Behavior of calciotropic hormones in psoriasis.

This report was motivated by the finding that calciotropic hormones, except parathormone (PTH), do not show any response to 1a(OH)D3 therapy in psoriatic patients. These showed high basal whole PTH levels (127 +/- 38 pg/ml) which declined to 45 +/- 15 pg/ml after treatment (p less than 0.001) but did not correlate to calcemia values. Basal calcemia and calciuria were normal and rose with treatment (p less than 0.001 for both). Basal calcitonin and 25(OH)D3 were normal and did not change with treatment nor correlate to changes in calcemia. Due to the effect of 1-hydroxyvitamin D and calcemia on calcitonin and 25(OH)D, the lack of changes in the latter suggests a calciotropic hormone imbalance in psoriasis, which may be an etiologic factor in this entity.

Acetylator phenotype in psoriasis.

Acetylator phenotype has been determined with sulfamethazine in 64 psoriatic patients and in 157 normal control subjects. Forty patients (62.5%) versus 90 control subjects (57.3%) were slow acetylators (p = NS). However, 81% of the 27 patients with psoriatic siblings were slow acetylators (p less than 0.05). Slow acetylator phenotype may be a genetic risk factor for the development of psoriasis.

[Adrenal gland masses and Addison's disease of tuberculous origin: apropos of 2 cases].

Two cases of confirmed tuberculous Addison disease, with an increase in size of the adrenal glands are presented. One patient presented also a renal adenocarcinoma; this association is very rare given the small frequency of these diseases. Based on these two cases we emphasize the utility of puncture aspiration with a fine needle (PAFN), echography and TC Scan in the study of Addison disease.

[Wilson Jones angiosarcoma of the face and scalp].

Because of its rarity and its fast development, we report a case of a 84-year-old woman, who developed all the clinical and histopathological characteristics of the angiosarcoma of face and scalp, with a fatal evolution within four months and metastases to liver, lungs and bone marrow.

Acetylator polymorphism in discoid lupus erythematosus.

Acetylator phenotype was determined, using sulphamethazine, in 37 patients with histologically confirmed discoid lupus erythematosus, who were free from visceral damage, and in 157 normal control subjects. Twenty patients (54%) and 90 control subjects (57.4%) were slow acetylators (p not significant). Acetylator polymorphism appears not to be related to the risk of developing pure cutaneous discoid lupus erythematosus.

Verrucous carcinoma of the back.

A case of verrucous carcinoma occurring on the dorsolumbar region of the back is reported. Verrucous carcinoma, although usually appearing on three leading sites (pan-oral, genitogluteal, and plantar), can exceptionally arise anywhere on the skin surface. It is important to be aware of this fact for more accurate diagnosis and treatment.
