[Hospital infection and resistance to antibiotics at a pediatric intensive care unit].

A retrospective study of nosocomial infection and its aetiological agents antimicrobial sensitivity in a pediatric ICU during 65 months is reported. Using statistical analysis association was found between infection site and certain diagnostic-therapeutic procedures, such as urinary infection and urinary tract catheter, respiratory lower tract infection and respiratory assistance devices, bacteremia with intravenous catheters. Also found relationship between age and causal agents: Streptococcus faecalis is the main microorganism in newborns and Pseudomonas aeruginosa in older patients. Gram negative bacilli sensitivity increased significantly for some antimicrobial drugs: carbenicillin, cefotaxime, gentamicin, fosfomycin and tobramycin, whereas gram positive microorganisms sensitivity decreased to cephalothin, cefoxitin, trimethoprim-sulphamethoxazole and gentamicin.

[Biliary lithiasis in childhood: apropos of 10 cases].

Ten cases of cholelithiasis in children are reviewed. Mean age was seven years. Abdominal pain was the main symptom leading to diagnosis. In all the cases, the diagnosis was made by ultrasonography. Risk factors were found in two cases. Surgical treatment was performed in four cases.

[Experimental study on the process of repair-healing of the flexor system in the growing chick].

In the present work, in view of the concern for tendinous injuries in children hand flexor apparatus, tendinous healing and its functional and morphological recuperation were studied after section and surgical restoration of the leg flexor apparatus of chicken at different growth stages. We conclude that best results are obtained when the rehabilitation begins ten days after surgery.

[Hemodynamic course in relation to mechanical ventilation in the postoperative period of extracorporeal surgery in children].

Hemodynamic evolution and its relationship with the factors that have an effect upon gaseous exchange, during the early phase after extracorporeal circulation in sixty nine children, are reviewed. They are divided into three groups: group I (APH), arterial pulmonary hypertension; group II (OF), overflow, and group III (C), control. Results in the three phases of mechanical ventilation (IPPV, IMV and CPAP), are compared. We found that only in group I (APH), the pulmonary mechanics was altered. The programmed hyperventilation (IPPV) in group III (C), underlie the results of correlation between ventilatory and hemodynamic parameters with the oxygenation. There are high pulmonary vascular resistance in group I (APH); this explains the positive correlation between diastolic pulmonary arterial pressure and cardiac output. There are a good right ventricle function in I (APH) and III (C) groups. The hemodynamic patterns in IPPV are not depending on the pulmonary state.

[Glomerular changes in the contralateral kidney in the rat with experimental hydronephrosis].

We have studied under optic and electronmicroscopes the alterations of glomeruli in contralateral kidneys of rats with experimental hydronephrosis. Forty-eight Wistar rats, divided into two groups (control and experimental) were used. They were sacrificed 3, 6, 9 and 12 days after ureteral obstruction. There was a slight hypertrophy of glomeruli and hiperplasia of other components accompanied by a increased development of podocytes.

[Psychologic iatrogenic disease in the postsurgical child].

In the present paper, we comment on the psychological and emotional disorders occurring after surgery. We surveyed 150 children that were admitted in several surgical units of our Children's Hospital. We used the Vernon's test in a questionaire. We reached a high level of comprehension. As a result, 121 valid questionaires were returned. The considerable amount of information was statistically studied with a computer, obtaining data on the following aspects: Filiation, frequency and percentage of every answer. Post-hospitalization behaviour evaluated by Vernon's test. Partial and total results under the 6 factors of the questionaire: General anxiety and regression, anxiety about separation, anxiety about sleeping, eating disturbances, agressiveness against authority, apathy and isolation. Previous results in connection with the diverse filiation variables. When we analyzed the results, we confirmed the findings of other investigators: A global damage in child behaviour, that is present in two main aspects: "Anxiety about separation" and "Agressiveness against authority". In conclusion, our results support the need for implementing prophylactic measures, of which "psychological presentation" is the most important, to children that are going to be operated upon.

[Left ventricular pressure response to acute volumetric overload in congenital heart disease (author's transl)].

Fifty patients with congenital heart disease were studied hemodynamically to evaluate their functionally dynamic situation. The peak systolic left ventricular pressure and the left ventricular endiastolic pressure were analyzed in basal conditions and following the volumetric overload of the left ventricular angiogram as parameters reflecting afterload and preload, respectively. Highly significant correlations were obtained among those parameters in all patients and taking in consideration different syndromic groups. Regression lines obtained from these correlations permitted to establish, in a given patient, a quantitative prediction of response to volumetric overload which can estimate qualitative situation of his ventricle into the Starling's curve.
