[Contribution of digital intravenous angiography in the assessment of the postoperative results after surgery of the vessels of the neck].

Forty-six patients who had undergone neck vessel surgery were investigated postoperatively by intravenous digitalised angiography. In 30 patients an endarterectomy had been performed and in one case the examination did not provide satisfactory opacification of the carotid vessels. In 29 cases the result was of satisfactory quality: the appearance of the operative site was normal in 21 patients, dilated in 3, irregular in 7 and stenosed in 1. Twenty patients had undergone a bypass. There was one failure and 19 results suitable for analysis. The bypass was normal in appearance and permeable in 14 cases, irregular in 1, dilated in 2 and obstructed in 2. Overall, intravenous digitalised angiography provided satisfactory assessment of the operative result in 96% of patients.

[Current indications for intravenous numerical angiography in the study of the ilio-femoral axis].

A study of the arterial axis from the aortic bifurcation to the femoral bifurcation was conducted by intravenous digital angiography in 40 patients. In 92.5% of these, opacification and visualization of the arteries were good or excellent. In 12 out of 15 patients the images were judged to be of the same quality as those obtained by conventional arteriography. A limitation of the method is the size of the intensifying screen (23 cm), which only allows exploration of one segment at each injection. This, however, is a minor drawback when this technique is used for control examination after percutaneous angioplasty or to investigate an accident of catheterization.

[Percutaneous transluminal angioplasty of the iliac arteries. Immediate and long-term results].

Percutaneous transluminal angioplasty (PTA) was performed 30 times in 28 patients aged from 31 to 77 years (mean: 50.8 years) with stenosis of the iliac arteries. All patients were evaluated before and immediately after PTA by measuring the trans-stenotic pressure gradient and the degree of angiographic stenosis (grade 1 less than or equal to 50%; grade 2 = 50 to less than 75% and grade 3 greater than or equal to 75%). Prior to PTA 27 patients had grade 3 and three patients had grade 2 stenosis. The figures after PTA were 23 grade 1 and seven grade 2 stenosis. The mean trans-stenotic pressure gradient was reduced from 43 +/- 35 to 2.3 +/- 5.9 mmHg, the difference being highly significant (p less than 0.001). Eighteen patients were evaluated clinically and angiographically 8.4 +/- 6.1 months after PTA. Considerable clinical improvement was observed in 15, and 16 had a residual stenosis of less than 50%. PTA therefore appears to be a satisfactory treatment of stenosis of the iliac arteries. The long-term results can be assessed by graded intravenous angiography without any risk of damage to the arteries.

Digital intravenous subtraction angiography of intracranial arteriovenous malformations.

Twenty-one intracranial arteriovenous malformations (17 pial, 3 dural, 1 mixed) have been studied by digital intravenous subtraction angiography (DIVA). A comparison with conventional arteriography has been performed. In this series DIVA was 100% diagnostic for AVM. However DIVA missed some small arterial pedicles (1 lenticulostriate, 1 thalamoperforating, 1 anterior choroidal artery and 2 pharyngeal arteries). Sectorial analysis of AVM needs selective arteriography. DIVA is a good method for screening and follow-up of intracranial AVM, but selective arteriography is often still necessary to complete the pre-operative evaluation.

Percutaneous transluminal angioplasty of stenotic lesions in dialysis vascular accesses.

Forty-five stenoses behind arteriovenous fistulas in 35' haemodialysed patients were treated with percutaneous transluminal angioplasty (PTA). Immediate stenoses dilatation results in relation to the AVF type were haemodynamically significant in 40 cases (88%), with poorer results on bovine heterograft stenoses. Dilatation results in relation to the site of the lesion showed haemodynamically efficient angioplasty on stenoses next to the anastomosis (67.5%) but a poor result on distal lesions (4 cases). We observed 4 cases of thrombosis but 2 recovered after treatment, one after local fibrinolytic perfusion one after surgical thrombectomy. Follow-up over a period of one to 18 months (mean 8 months) including angiography and appreciation of the dialysis quality were performed in 30 patients who had a successful dilatation. PTA appears to be a technically feasible and clinically effective method of treating stenoses lying on the venous limb fistula in patients on chronic haemodialysis.

[Intravenous subtraction angiography in kidney transplant patients].

Conventional arteriography in patients with kidney transplants and severe hypertension is performed principally to determine the presence of a renal artery stenosis. In patients with renal failure its use enables detection of any arterial lesions. Technological advantages provided by computerized angiography within the field of medical imaging should permit earlier diagnosis of arterial lesions, with a reduced risk and at a lower cost using a simple intravenous injection of the iodized contrast medium.

[Angiography and perfusion pulmonary scintigraphy in pulmonary embolism].

Clinical signs suggestive of pulmonary embolism require diagnostic confirmation from complementary scintigraphic and/or angiographic examinations, therapy being adapted to the results obtained. A comparative study of the value of the two techniques was conducted in 20 patients with suspected pulmonary embolism investigated by a perfusion pulmonary scan and selective pulmonary angiography. Results confirmed the diagnostic superiority of angiography, the high percentage of false positives obtained by perfusion scintigraphy alone indicating the need for associated ventilatory function tests.

[Teratogenic effects of 9-hydroxy-ellipticine in mouse embryo].

Administered to pregnant Mice of Swiss strain, at stages of 9 to 12 days of gestation and at doses of 15 to 50 mg/kg, 9-hydroxy-ellipticine acts as an acute teratogen; it induces in embryos, cleft palate and limb deformities. Histological examination of embryos from mothers treated by a dose of 30 mg/kg at day 10 or 11 revealed defects in the eyes, spinal cord and a reduction in the number of germinal cells.

[Hepatitis caused by Campylobacter coli (author's transl)].

Campylobacter coli is known to cause ulcerous enterocolitis, but hepatitis has not yet been reported. A 50-year-old woman without history of liver disease was admitted with diarrhoea, fever, poor general condition, subicterus and enlarged liver. Campylobacter coli was grown in haemoculture, and a specific antibiotic treatment resulted in complete cure. The results of haemoculture, the necrosis and polymorphonuclear infiltrates found in liver biopsies, the return to normal of biochemical tests and liver size under antibiotic therapy and the absence of any other cause of acute or chronic liver disease are strong arguments in favour of the hepatitis being caused by Campylobacter coli in this patient.

The value of a portal study in determining the etiology of hepatic masses in the adult.

The intrahepatic portal system was studied by superior mesenteric angiography in 80 patients with hepatic masses. The portal study helped to identify the highly significant portal signs of malignancy (amputations, irregular stenosis, portal hypertension), and, among the malignant tumors, the specific signs of hepatoma (portal trunk amputation, ill-defined hepatographic lacunae, association of portal hypertension with proximal portal amputation). It is concluded that the intrahepatic portal study is essential in pretherapeutic staging. It is better than superselective hepatic arteriography in assessing the involvement of one hepatic lobe when a hypervascular tumor involves the other lobe, and it is indispensable if embolization is planned.

[Effects of cytosine-arabinofuranoside on the development of reptilian embryos (Lacerta viridis, Laur. and Anguis fragilis, L.)].

Administered into the yolk sac of eggs of Lacerta viridis as a single dose of 17 to 40 micrograms, cytosine-arabinoside (Ara-C) was compatible with survival of the embryo, from the sixth day of incubation, for at least 20 to 25 days. The LD50 was 40 to 50 micrograms per egg. Doses of 20 to 40 micrograms of Ara-C introduced in the yolk sac of eggs of the slow-worm (Anguis fragilis) cultured in vitro, at stages of the allantoid bud of 0,5 mm to 2,5 mm long, killed the embryo in 4 to 8 days (possibly due to alterations of capillary blood vessels of allantois and area vasculosa). In the two species, these doses caused cytotoxic effects on embryonic proliferating tissues, growth inhibition and a variety of developmental defects. In young embryos of Anguis fragilis, similar doses of 20 to 40 micrograms of Ara-C caused, in 2 to 4 days, death of many cells in the anlagen of growing organs: neural tube, sensory organs, bronchi, mesoderm of the limb bud, subcutaneous mesenchyme, anlage of dorsal skeletal structures, etc.; followed by growth inhibition and malformations. On the other hand, in the limb bud, the apical ridge was less retrogressed than in control embryos; the limb buds showed slightly better development in treated embryos than in controls, but, Ara-C induced severe damage in their mesoderm. In all embryos of Lacerta viridis, treated at the stage of 6 days or of 10 days of incubation by doses of 20 to 40 micrograms of Ara-C and killed 15 to 35 days later, there was a general reduction of size and of weight and external and internal malformations, more or less severe, were present: modifications of the form of the head, shortening of the lower jaw, labial clefts, microphthalmia, micromelia and other limbs defects, developmental defects of the tail. In some embryos, the only external defects observed were missing fingers and toes; in three of these embryos, the same digits were missing in the four limbs. Modifications of limb morphogenesis induced by Ara-C are compared to structural modifications of the limbs of snake-like Reptilia, and the mechanisms involved in the two series are discussed. These results emphasize the interest of the use of drugs interfering with DNA synthesis, in the field of teratology and in the experimental study of regressive evolution.
