[Results of treatment of extradural spinal neoplasms].

82 patients with malignant epidural neoplasms (s.e.n.) were operated on between 1980 and 1990. Some underwent radio- or chemotherapy following surgery. Exact data on neurological results of treatment were obtained from 42 cases which represented the analysed group. When patients were ambulatory and/or had useful motor function of their upper extremities following treatment the result was accepted as positive. This was found in 71% of cases. In patients with severe paresis (non-walking before surgery) positive result was obtained in 59% of cases. In patients with mild paresis or neurologically without signs, positive results were obtained in 100% of cases. Of patients who died, 45% had positive neurological result until death. Overall 21% of cases (49% of those with severe neurological deficits) did not regain the ability of walking that is the most important benefit which may result from treatment. Those patients presented in severe general condition, advanced neoplastic spread to other systems and total transverse spinal cord damage. In about 90% of cases there was a significant relief of back and radical pain.

[Thoracic outlet syndrome: results of surgical treatment].

21 patients with thoracic outlet syndrome (t.o.s.) were operated on in the Department of Neurosurgery of MMA in Lódź between 1982-1991. Early and long-term results of treatment were analyzed in the group of 13 cases. In early postoperative period 5 patients were free of pain and neurologic or vascular disturbances in the upper limb. In other 6 cases pain disappeared partially and mild neurological or vascular syndromes persisted. Altogether satisfactory results of treatment were obtained in 8 patients (85%). There was some improvement in 2 patients but pain and neurological or vascular disorders were still significant after surgery. Long-term results were assessed at least 1 year after operative treatment. Recurrent pain with or without mild neurological (vascular) syndromes appeared in 6 patients (46%). Recurrent t.o.s. occurred mainly in cases of young women with low position of shoulder girdle and poorly developed musculature of this region. Only 2 patients still remained free of preoperative syndromes. Altogether only 8 patients (61%) had acceptable results of surgical treatment at long-term follow-up. The remaining 5 patients (39%) had unsatisfactory long-term results.

[Diagnosis of the upper thoracic outlet syndrome].

Basing on a review of literature and own experience the authors present the aetiology and various diagnostic methods of thoracic outlet syndrome (t.o.s.), especially electrodiagnostic and Doppler studies. Provocative tests like Wright, Adson or hyperabduction manouvers are discussed and their role in modern diagnosis of t.o.s. is presented.

[Somatosensory evoked potentials in the assessment of preserved function of long spinal cord pathways versus decisions of operation].

In the light of several presented clinical cases the authors point out the usefulness of somatosensory evoked potentials for the objective determination of the continuity of long pathways in the spinal cord, which in correlation with the clinical findings and results of imaging methods helps in making the decision of surgical intervention. The non-invasiveness of this method makes possible repeated safe control examinations for assessment of the dynamic of lesions in the cord and objective assessment of the neurological condition after operations on the cord.

[A case of complete Th7-Th8 fracture-dislocation treated by one-stage posterior and transthoracic approach].

The authors present a case of severe traumatic fracture-dislocation of Th8 with total anterior slippage of Th7 on Th8 without spinal cord injury. The patient underwent one-stage anterior and posterior decompression. Posterior surgery included Th4 to Th9 laminectomy and Th7-Th8 facetectomy and posterior bone graft. Anterior procedure included Th8 vertebrectomy, anterior interbody bone graft and anterior plating with usage of "Zespol" plate and screws anchored in Th6, Th7, Th8 and Th9 vertebral bodies. Postoperative period was uneventful and the patient remained neurologically intact. He was placed in a plaster jacket and mobilized of fifth day after surgery.

[The physiopathology of acute spinal cord injury and a hope for a successful treatment].

The acute phase of spinal cord injury includes primary and secondary pathological patterns. Primary patterns include the effects of contusion, laceration, stretch of neural tissue and direct vascular trauma. These changes are irreversible. Secondary patterns include posttraumatic ischemic changes, loss of energy metabolism, oedema, release of cytotoxic substances such as free radicals, and electrolyte changes such as an increase in intracellular calcium ions. These changes may be reversible. This determines treatment strategies. Free-radical scavengers, opioid receptor antagonists include TRH and its analogues, calcium channel blockers, volume expander, osmotic diuretics, hypothermia, antioxidants, cycloxygenase inhibitors, serotonin antagonists and NMDA receptor antagonists were tested in experimental models during the last 4 years. The successful treatment should break the feedback loops and trails of secondary injury cascade in many places so combined treatment connected with many elements and surgery decompression is necessary.

[Results of surgical treatment of patients after spinal trauma with partial spinal cord damage].

Treatment of spinal injuries is a special line in the Dept. of Neurosurgery of Military Academy in Lódź. The authors reviewed the results of surgical treatment of cervical spine injuries in the Dept. between 1982-1987. Follow-up was performed in a group of 49 cases, who vosited the Dept. between 5 to 1 year after surgery. This group of patients underwent detailed clinical analysis in terms of long-term results of treatment. Radiological results were assessed as well. The authors analyzed the relationship between timing of surgical decompression of the spinal cord and clinical improvement of neurological status. The concluded that early decompression in case of incomplete spinal cord injury resulted in better neurological outcome.

[Embolization of AVM's of thoracic spinal cord with histoacryl glue].

The authors present two cases of inoperable AVM's of thoracic spinal cord successfully treated by embolization with histoacryl glue (B. Brown Melsungen AG). The glue used for embolization is characterized by instant polymerization when comes in contact with blood. A mass of polymer is visible on X-ray thanks to contrast medium Lipiodol and metallic powder Tungsten that are added to glue and injected together with the latter. In the first case, a female who presented with paraparesis and walked only with assistance embolization performed in one session resulted in neurological improvement enabling independent walking at follow-up 2 yrs later. In the second case of a young male not walking for severe paraparesis embolization performed in three sessions resulted in significant neurological recovery. 10 days after the first session the patient became ambulatory. 6 months after treatment he presented with mild paraparesis and was still ambulatory and leading independent life. In each case embolization was performed after balloon occlusion test during which the function of spinal cord was monitored by somatosensory evoked potentials and neurological assessment.

[Profiles of daily secretion of ACTH, beta-endorphins and cortisol in patients after minor craniocerebral trauma].

In 12 men aged 20-25 men who had minor craniocerebral trauma the profiles of daily secretion of ACTH, beta-endorphin and cortisol were studied. The studies were carried out on the second day after the trauma. Plasma concentrations of the hormones were determined on 8.00, 12.00, 17.00 and 22.00 hours. Statistically significantly increased, in relation to healthy persons, (p < 0.01) values of plasma hormone concentration were found at all time points. In 50% of the patients (six cases) a disturbance was found of the profile of daily secretion of ACTH and beta-endorphin. The disturbance of the profile of cortisol secretion was found only in one patient.

[Diagnostic difficulties in a case of multifocal changes in the brain in computerized tomography and in magnetic resonance imaging].

This paper presents a case of multifocal metastases in the brain, which on the basis of typical imaging magnetic resonance and computerized tomography suggested possible parasitic changes (cerebral cysticercosis). On the basis of these pictures cerebral cysticercosis was diagnosed, in spite in the absence of serologic changes in blood and cerebrospinal fluid. The lack of improvement after anticysticercosis drugs, the further course of the disease as well as new foci located in other organs outside the brain, allowed to recognize numerous metastases in the brain although until the end of the clinical observation the primary lesion of the neoplasm was not found. The histopathological examination of samples of brain tissue confirmed the diagnosis of numerous metastases of poorly differentiated carcinoma.
