[Diagnosis of mild cranio-cerebral injury].

As many as 143 patients with concussion and 79 with contusion of the brain of the mild degree were examined. In concussion, the oral reflex never occurs. Meanwhile Marinesco-Radovici reflex is positive almost in 90%, appearing, however, not earlier than 6 hours after the injury and lasting up to 7-16 days. It should be mentioned that in 53.5%, the reflex is unilateral. The latter fact indicates that in concussion, over half the patients manifest the signs of cortical focal derangement together with general cerebral disseminated minisymptoms, seen throughout 1-2 weeks. In mild contusion of the brain in the acute stage, the oral reflex is detectable in 27% of cases. Marinesco-Radovici reflex appears only by days 2-4 after the contusion. The data obtained are helpful in defining the level, degree of the involvement of brain structures, and dynamics of their reverse development in patients with craniocerebral injury, thereby contributing to adequate differential diagnosis of concussion and contusion of the brain of the mild degree.

[Characteristics of acoustic neuroma in the group of schwannomas of different localizations].

Neurinomas and schwannomas affect sensitive fibers almost exclusively. Among craniocerebral neurinomas, nerve VIII neurinomas account for 90%, occurring 30 to 40 times more frequently as compared to nerve V neurinomas. The latter circumstance may be determined by more complex evolution of the vestibular ganglion as compared to the Gasserian ganglion. Nerve VIII neurinoma never becomes malignant whereas the peripheral spinal nerves and radicles undergo degeneration to sarcomas in 17% of cases. As for neurologic oncology, there are no sex differences in the disease incidence. However, women get afflicted with nerve VIII neurinoma several times more often than men.

[The mechanisms of speech recovery following revascularization surgery in patients having had an ischemic stroke].

A study is presented of 20 patients with ischemic stroke showing speech disorders. After the surgically performed extra-intracranial microanastomosis of the superficial temporal artery with branches of the middle cerebral artery there always followed an improvement of the aphasic defect. During control angiographic examination a 2% solution of novocaine was introduced into the carotid artery. Injection of the preparation into the left carotid artery produced a short-term cessation of speech while administration of novocaine into the right carotid artery did not effect the speech function. It is concluded that speech restoration occurs due to rehabilitation processes in the dominant hemisphere of the brain.

[The endonasopalpebral reflex].

The authors describe the endonasopalpebral reflex which is evoked from the nasal mucosa. Reduction of this reflex may be used as a fine index of involvement of the trigeminus in patients with acoustic neuromas. This makes it possible to differentiate this tumour at early stages of its development from neuritis of the acoustic nerve. This is of great importance for this pathological situation.
