Combined use of Orgaran and Reopro during coronary angioplasty in patients unable to receive heparin.

Orgaran, a heparinoid, has been used successfully in patients with heparin-induced thrombocytopenia. We report three cases in which Orgaran was combined with the glycoprotein IIbIIa receptor antagonist Reopro during coronary angioplasty. Orgaran was given as a single intravenous bolus of 1500 anti-factor Xa units. No ischemic or hemorrhagic complications occurred during or following the procedure.

Basic pharmacology and pharmacokinetics.

The processes of absorption, distribution, metabolism, and elimination are involved in the determination of blood drug concentrations from a certain dosage regimen. Various physiologic factors ranging from gastric pH to hepatic blood flow can affect the pharmacokinetic parameters that help to describe these processes. The result of altered physiology may be a change in drug concentrations that may cause toxic effects or no desired effect. Therefore, the clinician must have a knowledge of basic pharmacokinetics and pharmacodynamics to be able to predict the behavior of drugs in a body and ensure that the drugs are used in a safe and effective way.

Impact of a pharmacist education program on nonformulary drug use.

The effect of an educational program for pharmacists that included instructions for handling nonformulary drug requests and determining suitable formulary alternatives for frequently prescribed nonformulary drug requests was measured. Results indicate a significant increase in the number of appropriate nonformulary drug dispensings when the postintervention phase was compared with the preintervention phase. Other values, including the financial impact per nonformulary drug request, did not differ significantly between the two phases. Since all nonformulary drug requests require pharmacist time, a more cost-effective nonformulary drug policy might focus the pharmacist's efforts on nonformulary drug products that are more costly than their formulary alternatives.
