Beta-adrenergic blockade in stress protection. Limited effect of metoprolol in psychological stress reaction.

In a model system the influence of mental stress on blood pressure and heart rate was studied in normal persons and in patients with hypertension (WHO grade III). Metoprolol was employed to investigate the preventive effect of beta-adrenergic blockade on the response to stress. In all groups blood pressure increased significantly during mental stress. The effect was not inhibited by metoprolol. The rise in heart rate, however, was depressed by beta-blockade. Reaction time, opticomotor coordination and concentration ability were studied as parameters of vigilance, but no significant difference between the metoprolol and control groups were observed. Thus, metoprolol only influenced the heart rate in mental stress and it did not affect vigilance.

A simplified method for measurement of Ca++-activity in serum, using serum-monovettes and a Ca++-selective disk-electrode.

A simplified method for the anaerobic measurement of ionized calcium in serum of venous blood by use of ion-selective electrodes--the monovette-technique--is described. For estimation of Ca++ a new electrode type--the disk-electrode (11, 12, 13, 14)--charged with a synthetic, electrically neutral Ca++-carrier of the octandioic acid-type (1), is tested.

[Transfemoral embolus aspiration from both renal arteries].

In a 50-year-old female patient who had suffered from coronary heart disease for about 3 years, a cerebral embolus resulted in left-sided hemiparesis. Four days later she became anuric. Acute thromboembolis occlusion of the renal arteries was assumed and renal angiography was performed. The angiography showed complete obstruction of the left renal artery and partial obstruction of the right renal artery. Due to the poor general condition of the patient it was not possible to carry out renovascular surgery or streptokinasetherapy. An attempt was therefore made to remove the emboli from the renal arteries by a new technique (transfemoral embolus aspiration). The subsequent angiography showed normal circulation in the right kidney and improved circulation in the left kidney. Ten days of dialysis treatment were required until urine production started again. Death then occurred from other causes. The autopsy confirmed free permeable renal vessels and absence of permanent sequelae in the parenchyma.

Haemodialysis in rats.

A plate dialyser requiring an extracorporeal blood volume of 1.5 ml was developed to dialyse conscious rats. In experiments in vitro and in vivo its function was tested. The in vitro clearances of urea, creatinine and potassium were 126+/-9 ml/min/m2, 70.5+/-9 ml/min/m2, and 132.5+/-13 ml/min/m2, respectively. The method appears to be suitable for pharmacological and toxicological studies.

[Investigations on the performance of calcium selective disc-electrodes with electrically charged and neutral ligands in anaerobic serum measurements (author's transl)].

Anaerobically prepared serum samples from healthy subjects were simultaneously analysed for Ca++ in a flow-through-system of three ion selective disc-electrodes (electrochemical multimeasurementsystem). Two of the sensors contained electrically charged ligands, the third contained a neutral carrier. With neutral carriers an improvement of performance was noted in that there was a large increase in selectivity toward H+- and NH+4-ions and a distinct enhancement of reproducibility.

Evaluation of essential amino acids and keto acids in uremic patients on low-protein diet.

The effectiveness of a mixture of five analogues of essential amino acids and the remaining four essential amino acids as compared to a preceeding treatment period of the nine essential amino acids was evaluated in 16 chronic uremic patients fed a low-protein diet. During amino acid analogues supplementation, there was a tendency for blood urea nitrogen to fall whereas creatinine did not change. Serum phosphate decreased in most patients, whereas serum calcium rose in some subjects. Protein metabolism, as judged by serum transferrin, Clq, C3c, total complement activity, was improved. Furthermore, the concentrations of prealbumin and retinol-binding protein, which are elevated in uremia, showed a further increase that might favor a vitamin A intoxication.

[Critical analysis of the saralasintest in the diagnosis of hypertension (author's transl)].

The reliability of the angiotensin II (AT II)-antagonist Saralasin in the diagnosis of AT II-dependent forms of hypertension was investigated in 61 cases of hypertension of different etiology. In 14 patients, lowering of blood pressure by Saralasin suggested an AT II-dependent hypertension which could be ascertained in 8 patients (5 had undergone successful surgery) by increased levels of plasma-renin-activity (PRA), AT II, PRA-ratio in renal vein blood and by angiography. Besides, depressor reactions by Saralasin yielded additional information in three patients with renovascular hypertension but normal levels of PRA and AT II, in two patients with high renin essential hypertension and one patient with pheochromocytoma. This test seems to be valuable in the diagnosis of renin-dependent hypertension.

[The pros and cons of haemoperfusion (author's transl)].

6 patients with severe self-poisoning were treated by charcoal-haemoperfusion in our centre up to now. In four of them (all suffering from sleeping drug overdosage) the treatment was successful. Two patients with intoxications by agrochemicals died in spite of haemoperfusion. Side effects of haemoperfusion were drops of blood pressure and platelet count, depletion of immune bodies, and adsorption of remedies. Up to now, the indication for haemoperfusion has to consider these secondary actions of encapsulated charcoal as inevitable.

Diuretic effects of Bay g 2821 in patients with advanced renal failure.

Clearance studies were carried out in 18 non-oedematous patients with advanced renal failure (glomerular filtration rate less than 12 ml/min) to assess the diuretic effects of Bay g 2821 given different oral doses (120 mg to 360 mg). The results showed a significant increase in urinary volume and in clearances of sodium, calcium, magnesium, chloride, inorganic phosphorus and 51CrEDTA. There were no significant changes, however, in clearance of PAH, urea and uric acid. The diuretic response started in the first hour after drug intake, reached a maximum after 3 to 5 hours, and declined slowly in the following 3 to 4 hours. In patients with a glomerular filtration rate less than 3 ml/min the drug lost its effect. In rates between 5 to 10 ml/min the efficacy of Bay g 2821 was comparable to that of furosemide and ethacrynic acid. No side-effects were reported.
