[Postoperative spinal cord ischemia in thoracoabdominal aneurysms].

At the present article, 12 cases of paraplegia secondary to the surgical treatment of thoracoabdominal aneurysms are presented. Study includes patients admitted between 1973-1987 for treatment and rehabilitation of its medular injury. Factors of risk, surgical technics practiced, peroperative complications and type of medular injury are analyzed. The most common medular injury was an anterior medular syndrome (rather than 60%). In the same way, preventive methods are analyzed and somatosensory evoked potentials (SEP) and motor evoked potentials (MEP), both in developing phase at the present moment, are recommended as the most viable method for detecting changes during the surgical procedure.

[Thrombolytic efficacy of a Lys-plasminogen-urokinase combination: studies in experimental animals and humans].

During animal experimental phase, lis-pg combined with UK produced a thrombolysis of about a 62.5%. This effect is accompanied by an important fibrinolytic system activation, a decrease in fibrinogen levels (0.37 +/- 0.2 gr/l) and an increase PDF/Fg (120.5 +/- 30 ng/ml). Such thrombolytic stage produced diverse hemorrhagic complications in experimental animals. During human clinical trial stage, then patients with Deep Venous Thrombosis (DVT) at proximal lower limbs level were submitted to diverse treatment protocols with Lis-Plasminogen (Lis-plg) and Urokinase (UK). After preliminary outcomes we can conclude that administration of Lis-plg followed by UK increases the fibrinolytic activity but also increases the risk of hemorrhagic complications. This second effect is not probably caused by an specific absorption on the thrombo surface, but by an increase of circulating plasminogen levels Lis-plg exogenous-induced.

[Plasminogen depletion and incorporation in experimental venous thrombosis].

The objective of this piece of work is to study (using animals): venous thrombosis, the depletion and incorporation of plasminogen in thrombus of different stages of time, in order to devise better guide lines for the treatment of thrombosis than the conventional treatment. The studies relating to the plasminogen content of the thrombus show us that, on experimental venous thrombosis, as from the fifth day after which it occurred, the content of the plasminogen thrombus is reduced considerably. This data underlines the fact some venous thrombosis of over five days are resistant to conventional thrombolytic treatment. In relation to this we have to report that the mean lifespan according to our experiments of the plasminogen on beagle dogs is approximately sixteen hours. On the other hand the study of the addition of 125Lis-plasminogen, on experimental thrombosis of different stages of oldness show us that the above addition results in a 54% intake into the thrombus on three day, old thrombosis and 17% on five day old thrombosis. In conclusion we can suggest that the resistance to conventional thrombolytic treatment of old venous thrombosis (5-7 days old) is probably due to a reduction of efficiency of the thrombus plasminogen. At the same time the contribution of exogenous lis-plasminogen does not produce a sufficient enrichment of the thrombus in plasminogen. Therefore the therapeutic exogenous plasminogen addition is not advisable for relatively old thrombosis (less than or equal to 5 days old).
