[Hepato-splenic scintigraphy in finding indications of bacterial endocarditis. Preliminary report].

Hepato-splenic scintigraphy with 99mTc-S-colloid was performed in twelve patients with bacterial endocarditis. These images showed that the size of the liver varied from normal to a severe hepatomegaly, depending on the presence of congestive heart failure. Intrahepatic distribution of the radiocolloid was slighty irregular in all cases. The spleen was conspicuously enlarged, and showed irregular distribution of the radiopharmaceutical. In some cases intrasplenic concentration defects caused by infarcts, abscesses or cysts, were observed. The relative uptake of radiocolloid by the spleen, varied from hypo- to hyperconcentration according to the degree of lymphoid hyperplasia caused by the infection. In two cases, both with congestive heart failure, concentration of the radiocolloid was evident in the bone marrow. The scintigraphic pattern observed in these patients with bacterial endocarditis can be easily differentiated from that caused by only congestive heart failure, which is similar to the observed in patients with cirrhosis of the liver and/or portal hypertension.

Immunotherapy with a standardized Dermatophagoides pteronyssinus glutaraldehyde-modified extract against an unmodified extract: a comparative study of efficacy, tolerance and in vivo and in vitro modification of parameters.

We comparatively studied clinical efficacy, tolerance and modifications of different in vivo and in vitro parameters induced by two biologically standardized Dermatophagoides pteronyssinus extracts (HEP units), one glutaraldehyde-modified, in patients with allergic rhinitis and/or bronchial asthma after a year of treatment. A decrease in drug consumption was observed in both treatment groups (p < 0.0001). Of all the in vivo parameters studied (cutaneous, conjunctival and bronchial reactivity to the allergen), a decrease in specific bronchial reactivity was only observed in the group treated with the modified extract (p < 0.05). The modifications in total IgE, specific IgE and specific total IgG levels are superimposable on those described in previous papers on immunotherapy. However, IgG4 levels remained stable with respect to time. Tolerance was good and very similar for both treatments; both types of extracts are equally efficacious and safe.

Sensitization to neumoallergens influence in mothers of asthmatic children.

Allergy diseases in our province have the characteristic of early appearance of asthma and predominant monosensitization to domestic dust mites due to the climatic condition. In this work we study the influence in allergic children. We selected 1028 mothers that brought their children to hospital for the first time. We divided them in two groups. One was comprised of 459 mothers of atopic children (MA) and another of 569 mothers of non atopic children (MC). All mothers were skin-prick tested (Phazet). The papula area, the mothers with aeroallergen sensitization, were 180 (17.5%) between MA 126 (27.4%) and MC 54 (9.5%). The diagnosis of allergic mothers was higher in MA than in MC. Asthma 8.39%, Rhinitis 4.75% and dermatitis 2%. 53.31% lived on the coast. 51.45% had house moulds. 31% had animals. 34.9% were smokers. There were more adverse drug reactions in MA than in MC. 180 mothers presented sensitization. D. pteronyssinus 66.3%, D. farinae 65.5% Lolium P. 14.55%, Cats 11.6% (MA > MC), Artemisia Vulgaris 6.25% and Cladosporium Herbarum 0.6%. There was no correlation between skin sensitization and domestic animals. There was no influence between children's IgE and mothers with skin sensitization. Skin sensitization to grass pollen correlated with the mother who was living in another country. The mothers who have been living more than five years in our province presented more skin sensitization to aero-allergens.

[Nutritional support in bone marrow transplantation].

UNLABELLED: Bone marrow transplant (BMT) implies the treatment with substances which may compromise the nutritional condition, thus increasing the morbido-mortality of these patients. The objective of this study is to evaluate the efficacy of the nutritional support (NS) protocol for patients subjected to a BMT in our center. PATIENTS AND METHODS: 55 patients were included (24 men and 31 women), who were subjected to BMT during 1994, with prior chemotherapy depending on the underlying disease. The nutritional condition (NC) was evaluated upon initiation and at the end of the NS, using anthropometric, biochemical, and immunological parameters. The NS was given by total parenteral nutrition (TPN), adapted to the needs, as of the second post-transplant day, until such time that oral nutrition was sufficient to supply the nutritional needs of the patients; oral ingestion was permitted at all times, according to the possibilities of the patient. For the statistical analysis, we used the Student's t test, Pearson's Chi squared test, and Spearman's test, with differences being considered significant for values < 0.05. RESULTS: The average duration of the TPN was 16 +/- 6 days, with a significantly longer time (p < 0.05) in patients with leukemia. The NC assessment was no different at the beginning and at the end of the NS, although all groups show a drop in the albumin levels at the end with respect to those at the beginning, with this being statistically significant in patients with leukemia (p < 0.05), and with solid tumors (p < 0.01), 14.5% of the patients maintained an acceptable oral ingestion (with 75% having lymphomas), and 34.5% did not show any associated oral ingestion. Te better albumin maintenance was correlated with acceptable oral ingestion (p < 0.05). CONCLUSIONS: Nutritional support of patients subjected to a BMT is effective for maintaining their NC levels. The longest duration of the TPN, the lowest frequency of associated oral ingestion, and the greatest decrease of the serum albumin, levels are seen in those cases which had the most aggressive chemotherapy prior to the BMT, which requires adaptation of the NS in function of the underlying disease. The association of oral ingestion may be beneficial due to its effect on the gastrointestinal tract.
