Acute myeloid leukemia presenting as splenic rupture.

We describe a rare case of acute myeloid leukemia presenting primarily as an acute abdomen due to spontaneous splenic rupture in a 19 years male patient. He was treated with splenectomy after failure of conservative management for splenic preservation but later succumbed to an intracerebral haemorrhage.

Ingested needles in a 3-month-old infant.

This case describes a 3-month-old infant who swallowed multiple straight sewing needles that were removed at laparotomy. A motherhood psychiatric disorder was suspected.

A role for ectophosphatase in xenobiotic resistance.

Xenobiotic resistance in animals, plants, yeast, and bacteria is known to involve ATP binding cassette transporters that efflux invading toxins. We present data from yeast and a higher plant indicating that xenobiotic resistance also involves extracellular ATP degradation. Transgenic upregulation of ecto-ATPase alone confers resistance to organisms that have had no previous exposure to toxins. Similarly, cells that are deficient in extracellular ATPase activity are more sensitive to xenobiotics. On the basis of these and other supporting data, we hypothesize that the hydrolysis of extracellular ATP by phosphatases and ATPases may be necessary for the resistance conferred by P-glycoprotein.

13C-NMR: a simple yet comprehensive method for analysis of intermediary metabolism.

13C-NMR is a particularly attractive tool for metabolic studies because of its inherent simplicity: all labeled products at sufficient concentration may be identified and analysed in a single spectrum. However, the real power behind the approach presented here is the ability to measure groups of individual 13C-isotopomers (isotope isomers). The information that this provides is superior to conventional tracer techniques, allowing a very detailed description of metabolic events. Several examples are given of the value of this convenient and straightforward analysis for some problems of current interest in intermediary metabolism.

Multiple synovial swellings in BT leprosy. A case report.

A case of Leprosy with multiple synovial swellings has been reported. Rarely these swellings may be an initial presentation and at sites other than dorsum of hand or ankle as was in this case. The literature on the subject is briefly reviewed.

Propionate metabolism in the rat heart by 13C n.m.r. spectroscopy.

High-resolution 13C n.m.r. spectroscopy has been used to examine propionate metabolism in the perfused rat heart. A number of tricarboxylic acid (TCA) cycle intermediates are observable by 13C n.m.r. in hearts perfused with mixtures of pyruvate and propionate. When the enriched 13C-labelled nucleus originates with pyruvate, the resonances of the intermediates appear as multiplets due to formation of multiply-enriched 13C-labelled isotopomers, whereas when the 13C-labelled nucleus originates with propionate, these same intermediates appear as singlets in the 13C spectrum since entry of propionate into the TCA cycle occurs via succinyl-CoA. An analysis of the isotopomer populations in hearts perfused with [3-13C]pyruvate plus unlabelled propionate indicates that about 27% of the total pyruvate pool available to the heart is derived directly from unlabelled propionate. This was substantiated by perfusing a heart for 2 h with [3-13C]propionate as the only available exogenous substrate. Under these conditions, all of the propionate consumed by the heart, as measured by conventional chemical analysis, ultimately entered the oxidative pathway as [2-13C] or [3-13C]pyruvate. This is consistent with entry of propionate into the TCA cycle intermediate pools as succinyl-CoA and concomitant disposal of malate to pyruvate via the malic enzyme. 13C resonances arising from enriched methylmalonate and propionylcarnitine are also detected in hearts perfused with [3-13C] or [1-13C]propionate which suggests that 13C n.m.r. may be useful as a non-invasive probe in vivo of metabolic abnormalities involving the propionate pathway, such as methylmalonic aciduria or propionic acidaemia.

Hepatic abscess complicating paratyphoid infection.

Salmonellosis is ubiquitous and is a world-wide public health concern. Liver abscesses are occasionally reported in Salmonella typhi infections, they are a very rare complication of Salmonella paratyphi infections. A 28 year old male patient without any prior medical history presented with fever, abdominal pain and a tender hepatomegaly. The imaging studies revealed multiple liver abscesses and an ultrasound (US) guided aspiration of the abscess yielded heavy growth of Salmonella paratyphi A. He was treated successfully by percutaneous drainage of the abscesses and appropriate antibiotics.
