A newbron baby that following an exchange transfusion due to A incompatibility, presented an hepatic and renal failure, is reported. Hepatic necrosis and associated renal patholgy suspected ruging evolution treated with exchange transfusions and peritoneal dialysis for three days, with improvement but, died at eight days of age. Necropsy findings confirm clinical suspect of hepatic necrosis. Pathology more frequently associated and management of fulminant hepatic failure is commented.
BACKGROUND: The aim of the present study was to evaluate the influence of p53 protein on the survival of patients undergoing radical gastrectomy and postoperative adjuvant chemotherapy for gastric cancer. PATIENTS AND METHODS: It was a retrospective study of 46 patients with gastric adenocarcinoma (Stage II and III of the Japanese staging system). Alypatients were treated by curative radical gastrectomy with regional lymphadenectomy plus adjuvant chemotherapy. This regime included Mitomycin (20 mg one hour before surgery, followed by 10 mg the day after) and Fluorinated Pyrimidine (UFT) (400 mg/m2/day orally) (started four weeks after operation, and continued for one year). Immunohistochemical expression of p53 protein was determined on tumor samples from the removed specimens. The influence of p53 on survival was assessed in a Cox's proportional hazard regression analysis. RESULTS: Sixteen tumors (34.7%) manifested nuclear overexpression of p53 protein. Patients with p53-negative tumors showed higher cumulative survival at 4 years follow-up than patients with p53-positive tumors (82% versus 45%) (p < 0.01). Multivariate analysis identified p53 overexpression as a negative independent predictive factor (hazard ratio: 11.15) (95% CI: 1.93-64.42). Multivariate analysis performed on patients with Stage III tumors, separately, confirmed the predictive effect of p53 overexpression. CONCLUSION: The results suggest that postoperative adjuvant chemotherapy acted differently in p53-positive than in p53-negative gastric tumors. Absence of p53 overexpression is associated to longer survival when adjuvant therapy is administered.

Seventy one per cent of neonates had a gestational age longer than 37 weeks. Sixteen showed respiratory distress, and shock occured in twelve cases. The onset of illness happened within the first 12 hours of life, in 15 cases. No differences were observed among preterm and term infants. Differential leukocyte count were altered in all infants, being particularly relevant the fact that the relation between immature neutrophil/total neutrophil, was elevated in 50 per cent of cases. Mortality rate was 52 per cent. Clinical alert and early treatment with penicillin, together with the use of fresh blood exchange transfusion, can be considered useful methods in the treatment of these cases.

OBJECTIVE: The degree of aggregation is determined for the whole population and for different age-specific strata and is used for assessing the reinfection rate after a mass antihelminthic chemotherapeutic intervention. MATERIAL AND METHODS: The degree of aggregation of Ascaris lumbricoides is derived from prevalence and intensity of infection date obtained from an epidemiological study of ascariasis carried out in Coatzacoalcos, Veracruz, México by means of maximum likelihood estimation of the negative binomial distribution. RESULTS: The results show that the distribution of the parasite for the whole population differs from the distribution of the parasite within each age-specific stratum; the reinfection rate is not uniform, the dynamics of the host-parasite association exhibits a clear destabilization of its endemic equilibrium after a first round of mass treatment. CONCLUSIONS: It is suggested that the aggregation among the different age-groups is highly variable and hence the infection and reinfection rates are heterogeneous with regard to age; these states of the host-parasite system are transient. Given the heterogeneity of the infection, the selective treatment seems to be the most adequate strategy to diminish helminth infection rates.
