[The use of FSH in adolescents and young adults with idiopathic, unilateral, left varicocele not undergoing surgical intervention. Preliminary study].

BACKGROUND: To study gonadal function in young patients suffering from left varicocele. METHODS DESIGN: prospective study. SETTING: university laboratory and clinical services. PATIENTS: 20 boys (age range: 15-20 yrs; mean +/- DS: 17.9 +/- 1.48 yrs), affected by left idiopathic varicocele. INTERVENTION: high purified urinary FSH s.c. (Metrodin HP 75, Ares-Serono), three times every week, for three months. MAIN OUTCOME MEASURES: seminal fluid examination before and after therapy (WHO). RESULTS: At the end of the study period a statistically increase of sperm density/ml (38.45 x 10(6) vs 49.10 x 10(6), total sperm number/ejaculate with forward motility (37.41 x 10(6) vs 48.52 x 10(6)) and a decrease of atypical forms (54.35% vs 49.60%) has been observed; the increase of the total sperm number/ejaculate after FSH treatment greater than 50% compared with basal values allows to define 9 patients as responders. CONCLUSIONS: In this study the FSH treatment in young people with varicocele leads to a statistically significant increase in the seminal fluid parameters and may characterize a group of patients who may have a better prognostic outcome regarding the tubular gonadal function and thus the fertility potential in adult life.

Gram staining applied to human spermatozoa: a simple method for studying chromatin condensation status.

Gram staining applied to human spermatozoa from fertile donors is described. The stain revealed populations of Gram-positive and Gram-negative spermatozoa. Data showed a significant and progressive decrease in the percentage of Gram-positive spermatozoa at different times during the chromatin decondensation procedure (SDS-BSA and SDS-EDTA). No significant correlation could be found between Gram staining and other functional tests used for spermatozoa; only the aniline blue staining test showed a poor correlation. Our study demonstrates that normal spermatozoa with regular chromatin condensation appear Gram-positive, while spermatozoa with altered chromatin condensation appear Gram-negative.

Immunoglobulins in human seminal plasma.

Single radial immunodiffusion has been used to evaluate immunoglobulins and secretory piece (SP) in human seminal plasma. Samples were collected from 90 healthy volunteers, 202 subjects submitted to vasectomy for contraceptive purposes, tested at various intervals after surgery, and 725 patients grouped according to selected andrological disorders. Results may be summarized as follows. In normal subjects IgG and IgA were constantly present (mean values +/- SD: 8.14 +/- 2.82 mg/dl; 1.91 +/- 1.03 mg/dl, respectively) while IgM were detected in trace amounts (from 0.7 to 3.3 mg/dl) in 10% of subjects, and negligible or absent in the remaining subjects. In vasectomized subjects IgG and IgA showed a significant increase only in the first 3 months after vasectomy, probably due to surgery. In andrological patients Ig showed an increase in cases with antisperm antibodies (A b) and in those with infections of genital tract and positive semen culture. On the basis of these findings and the secretory piece assay data the importance appears to be stressed of the local immunocompetent system at least in some andrological diseases.

Structural variants of chromosome 9: a possible association with hypogonadotropic hypogonadism.

We report two cases of structural variations of chromosome 9 associated with hypogonadotropic hypogonadism and azoospermia in adolescent boys. One patient also had a partially imperforated urethral meatus. Histological examination revealed that both had hypotrophic and underdeveloped testes. There was no LH and FSH response to LH-RH stimulation nor was there any response to naloxone tests. Basal and HCG stimulated plasma testosterone values were below normal prepubertal levels. As the administration of gonadotrophins did not improve the clinical and hormonal findings, alternative androgen therapy was necessary to achieve secondary sexual characteristics. Although they reached a good level of androgenization, their testes were still very small and azoospermia remained, as confirmed by repeated semen analyses. A possible association between chromosome 9 polymorphisms and hypothalamo-pituitary axis abnormalities is suggested. It is hypothesized that structural variants of chromosome 9 are not unrelated occurrences. Furthermore, and in view of the fact that they can lead to a high risk of azoospermia and infertility, such variants call for clinical investigation.

Recombinant-growth hormone (rec-hGH) therapy in infertile men with idiopathic oligozoospermia.

The study was designed to be an open test for short-term efficacy of rec hGH-therapy on spermatogenesis of ten infertile men with idiopathic severe oligozoospermia (1-5 x 10(6)) and normogonadotropinemia or moderate ipergonadotropinemia (FSH: range 1.90-9.60 IU/L, mean +/- SD 5.78 +/- 2.84; LH: 2.70-12.70 IU/L, mean +/- SD 6.60 +/- 3.61). FSH, LH, Testosterone, GH, and IGF-I were evaluated. Seminal parameters: sperm concentration, sperm motility and morphology were studied before and after therapy. Five responder patients showed an increase of seminal parameters both in sperm concentration and total number of motile spermatozoa. The short-term rec-hGH therapy significantly increased sperm concentration and total motile spermatozoa in five out of ten infertile patients who didn't show any modification in seminal parameters to other preavious treatment.

Evaluation of the morphological and functional damage to human sperm subjected to freezing at -196 degrees C and to refrigeration at +4 degrees C.

A study was carried out on five healthy, fertile donors to evaluate refrigeration at +4 C compared to cryopreservation at -196 degrees C. These donors had produced more than two pregnancies in different women with their cryopreserved semen in an AID program. The following parameters for evaluation and comparison were used: (i) the percentage of forward sperm motility, (ii) the percentage of swollen sperm after hypoosmotic stress (swelling test) and (iii) the sperm morphology observed both with a light microscope after staining and with an electron microscope. After 48 hours of refrigeration the result obtained were comparable with those observed after one week of cryopreservation. After 72 hours of refrigeration, a sharp and significant decrease of these values was noted. Our data underlined the fact that there is an individual variability in subject response to the method of preservation employed. Our findings show the possibility of using sperm refrigerated for up to 48 hours in AIH programs.
