Fetal fibronectin as a predictor of preterm birth: an overview.

The objective of this study was to determine if cervicovaginal fibronectin is predictive of preterm birth. Articles involving human subjects published in English between January 1976 and December 1997 were retrieved from MEDLINE using the keywords: fibronectin, fetal fibronectin, oncofetal fibronectin, preterm, PROM, preterm birth, and preterm labor. We included studies that were prospective, enrolled women at less than 37 weeks' gestation and blinded caregivers to the fetal fibronectin test results. Of the 24 studies meeting the inclusion criteria, 15 included symptomatic women; 8 included asymptomatic women; and 1 study included both. Data were abstracted independently by two authors and a meta-analysis of results of test characteristics, using summary values, was undertaken where possible. For symptomatic women, the sensitivity for delivery within 7 to 10 days of sampling was excellent (summary value [95% confidence interval ¿CI¿]: 98% [95%, 100%]). For delivery within 14 days, the sensitivity was somewhat lower (summary value [95% CI]: 82% [74%, 90%]) and lower still for delivery within 21 days (summary value [95% CI]: 73% [67%, 80%]). For delivery less than 34 weeks the sensitivity was good, but poor for delivery less than 37 weeks (summary value [95% CI]: 87% [81%, 94%], 54% [51%, 58%] respectively). For asymptomatic women, the sensitivity of fetal fibronectin for delivery less than 34 and less than 37 weeks was poor (summary value [95% CI]: 43% [36%, 50%] and 64%[57%, 71%] respectively). The specificity of fetal fibronectin for symptomatic and asymptomatic women was over 80% for all outcomes. For symptomatic women, a negative test for fetal fibronectin should be useful at ruling out the likelihood of delivery within 7 to 10 days of sampling. For asymptomatic women, testing for fetal fibronectin is unlikely to be useful as many women at risk will be misclassified as normal.

Failed trial of vacuum or forceps--maternal and fetal outcome.

OBJECTIVE: Our purpose was to compare the maternal and neonatal morbidity associated with a failed trial of instrumental delivery with that of proceeding directly to cesarean section during the second stage of labor. STUDY DESIGN: All second-stage cesarean deliveries between January 1986 and December 1992 in a tertiary care teaching hospital were retrospectively reviewed. Specific maternal and neonatal outcome parameters were studied to compare the failed instrumental group with the direct-to-cesarean section group. RESULTS: Of 29,457 live births at > 37 weeks' gestation, 401 women had a cesarean section performed in the second stage of labor. There were 326 cases in which cesarean section was performed directly during the second stage of labor and 75 women who had a failed attempt of instrumental delivery (forceps 33, vacuum 25, both 17) before cesarean delivery was done. The three instrumental groups and the direct-to-cesarean section group did not differ in any of the outcome variables for either mother or newborn. CONCLUSIONS: Failed instrumental delivery performed as a trial of forceps and/or vacuum in a setting where a cesarean section can follow promptly is not associated with increased morbidity of either mother or baby.

The tilted posterior tooth. Part III: Abutment for a fixed partial denture.

The problem of achieving a common path of insertion for a fixed partial denture when a tilted posterior abutment is involved can usually be solved by well planned tooth preparation in conjunction at times with intentional endodontic therapy. When tooth preparation alone cannot solve the problem, the mechanical solutions of the locked attachment and the telescopic retainer are available and must be considered. Other problems involved with the tilted abutment, adjacent tilted teeth, space reduction, supraversion of the antagonist, and the problem of a long-span fixed partial denture were discussed.

Repairing a crown and sleeve-coping prosthesis.

A crown and sleeve-coping prosthesis retainer has been defined. Some causes for the loss of the coping and subsequential clinical manifestations have been discussed. A step-by-step procedure for remaking the coping was presented.

Finish lines in fixed prosthodontics.

Ideally, all finish lines should be placed supragingivally. Due to esthetic and carious considerations, however, subgingival placement of the finish line is preferred. No one type of finish line can be used for all crown preparations. In lower anterior teeth or periodontically-treated teeth, the knife-edge finish line appears to be the treatment of choice. In short teeth and preparations for porcelain and porcelain-gold crowns, the full shoulder bevelled preparation is the treatment of choice. In full gold and acrylic veneered gold crowns, as in endodontically-treated teeth, the chamfer preparation is the treatment of choice.

Reflectance spectra of natural and acrylic resin teeth.

A method has been described for determining the reflectance spectra of teeth at various parts of the tooth surface. The shape and size of the teeth were accounted for by appropriate experimental and mathematical techniques. In all instances where a subtle difference in tooth color was discernible, this difference was also apparent on the corresponding spectra. The spectra of acrylic resin teeth showed characteristic absorption shoulders caused by pigmentation. These shoulders did not appear in the spectra of natural teeth where the reflectivity gradually increased with increasing wavelength. Differences were observed in the colors as well as the spectra of teeth from different shade guides which were marked with the same shade number by the manufacturer. The method may be applied for control of the color quality of artificial teeth as well as the selection of pigments in order to obtain an appropriate shade range and to reduce effects of metamerism.
