[Diagnosis of renovascular hypertension by pre- and post-captopril renal scintigraphy with 99mTc-DMSA].

AIM: Value the utility of studies with 99mTc-DMSA with captopril (scintigraphy imaging and unilateral renal function) in identifying renovascular disease or renal disease in hypertension patients with high probability of renovascular hypertension. METHODS: We studied 27 hypertension patients (12 males, 15 females) mean age 49 years, SD: 9.46, with high probability of renovascular disease. General analysis and radiology was made to all of them including renal arteriography as gold standard. Two renal scintigraphies with 99mTc-DMSA were made to all of them, baseline study and one hour after administration of 50 mg of captopril, a week between both studies. Renal scintigraphy (anterior and posterior images) was obtained 4 hours after the administration of 111 MBq (3 mCi) of 99mTc-DMSA and results were evaluated quantitatively and qualitatively calculating unilateral renal function. We considered positive for renovascular hypertension studies with decrease of more than five percentual points in unilateral renal function or a new defect absent in baseline study. Scintigraphic results were compared with arteriography to determine sensitivity, specificity and predictive values. RESULTS: Arteriography demonstrated significant renal artery stenosis in 18.5% of patients. Renal scintigraphy with captopril using 99mTc-DMSA demonstrated sensitivity of 60%, specificity of 91%, positive predictive value of 60% and negative predictive value of 91%. We identified 3 patients with segmental defects of uptake consistent with chronic pyelonephritis scarring. CONCLUSIONS: Studies with 99mTc-DMSA using captopril present high specificity and are useful in patients with high probability for renovascular hypertension, identifying high number of patients with significant renal artery stenosis. The utility of DMSA in identify renal abnormalities, as specially pyelonephritis scarring, add value to renal studies with captopril using others tracers. DMSA could identify renal etiologies of renovascular hypertension different to renal artery stenosis.

[Limb reduction defects in Asturias (1986-1997): prevalence and clinical presentation].

AIM: To determine the prevalence and clinical presentation of limb reduction defects in Asturias (Spain). PATIENTS AND METHODS: Among the 92300 newborns, stillbirths and induced abortions for congenital defects surveyed by the Registry of Congenital Defects in Asturias (population-based registry) during 1986-1997, we studied the children with limb reduction defects. Frequency is expressed as total prevalence per 10000 births. RESULTS: Seventy-three children with limb reduction defects were identified, with a total prevalence of 8 per 10000 births (CI 6.2-9.8): upper limbs were affected in 65.7%, lower limbs in 23.3% and both upper and lower limbs were affected in 11%. Transverse terminal defects were present in 49.3%, and were intercalary in 16.4%, preaxial longitudinal in 8.2%, postaxial longitudinal in 9.6%, split hand/foot in 9.6% and multiple in 6.8%. Limb deficiencies were found in 52%; of these, 48% were associated with other congenital defects. Four had chromosomal anomalies and 20 were syndromes. 31% had low birth weight for their gestational age. Alcohol exposure during pregnancy was recorded in three mothers. CONCLUSIONS: A detailed study of limb reduction defects could lead to a better understanding of clinical presentation and to an etio-pathogenic diagnosis to control risk factors.

[Blood sampling and transfusion therapy in the preterm infant].

OBJECTIVE: The purpose of this study was to determine the number of skin punctures and volume of blood removed for analytical purposes and the number and volume of red blood cell transfusions performed in premature neonates. PATIENTS AND METHODS: A case-control study was performed with a retrospective review of clinical records. Data about blood samples was gathered from the nursery forms and descriptive statistics were performed. Neonates less than 37 weeks gestational age, born in our hospital between 1/1/1992 and 12/31/1996, were studied. They were divided into two groups: Group 1) Those neonates receiving red blood cell transfusions; and Group 2) The control groups, which was comprised of the next premature baby born after each infant who underwent transfusion. RESULTS: There were 419 premature infants out of 9,635 live newborns (4.3%). Of the 419 premature infants, 123 (29.3%) underwent transfusion therapy. The study groups after exclusions were comprised of 105 infants in groups 1 and 105 infants in the control group, or group 2. The average number of blood samples was 5.4 per infant (range 0-37). The mean volume of blood removed was 26.3 cc per infant (range 0-167 cc) or 15.1 cc/kg (birthweight). The total number of transfusions was 187, with a mean of 1.8 per infant (range 1-12). The mean volume transfused was 43.1 cc per infant (range 20-146 cc). The number of blood samples, total volume of blood removed and percentage of blood volume removed were significantly higher in the transfusion group than in the control group. CONCLUSIONS: The number and volume of blood samples in our premature infants are excessive. We should reduce both to minimize the need of red blood cell transfusions and the associated risks.

Primary colorectal lymphoma.

OBJECT: To analyze our experience in the treatment of primary colorectal lymphomas. PATIENTS: During the recent period from 1989 to 1995, a total number of 406 cases of malignant neoplasms of large intestine were treated at Department of Surgery, Hospital del S.A.S de Jerez de la Frontera. There were 5 cases of primary colorectal Non-Hodgkin's lymphomas, constituting 1.2% of all colorectal malignancies. METHODS: Medical records of all five patients were carefully reviewed. Surgical pathology specimens were retrospectively reexamined to confirm the diagnosis, and immunohistochemistry staining methods were performed. Dawson, Cornes and Morson criteria were used to distinguish primary colonic lymphoma. For tumor staging, a modification of the Ann-Arbor system for gastrointestinal lymphoma proposed by Musshoff and Schmidt-Vollmer was used. RESULTS: All cases were Non-Hodgkin's lymphomas. There were no cases of lymphoma of the rectum. There were 2 males and 3 females. Their ages ranged from 12 to 73 years, with an average age of 31 years. The cecum was the site of the lesions in 4 patients and the other was located in the sigmoid colon. Acute abdomen in three cases, undiagnosed abdominal mass in one and intussusception in the other one were the clinical presentation. Inmunohistochemical studies showed B-cell phenotype in 4 and non-B non-T Ki-1-positive phenotype in 1 patient. Chemotherapy was administered following resection in 100% of patients. CONCLUSIONS: Intraoperative diagnosis of lymphoma was not suspected in all cases but the malignancy was always present. The cecum is the commonest site of involvement. Both of our two patients presented with perforated lymphoma were associated with a poor short term prognosis. It is necessary to unify the staging system and the histologic classifications enabling a comparison of several studies.

[Acute recurrent pancreatitis and ampullary tumor: a case report].

A case of ampullary cancer, the first clinical manifestation of which was two episodes of acute pancreatitis is presented with review of the literature on the possible role of this type of cancer as a causal agent of episodes of acute pancreatitis. We support the use of ERCP in all patients with acute idiopathic pancreatitis with the aim of discarding the possibility of a tributary ampullary tumor of curative removal.

[Attitudes, beliefs, and motivations in blood donors and non-donors].

OBJECTIVE: To know and compare the beliefs, attitudes and motivations of donors and non-donors in relation with blood donation. MATERIAL AND METHODS: Transversal study by questionnaires administered to 197 blood donors and 303 non-donors. The questionnaires gathered: sociodemographic data, beliefs and attitudes (using a Likert scale), and motivations and demotivations (using upon questions). The responses were analysed using different descriptive analyses and bivariant tests, and factorial analysis and discriminant analysis were performed as multivariant analyses. RESULTS: The donor group had a mean age of 37.8 years, were 58.4 female and 41.6% male, and had made 6.02 donations. The non-donor group had a mean age of 39.8 years and were 53.8% female and 46.2 male. Regarding attitudes and beliefs, the donor group showed more confidence in the good conditions in which donations are performed, had less fears about the possibility that donation can affect health, their attitudes were generally less egotistical, and they had less fears about possible commercialisation of the blood. Both groups believed that blood banks and society itself give inadequate compensation to donors, but this belief was significantly greater among non-donors. Regarding the motivations of donors, the highest proportion were related with a sense of solidarity or duty (26.4%), followed by issues related with information or pressure (23.4%), and the possible personal or family benefits that donation might bring (21.8%). The initial motivations of most donors (75.9%) had not varied with the passage of time, and the factors found to be most demotivating for donation had to do with problems of access and comfort (74.6%). We would highlight that most non-donors denied any worries about blood donation (70.9%) and did not agree that those who need blood should be those who pay for it (92.8%). For non-donors, lack of information (43.6%) and different fears (32.3%) were the principal factors discouraging them from donating. Finally, discriminant analysis selected 11 of the items as providing 75.6% of correct discrimination between the one population and the other. CONCLUSIONS: Donors and non-donors reveal very different beliefs, attitudes and motivations towards blood donation, and the questionnaire employed in this study permits a high degree of discrimination between the two groups. The attitudes of non-donors towards donation are generally less favourable, but allow us to think that a high percentage of these could be changed with the right conditions.

[Hepatic alveolar hydatidosis].

Alveolar echinococcosis of the liver is a disease caused by the intrahepatic growth of Echinococcus multilocularis larvae. In contrast to hydatidic cyst caused by Echinococcus granulosus, this is a rare disease in Spain. We present a case of alveolar echinococcosis of the liver which presented with jaundice and hepatomegaly. An infiltrative solid tumor was diagnosed by US and CT scan. The patient required left hepatectomy and interventional radiology procedures with satisfactory results. Diagnosis and treatment of this disease are discussed.

[Fibrous bone dysplasia in the context of a McCune-Albright Syndrome].

Young male patient clinically and radiologically diagnosed of a tumour in IX right rib. A subsequent study with bone scintigraphy showed other areas of focal uptake in the skull, ribs and left trochanterian region. The biopsy of the costal lesion showed a fibrous dysplasia in the clinical context of a McCune-Albright syndrome. The convenience of using complementary imaging techniques is discussed to establish adequate therapeutic behavior.

[Value of thallium 201-SPECT in typing brain space-occupying lesions].

OBJECTIVE: This study was designed to assess the diagnostic accuracy of single photon emission computed tomography with Tl-201 (SPECT Tl-201) to establish the tumoral or non-tumoral nature of brain space occupying lesions in comparison with usual diagnostic techniques. MATERIAL AND METHODS: The study population consisted of 37 patients, 24 men (64.9 %) and 13 women (35.1 %), mean age 48 +/- 16 years. After establishing the clinical and radiological diagnosis of brain lesion, all patients underwent SPECT Tl-201, evaluating it only by subjective analysis and blinded to neuroestructural techniques. After surgical resection all patients were evaluated anatomopathologically to establish the histologic nature. RESULTS: The sensitivity of SPECT Tl-201 (0.87) was higher than standard neuroimaging techniques (0.78). Specificity (0.43), positive (0.87) and negative (0.43) predictive values of SPECT were similar to neuroestructural procedures (MRI and CT scan) with 0.43, 0.82 and 0.38 values. Tumoral disease prevalence was 0.81. Neuroestructural procedures were non-conclusive in 18.9 % of the studies. No non-conclusive results were obtained with SPECT Tl-201. CONCLUSIONS: SPECT Tl-201 is a diagnostic procedure of high sensitivity to establish the tumoral nature of brain lesions, with poor specificity, similar to structural X-ray techniques.
