their suggestions to avoid evasion. We have checked out according to the answers that nurse's evasion is based on a set of circumstances from strictly personal ones to formal education as well as others resulting from the social and political structure.

A model for analysis of the nurse-patient interactive process (MAIP).

A model for analysis of the nurse-patient interactive process (MAIP), based on the analysis of the content of that interaction, is proposed as a method for determining appropriate psychosocial nursing diagnoses for the patient. The model focuses on the qualitative aspects of the phenomenon of interaction. The MAIP permits a systematic analysis of the nurse-patient interaction, and in turn will provide guidance for the professional in their actions in the interactive process.

[Group process in nursing: possibilities and limits].

This work shows studies and reflections about the utilisation of groups in nursing assistance. As an aim sought to identify, through nurses perspective, aspects about their motivations to work with groups, their source of knowledge and relevant points of their experience with groups. The results demonstrates that nurses recognise the therapeutic value of groups in assistance and provide suitable conditions to its development. However, difficulties to handling groupal situations are limiting elements to develop this activity that reveal the human sentiment meander, indicating that nurse group co-ordinator needs, beyond theoretical establishment, to practice by their self-knowledge to provide an ambient and a interpersonal relationship able to obtain the real significance of this activity.

[Non-directive nurse-patient relationship in the process of re-socialization].

The present study aimed at analysing a nurse-patient interaction through a non-directive approach, centred in the care to patients in the stage of re-socialization. It was based on Rogers' studies and their application in nursing by Braga and Rodrigues. A new project of re-socialization of patients from a state hospital was followed. A patient participating in this program agreed to be interviewed with a nurse. The non-directive relationship was registered and analysed, focussing on the nurse's feelings, as well as her interventions considering verbal and non-verbal communication. The anxiety of the nurse was identified especially regarding technical application and the patient's anxiety reflected the consequences of her adaptation outside the hospital environment.

[Support services for students offered in Brazilian nursing schools].

The authors present an exploratory study realized with 103 brazilian Colleges of Nursing. They aimed at identifying the types of assistance offered to the undergraduate students. The results of 61 courses revealed two categories: 1- offering pedagogic-administrative support services, showing the concern with the formal orientation about nursing education; 2- adding any kind of care delivery to the students needs. Nine courses surpassed the essential needs (mentioned above), concerning with the student's mental health.

Characterization of a new angiotensin antagonist selective for angiotensin-(1-7): evidence that the actions of angiotensin-(1-7) are mediated by specific angiotensin receptors.

In this study we describe a new angiotensin antagonist [Asp1-Arg2-Val3-Tyr4-Ile5-His6-D-Ala7, (A-779)] selective for the heptapeptide angiotensin-(1-7) [Ang-(1-7)]. A-779 blocked the antidiuretic effect of Ang-(1-7) in water-loaded rats and the changes in blood pressure produced by Ang-(1-7) microinjection into the dorsal-medial and ventrolateral medulla. In contrast, A-779 did not change the dipsogenic, pressor, or myotropic effects of angiotensin II (Ang II). Also, A-779 did not affect the antidiuretic effect of vasopressin or the contractile effects of angiotensin III, bradykinin, or substance P on the rat ileum. In the rostral ventrolateral medulla, the pressor effect produced by Ang-(1-7) microinjection was completely blocked by A-779 but not by AT1 or AT2 receptor antagonists (DUP 753 and CGP 42112A, respectively). Conversely, the pressor effect produced by Ang II was not changed by A-779 but was completely blocked by DUP 753. Binding studies substantiated these observations: A-779 did not compete significantly for 125I-Ang II binding to adrenocortical membranes at up to a 1 microM concentration. Low affinity binding was also observed in adrenomedullary membranes with an IC50 greater than 10 microM. Our results show that A-779 is a potent and selective antagonist for Ang-(1-7). More importantly, our data indicate that specific angiotensin receptors mediate the central and peripheral actions of Ang-(1-7).

[Surgical and trauma patients in a medical intensive care unit].

OBJECTIVE: to characterize and to assess in terms of severity the surgical and trauma patients admitted to a medical intensive care unit (ICU). DESIGN: retrospective study base on clinical records and the ICU computerized database. SETTING: the medical ICU of a tertiary hospital. RESULTS: of the 2468 patients admitted to the ICU in 1989, 289 (11.7%) were surgical or trauma ones. The more frequent reasons for admission were: the need for mechanical ventilation, metabolic problems, and depression of consciousness. Of these 289 patients, 48.1% required mechanical ventilation, 14.9 hemodialysis; 4.8% had a pulmonary artery catheter inserted. Mean APACHE II, TISS and MOF scores were high (20.09 +/- 9.29, 24.17 +/- 11.45 and 5.4 +/- 3.59); they were determined in 79.2, 88.2 and 43.9% of patients respectively. Both APACHE and TISS scores were correlated with mortality. When compared with medical patients, surgical/trauma ones although younger (52.9 +/- 20.7 years versus 55.9 +/- 20.2, p = 0.00152), had a longer mean stay in the ICU (7.63 +/- 12.7 days v. 3.64 +/- 7.61, p = 0.0001), and a higher mortality (also in the ICU) (28.7 v. 16.7, p = 0.0005. COMMENTS: these are seriously ill patients, who are frequently referred to the ICU in late stages of clinical evolution. We propose they should be closely followed, from the earliest possible stage, by medical-surgical teams, in order to benefit from a multidisciplinary approach.

[Poisoning at a polyvalent intensive care unit].

A retrospective analysis was made concerning 1340 admissions to a multidisciplinary Intensive Care Unit over four years (1986-1989), because of acute poisoning. In 666 patients occurred 735 drug overdoses and in 674 patients, 691 non-drug intoxications. The drugs with main action in central nervous system were the most frequent in drug overdoses and the organophosphates insecticides in the non-drug intoxications. During the considered period there was an absolute and relative fall in the number of admissions for acute poisoning (480 vs 244, 15.1% vs 9.9%). The patients admitted were 698 men (289 in drug-overdoses and 409 in non-drug group) with a statistically significant difference between the type of intoxication and sex (p less than 0.001). Age mean was 40.2 +/- 19.6 years, almost the same in the two groups of intoxications. The peak incidence occurred from March to October. From the hospital area of Lisbon came 814 patients and outside of Lisbon 495 patients. The mean duration of stay in the ICU was 3.90 +2- 6.15 days, being 2.71 +/- 3.80 in the drug overdoses and 5.08 +/- 7.62 in non-drug intoxications (p less than 0.001). From the therapeutic measures we emphasize mechanical ventilation (29.5%), total parenteral nutrition (18.5%), dialysis (2.9%), Swan-Ganz catheter (2.8%) and pacemaker (1.2%). The mortality rate was 13.7%, higher in non-drug intoxications (23.0%) than in drug overdoses (4.4%) (p less than 0.0001).

[Nurses interacting therapeutically with elderly patients in chronic depression].

In a humanist care model we searched to analyse experiences lived by nurses interacting with chronic depression patients. There was a female patient who was being assisted by Psycho-social Support Nucleus (NAP), 60 years old, presenting depressive symptoms, proper for her age, probably stressed by her suffering experiences in life. It was a therapeutic interaction as matters of the patient's interest (not the nurses interest) have been presented; the pace of communication has been determined b the patient herself; the nurse (except in the very beginning when the patient was a little anxious) followed her communication, physically approaching her whenever it was necessary; intervened only at necessary moments; inspired confidence that was necessary for the patient to talk about her intimate feelings; helped her to recover when she was out of control; conducted her safely, but with comprehensive attitude until the patient felt easy to leave her; demonstrated that she could help her in other moments if she thought it was necessary. Interaction achieved its aims helping the patient and being technically adequate for therapeutic and diagnosis aspects.

[Mental health nursing for women in a crisis due to accident].

This study shows that the psychiatric nurse has been called to act therapeutically with hazard population in the ambit of the mental health. The author uses the technique of the aid relationship to the persons in a systematic and empathetic performance. In respect to the results obtained in intervention near the women in accidental crisis the nurse earns a real space to her mental health actions.
