[Experimental osteoporosis caused by estrogen deficiency in rats. Role of a lactose, L-arginine and L-lysine combination in the prevention and therapy].

The role of a lactose, L-arginine and L-lysine association in the treatment of osteoporosis was evaluated in an in vivo model. Fifty Sprague Dawley female rats, 10 months age, were divided into 5 groups: 10 non-ovariectomized animals sacrificed at the beginning of the study (I group); 10 ovariectomized animals (OVX) without treatment and sacrificed at three months (II group); 10 OVX animals without treatment sacrificed at 6 months; 10 OVX animals treated starting from the I post-operative day with the association for three months (IV group, prevention) and 10 OVX animals treated like those of the IV group starting three months after surgery (V group, therapy). Animals were sacrificed in general anesthesia according to the scheduled times for radiologic, densitometric, histologic and mineralogic investigations. Preliminary results, which will need further researches, seem to show a progressive reduction of bone mass after ovariectomy in 10 months female rat and that the pharmacological association given as therapy (V group) may be able to lessen the reduction of bone mineral content.

[Surgical treatment of ovarian cysts: economic analysis as a means for the evaluation of alternative technics. Results of a biennial study].

The large use of new gynaecological technologies such as the operative laparoscopy, requires both efficacy and efficiency evaluation. The aim of this work is to compare costs of the surgical treatment of ovarian cysts between two groups of patients--35 patients undergone to laparotomic cystectomy (age: mean 27.5) and 34 operated by laparoscopic technique (age: mean 27). The analysis of the costs, related to three steps of health care (pre-operative, operative, post-operative) shows that the laparoscopic cystectomy results the more efficient intervention (L. 6,244,808 vs L. 8,310,002). This economic analysis may offer a planning tool for health care to hospital managers and represent an efficiency evaluation criterion of surgical techniques employed by the gynaecologists.

[Parma Stroke Data Bank: embolic stroke].

Cerebral infarction is one of the three main causes of death in most countries. It is very frequent and, since it is more often disabiliting rather than fatal, it is of high social impact. The correct classification of patients and the accurate diagnostic definition of the various subtypes of stroke is of great prognostic and therapeutic importance since cerebral infarction is not a single entity. In this study we report our findings concerning 244 patients with embolic infarction recorded in the Parma Stroke Data Bank hospital register. Clinical features were studied (risk factors, symptomatology of the onset, degree of severity within 3 days of the onset, post-stroke complications) as were instruments readings (TAC) and evolution (outcome, mortality, personal performance and environmental integration, both 4 weeks after the clinical onset and after one year).

[The choleretic effects of licorice: identification and determination of the pharmacologically active components of Glycyrrhiza glabra].

Recent studies indicate that licorice extract, when administered per os or i.v., causes an evident choleretic effect in rats. Aim of this research is to identify and quantify those licorice constituents which are responsable for the observed choleresis. The quali-quantitative analysis of umbelliferon (7-idroxycoumarin), was at first performed by a fluorimetric method, subsequently by a more selective HPLC method. Moreover, this HPLC method allows the determination of glycyrrhizin, an important licorice constituent. Unlike the glycyrrhizin, which is present in a fairly large amount, umbelliferon resulted to be present at a very low concentration (at trace level), both in licorice and in bile. Research is in progress, aiming to determine the substances, beyond glycyrrhizin, which are responsable for the choleretic effect of licorice.

Fibroblast proliferation over dialysis membrane: an experimental model for "tissue" biocompatibility evaluation.

The present study reports on a biological model based on fibroblast proliferation applied to 3 different types of flat-plate dialysis membrane, in order to ascertain whether the artificial materials currently used in hemodialysis cause in vitro cellular proliferation. The study plan we followed involved plate membrane isolation from non-used dialyzers and used dialyzers, observed through scanning electron microscopy (SEM) both before and after testing with human fibroblasts by means of cell culture. Fibroblast growth was assessed by phase contrast light microscopy examination and cytometric DNA content evaluation. Our investigations proved that the artificial materials we considered interact with fibroblast cultures. Noticeable proliferative response was observed both after contact with unused material and on mediation by the protein layer absorbed on the membrane surface at the end of dialysis sessions. In this last case fibroblast proliferative activity appeared higher than that observed with unused membranes, showing that the soluble molecules entrapped in the protein layer appeared able to exert a biological activity even in vitro tests.

[Parma stroke data bank: atherothrombotic and lacunar stroke].

In an epidemiological research about stroke, we studied 235 patients with atherothrombotic brain infarctions and 81 patients with lacunes. It was a longitudinal study concerning patients admitted to our Medical Division during the acute phase and followed up for one year after the onset. We report some anamnestic data, the frequency of positive brain CT scan, main risk factors, symptoms at the onset, severity degree of the stroke within the first 72 hours, complications during, and outcome after, the first four weeks, including personal performances and environmental fitness, mortality rate and frequency of relapses. We also report some of these data after one year from the acute episode.

Methotrexate in psoriatic polyarthritis.

Methotrexate (MTX) is widely accepted as an effective treatment for psoriasis and chronic polyarthrides. We report data from 54 psoriatic polyarthritis patients (354 encounters) treated with MTX. 65% were males, mean age 47.9 years, mean disease duration 9.8 years for arthritis, 14.2 years for psoriasis. The MTX dose was 10-12.5 mg/week. 32 patients are still on MTX after a mean treatment time of 1.6 years. 22 patients discontinued MTX treatment: 11 felt it to be ineffective, in 6 patients there were side effects, 3 patients obtained remission, while 2 patients underwent surgery. Efficacy was good on the whole: number of swollen and tender joints, global disease activity score, ESR, and CRP all underwent a significant reduction. The intake of symptomatic drugs was reduced in 40%. Psoriasis as assessed with the Psoriasis Activity and Severity Index showed a significant improvement. Our data confirm that MTX is of value in most psoriatic polyarthritis patients (60%). In our experience, this drug, gives the maximum efficacy within 6 months of therapy.

Is visual field reduction a component manifestation of osteopathia striata with cranial sclerosis?

A girl with fully expressed osteopathia striata with cranial sclerosis (OS) was also found to have a contraction of the two visual fields, a sign never previously described in OS syndrome. We suggest that the visual field defect is a component manifestation of OS syndrome, whose pathogenesis is represented by distortion of the optic canal and narrowing of the optic foramina.

Gingival fibroblasts "in vitro" and Down's syndrome.

Gingival fibroblast cultures from four patients with Down's Syndrome (DS) and periodontal disease were compared with four in vitro age-matched fibroblast cultures of handicapped subjects (ND) also affected by periodontitis. The extra copy of chromosome 21 could alter growth regulation and biochemical mechanisms, so we examined quantitatively some DS phenotypical aspects to detect possible differences from those of controls. The growth properties of gingival fibroblast cultures from DS patients were more elevated than their ND age-matched controls. There were no differences in plasma membrane polarization and in neutral endopeptidase activity. The succinate-cytochrome C reductase activity decreases in DS fibroblasts compared with ND. Our results outline the difficulties to in using fibroblast cultures as an in vitro system to study premature ageing Down's Syndrome.

[Cutaneous microcirculation in systemic sclerosis. Morpho-functional research using capillaroscopy and laser-Doppler].

Our previous researches proved that, in patients affected by Raynaud disease, microcirculatory alterations were greater in those cases in which the small vessels showed evocative appearances of a sclerodermatous connectivopathy. In this study we evaluated cutaneous microvasculature in 18 patients suffering from clear systemic sclerosis, compared with a group of 16 subjects affected by primitive Raynaud disease and a group of healthy subjects. We used videocapillaroscopy and laser-Doppler fluxmetry for morphological and functional study respectively. In patients affected by systemic sclerosis the rest flow was clearly reduced and morphological pattern of cutaneous small vessels deranged. The response to ischemic test allowed us to subdivide the skin-bounds in two subgroups: "no responders" 8 subjects (44%), in which reactive hyperaemia was completely absent, "responders" 10 patients (56%) in which the hyperaemia was completely overlapped with that of the two other groups, but with longer reaction times. In skinbounds the capillaroscopic pattern was clearly severe in comparison with other two groups (18.8 +/- 5.7 vs 5.7 +/- 2.3 and 3.6 +/- 1). Thus, in advanced stage of the pathology, is microangiopathy (in its tromboischemic and inflammatory aspects) that plays a very important role in the development of organ damage. Therefore, all those clinical and instrumental tests which may allow a differential early diagnosis between a primitive and a secondary Raynaud phenomenon be done, for its prognostic value in connection with the appearance of systemic sclerosis.

Reduced nephrotoxicity and hepatoxicity in cyclosporin A therapy by enalapril and spironolactone in rats.

The aim of the study was to define a therapy to be combined with an immunosuppressive drug such as cyclosporin A, in order to partially reduce the nephrotoxic and hepatotoxic effects in the treated rats. Two drugs were considered: enalapril, which is an inhibitor of the angiotensin-converting enzyme, and spironolactone, which is an antagonist of aldosterone. These two drugs interrupt the renin-angiotensinogen-angiotensin chain after this has been activated by cyclosporin A, preventing peripheral vasoconstriction and more specifically the constriction of both glomerular arterioles and hepatic vessels from occurring, thus diminishing the cyclosporin A toxicity in both liver and kidney.

Optic nerve hypoplasia associated with pupillary light-near dissociation, spastic paraparesis and other non-ocular anomalies.

We report a case of bilateral optic nerve hypoplasia in a patient with pupillary light-near dissociation, spastic paraparesis with deep hyperreflexia, mild distal hypotrophy especially of lower limbs, bilateral pes cavus and signs of motor peripheral neuropathy. Other associated signs were rhinolalia, hypotrophy and fasciculations of the tongue and cardiomyopathy. We differentiate the condition from a number of hereditary ataxias and paraplegias. The clinical features of Strümpell-Lorrain spastic paraplegia and amyotrophic lateral sclerosis seem to resemble most closely that of our patient.

[Hyponatremia during therapy with low doses of carbamazepine. Report on a clinical case].

A case of a patient who developed hyponatremia both during normal dose and during low dose carbamazepine therapy is described. The clinical characteristics of hypo-osmolar syndrome due to carbamazepine are similar to those of the syndrome of inappropriate antidiuretic hormone secretion. In literature, many cases of hyponatremia, related to assumption of normal dose carbamazepine, are reported; instead hyponatremia during low dose carbamazepine is rarely described. For this reason, a frequent monitoring of plasma sodium concentration, also in patients treated with low dose carbamazepine, is warranted.

Bronchopulmonary dysplasia: a longitudinal study of 20 cases.

Of 20 patients with bronchopulmonary dysplasia (BPD), 17 survived (85%) and were followed prospectively up to one year post-term. Lower respiratory tract infections occurred in 12 patients (70.5%), and in 7 (41%) at least one hospital admission was required. At one year post-term follow-up, 9 patient (52.9%) continued to present respiratory symptoms, and 5 out of 13 (38.4%) radiographic changes. Six cases (35%) presented chronic cor pulmonale. Only 6 children (35%) showed normal growth, while the others showed deficits in one or more growth parameters. Cerebral palsy occurred in 41% of the children; 3 cases of severe tetraparesis and 4 of moderately severe palsy (hemiparesis or diplegia). Developmental quotient (DQ) was less than 70 in 6 cases (35%), from 70-90 in 4 (23.5%), and greater than 90 in 7 (41%). The severe neurodevelopmental outcomes were significantly correlated with the presence of important neonatal cerebral pathology (3-4 degrees IVH or periventricular leukomalacia). Retinopathy due to prematurity was diagnosed in 7 patients (41%), and in 5 it progressed to retrolental fibroplasia. Ten cases (58.8%) showed strabism, of which 8 had previous eye background involvement. Hearing deficit was not detected in any patient. Esthetic and functional sequelae consisted of scalp eschar (3 cases), post-thoracotomy scar (1 case), pleural drainage scars (3 cases), nasal deformity due to prolonged intubation (1 case), laryngeal stenosis (1 case), and post-tracheostomy stenosis (1 case).(ABSTRACT TRUNCATED AT 250 WORDS)
