Analysis of adenomatous polyposis coli gene in thyroid tumours.

Familial adenomatous polyposis (FAP) is known to be associated with neoplasia of various tissues, including thyroid carcinoma. Germline mutations of the tumour-suppressor gene APC, responsible for the predisposition to FAP, may therefore be involved in the pathogenesis of these tumours. In this report the structure of the APC gene has been investigated in 26 thyroid tumours, at different stages of dedifferentiation, that were surgically excised from patients with a negative history of FAP. Approximately 35% of the APC gene coding region, where most of the mutations are clustered, has been analysed by a combination of single-strand conformation polymorphism and direct sequencing. No significant alterations could be demonstrated in any sample examined. It is concluded that, at least in patients not affected by FAP, APC gene abnormalities do not seem to play a relevant role in the pathogenesis of thyroid carcinoma.

[Nerve-sparing retroperitoneal lymph node dissection in patients with non-seminomatous testicular tumors at clinical stage 1].

Retroperitoneal lymph node dissection (RPLND) for low stage testicular cancer involves a low rate of dry ejaculation. The nerve-sparing technique avoids the damage of sympathetic fibers. 7 cases of non seminomatous testicular cancer have been treated by the Author. Through midline incision and intestinal derotation the vascular plane and the sympathetic fiber are identified before starting lymph node dissection. All the patients report physiological ejaculation; the post-operative follow-up is still short but at the present all patients are tumor free. In low stage testicular cancer nerve-sparing lymphadenectomy preserve physiological ejaculation and reducing surgical morbidity as well.

[Conservative therapy of parenchymal renal carcinoma].

From January 1987 to October 1990, 20 patients (16 M. 4 F.) affect by localized renal cell carcinoma, underwent a conservative surgical treatment. This case study includes 8 patients with bilateral (4 synchronous-4 asynchronous), and 12 patients with unilateral renal carcinoma. In the latter group only two patients had a pathological contralateral kidney. A parenchyma sparing operation was performed electively in 10 cases and was necessary in the other 10. The tumors were found to be PT1 or pT2 in all the elective cases, except one unexpected pT3a; 1pT1, 5pT2, 1pT3a and 3pT3b in the remaining group. The follow up ranges between 1 and 36 months, two deaths in bilateral tumors cases were observed. No local recurrences have been detected up to now.

[Neoadjuvant chemotherapy of infiltrating carcinoma of the bladder: our experience].

From January 1987 until October 1989, 96 patients have been treated with radical cystectomy for locally advanced bladder cancer. Nineteen, out of 96 underwent chemotherapy (either M-VAC or M-VEC) before cystectomy. Five had T1G3 bladder tumor. After chemotherapy the results had been the following: 5 complete responses (CR) 26%, 7 partial responses (PR) (37%) and no changes in 7 patients (37%). During the first two courses of chemotherapy, 36 administrations out of 152 scheduled should have not been done because of drug toxicity. The seven patients who had no changes had a drug administration reduced of 41% with respect of a rate of reduction of 12% in the group of patients who had complete response. The urinary diversion after cystectomy were performed with a neobladder according to Reddy in 4 patients, with a colon conduit in 1 patient and with an ileal conduit in 12 patients. All the patients survived the surgery. We did not notice any difference in the follow up after surgery in those patients treated before cystectomy with chemotherapy with respect to the patients who underwent cystectomy without chemotherapy. Despite the fact that by now we can not state any conclusion about the efficacy of neoadiuvant chemotherapy, due both to the short follow up and to the small number of patients treated, we should however perform cystectomy in all the patients after chemotherapy because up to now we have no evidence that a conservative treatment of the bladder after chemotherapy could be safe.

Potassium-Dependent Increase in RNA and Protein Synthesis in the Early Phase of Incubation of the Thermodormant Phacelia tanacetifolia Seeds.

The seeds of Phacelia tanacetifolia Benth. cv Bleu Clair incubated at 30 degrees C in the dark did not germinate and did not activate K(+) uptake capacity. The administration of 1 millimolar K(+) in the early phase of incubation stimulated RNA and protein synthesis. The possible role of K(+) in promoting the marcromolecular syntheses during the early phase of germination is discussed.

[The specific erythrocyte adherence method in the study of ABH antigens on normal and neoplastic bladder epithelium].

The isoantigens A, B, and H, identical with those present in erythrocytes, are also present in epithelial cells of some normal tissues, included the transitional epithelium of the urinary bladder. The cell surface ABH antigens can be detected with the specific erythrocyte adherence method (SEA), but could not be detected when carcinoma developed in this tissue: with rare exceptions, the loss of demonstrable antigens paralleled morphological anaplasia. We studied retrospectively by SEA test 51 specimens of urinary bladder, including 36 primary transitional cell carcinomas at the time of initial biopsy, and 15 controls. The SEA reaction was strongly positive in normal bladder epithelium, in vascular endothelium and in erythrocytes already present in sections, while was negative in connective tissue. 21 were classified as grade 0 (papilloma): in 19 of them the ABH antigens were fully or partially preserved, and the follow up from 5 to 15 years demonstrated no recurrences; the other 2 patients with negative SEA recurred locally. The only one graded as I, with strongly positive SEA, had not recurred in at least 6 years, 5 were classified as grade II; in 2 of graded as III, the SEA reaction was weakly positive in 3, while in remaining 6 the antigens were absent. The usefulness of the detection of ABH antigens as a prognostic tool in low grade and stage bladder carcinomas is confirmed by our data establishing the correlation of antigens deletion with subsequent clinical behavior. An alteration of blood group substance synthesis occurs in malignant epithelium, but it is not known whether it causes or results from malignant changes. The SEA method resulted highly sensitive and specific; however, the test is cumbersome and time-consuming, requires special expertise and shows some limitations, particularly the frequency of the occurrence of false-negative or weakly positive results in blood group O patients.

Nucleolar protein p120 expression in oral carcinoma.

Nucleolar protein p120 is a proliferation-associated antigen expressed by cells in early G1 phase, identified by the monoclonal antibody FB-2. Its expression has been evaluated in breast and prostate cancer, and proved to be significantly correlated with other prognostic parameters. In oral pathology, p120 protein content is able to distinguish between non-neoplastic and malignant lesions. In the present study, the immunohistochemical expression of p120 protein was evaluated in fifty cases of oral squamous carcinoma and compared with histological grading, pTNM staging, DNA ploidy status and follow-up of the patients, in order to establish its prognostic value, p120 mean area was significantly correlated to all these parameters, apart from lymph node involvement, indicating the strong predictive potential of this marker. In conclusion, quantitative immunohistochemical analysis of p120 protein represents an easy and reliable method for the assessment of clinical outcome and the definition of risk groups in oral carcinoma.

[Fine needle aspiration in the cytologic diagnosis of swellings in the parotid and submandibular regions. Cytologic-histologic correlations in 49 cases].

Fine needle aspiration biopsy (FNAB) has been widely used for a number of years and offers considerable advantages in comparison to conventional surgical biopsy. A number of organs can be subject to this diagnostic procedure with good results, including the salivary glands. This paper aimed to compare the cytological diagnosis of biopsy material obtained using FNAB and the subsequent histological diagnosis formulated after surgery in 49 patients with swellings in the parotid or submandibular region who were referred to the Department of Anatomy and Pathological Histology in L'Aquila. Cytohistological concordance was present in 87.7% of the cases in this series, a percentage that is in line with that reported by other authors. The most frequent causes of diagnostic error may be ascribed to moderate nodule size and/or the scanty material obtained not suitable for diagnosis. In this series all cases of malignant neoplasia were correctly diagnosed. In spite of the limitations of this method, the authors consider that it plays an important role in the preoperative diagnosis of salivary gland pathologies.
