Post-natal changes in biliary lipids in suckling goat kids.

The post-natal composition of biliary lipids in the bile of suckling goat kids has been studied during the first month of life. Samples of hepatic and vesicular bile were obtained from animals fed with milk. Bile flow increased with age and the concentration of bile acids also rose up to 14 days of age. Cholesterol reached maximum values immediately after birth and then decreased. Bile acids conjugated with taurine predominated and this pattern of preferential tauroconjugation persisted at all ages, as has also been found in carnivores and sheep. Comparison of vesicular and hepatic bile composition revealed a poor concentration capacity of the gallbladder in pre-ruminant kids.

Biliary secretion in suckling goats: the effect of maternal milk and of a lamb milk replacer.

A total of 45 preruminant goats were fed either goat milk or a milk substitute. The postnatal development of bile secretion and biliary lipids composition of suckling goats and the influence upon these parameters of maternal milk substitution were studied during the first month of life. Samples of hepatic and gallbladder bile were obtained from kids of 1, 7, 14, 21 and 28 day-old. Bile flow rate, cholesterol, total bile salts and phospholipids were measured. Our results show that the quality of the protein and fat used in the elaboration of the milk replacer, affects the evolution of both hepatic bile flow and biliary lipids composition. Differences between gallbladder and hepatic bile composition show a scarce concentration capacity of the gallbladder with both diets and at all ages studied.

Conjugated bile acids and intestinal flora during the preruminant stage in goat. Influence of a lamb milk replacer.

A total of 45 preruminant goats were fed either goat milk or a milk substitute to compare the evolution of taurine and glycine conjugated bile acids and the evolution of the intestinal flora during the first month of life. Samples of hepatic bile were obtained from kids of 1, 7, 14, 21, and 28 days old and conjugated bile acids were measured by high performance liquid chromatography. Bacterial flora were determined in 14 and 28 day old animals. Ours results show that under our experimental conditions tauroconjugated bile acids clearly predominated over the conjugated with glycine. This pattern of preferential tauroconjugation persisted at all ages tested and in both groups studied. On the other hand, it seems that the quality of the protein and fat used in the elaboration of the milk replacer affects the intestinal flora and the postnatal evolution of conjugated bile acids.

Cisplatin, 5-fluorouracil, and high-dose folinic acid in patients with advanced unresectable head and neck cancer.

For patients with advanced, unresectable head and neck (HN) cancer, surgery and/or radiotherapy are the standard treatments but have poor results. A phase II trial of a continuous infusion of cisplatin, 5-FU, and high dose folinic acid (PFL) as induction chemotherapy in patients with previously untreated, locally advanced HN cancer was performed in an attempt to confirm the encouraging results reported by Dana Farber investigators using an identical regimen. Forty-five consecutive patients with unresectable HN cancer were treated every 28 days with a continuous infusion of cisplatin 25 mg/m(2)/day (days 1-5), 5-FU 800 mg/m(2)/day (days 2-6), and folinic acid 500 mg/m(2)/day (days 1-6). After three courses of chemotherapy, patients were treated with surgery and/or radiotherapy. Objective responses were observed in 26 of 38(69%) evaluable patients with 14(37%) clinical complete responses. Grade III-IV toxicity was important and consisted mainly of mucositis and neutropenia that were found in 47 and 18%, respectively , of patients after the first course. There was one toxic death. PFL is an active, toxic induction regimen for far-advanced HN cancer, yielding a response rate in the range of the widely used cisplatin and 5-FU (PF) schedule; a comparative trial is warranted before concluding that PFL is superior to the latter combination.

[10 Year follow up of chemotherapy and surgery on the second laparotomy in advanced ovarian cancer].

BACKGROUND: Advanced ovarian neoplasm has bad prognosis. There is little knowledge as to the effect of surgical and chemotherapy treatments on long-term survival. METHODS: Seventy-two patients with advanced epithelial ovary carcinoma (53 stage III and 19 stage IV) were treated according to a treatment regimen with reduction surgery, five cycles of chemotherapy with cyclophosphamide, adriamycin and cisplatin (CAP) followed by second revision laparotomy. RESULTS: The rate of response for the CAP schedule was 80%, of which 16 patients (23%) showed complete response (CR), 7 (10%) partial microscopic response (PMiR) and 33 (47%) partial macroscopic response (PMR). Complete resection of residual masses was performed on the second laparotomy in 14 of the 33 patients with parital response. The median survival for all the group was 36 months with overall actuarial survival of 27% at 10 years. The survival of the group of patients with CR was significantly longer than that of PMiR and other groups. Significant differences favorable for the group of partial response with second attempt radical surgery were found versus the group in which te second surgical resection was not radical. FIGO III stage and prechemotherapy tumor size less than 5 cm were found to have favorable effect in the rate of response and survival. CONCLUSIONS: The use of CAP chemotherapy achieved complete response in 23% of the patients studied. This group of patients showed to have a greater probability of longer survival. Second attempt surgery on the second laparotomy offers therapeutic benefits when radical.

[Intensive chemotherapy with support of peripheral hematopoietic progenitor cells, mobilized with high-dose cyclophosphamide and G-CSF: fast hematologic recovery].

BACKGROUND: Hematopoietic progenitor cells mobilized to peripheral blood by a chemotherapy combined or not with hematopoietic growth factors and harvested with cyto-apheresis (CTA) provide a rapid hematological recovery when infused as a support step after intensive chemotherapy (IC). METHODS: Cyclophosphamide (CI) 4 g/m2 and G-CSF 5 mcg/kg/d were administered to 19 patients with a solid tumor or lymphoma. Daily CTA were performed during hematological recovery to harvest > 2.5 x 10(6) cells CD34+/kg. Seventeen patients received IC with infusion of peripheral hematopoietic progenitor cells (PHPC) and G-CSF. RESULTS: A total of 52 CTA were performed, with a median (M) of 2 per patient. A M of 4.4 x 10(8) mononucleated cells/kg and 9.8 x 10(6) CD34+/kg were harvested per patient. Hematological recovery after IC with support of PHPC and G-CSF was rapid in all cases, but the aplastic period was shorter in the ten patients who received > 5 x 10(6) CD34+/kg cells than in the seven patients with < 5 x 10(6) kg: The median of recovery to neutrophils > 0.5 x 10(9)/l was 8 days compared with 9 days (p = 0.0005), to platelets > 20 x 10(9)/l of 8 days compared with 12 days (p = 0.001), and to platelets > 50 x 10(9)/l of 11 days compared with 14 days (p = 0.001). CONCLUSIONS: Toxicity of IC (4 g/m2) with G-CSF is moderate and allows the harvesting of an adequate number of PHPC. Its infusion after IC provides a rapid hematological recovery, which was more marked in patients receiving 5 x 10(6) CD34+/kg cells, than with the same IC schedules of IC with autologous bone marrow transplantation.

[Changes in the lipid profiles of hyperlipemic patients treated with binifibrate].

Forty six, after six weeks of low fat diet (step 1) hyperlipidemic patients, age 20 to 65 years, both sexes, have been studied to recognize the effectiveness of three months drug treatment with Binifibrate (600 mg T.I.D.). Total cholesterol, triglycerides, LDL-cholesterol (cLDL), HDL-cholesterol (cHDL), apoprotein A, apoprotein B and other routinal biochemical parameters were controlled at 0, 30 and 60 days of treatment. A significative total cholesterol, cLDL and apo B lowering plus a significative cHDL and apo A increase was registered. Results reflect a beneficial effect of Binifibrate on lipoprotein metabolism and in such wise, locate this drug as an interesting one for hyperlipidemic situations treatment.

Glomerular pathology in surviving pigs experimentally infected with African swine fever virus.

Twelve miniature pigs were inoculated with an attenuated African swine fever virus to study glomerular involvement in surviving pigs. In acute phase, kidneys were severely affected and displayed a glomerular capillary thrombosis with fibrin deposition in vascular lumen, detected by immunofluorescence. Fibrin-positive deposits were progressively cleared between one to three months after infection in surviving pigs. The histological picture in kidneys of surviving pigs, up to one post-infection year, showed a focal and segmental glomerulonephritis with hyalinosis, and IgM and C3 deposition was detected by immunofluorescence. Its pathogeny as an evolutive stage of acute glomerular injury is pointed out.

Double labeling immunohistological study of African swine fever virus-infected spleen and lymph nodes.

To identify cells in situ in which African swine fever (ASF) virus is present, a double immunohistological labeling technique was used on sections of ASF-infected spleen and lymph nodes. Cells were identified by an indirect immunoalkaline phosphatase technique using monoclonal antibodies (MoAb) reactive against different leukocyte subsets. ASF virus, detected by a direct immunoperoxidase method using swine immunoglobulin G (IgG) anti-ASF virus antigens, was not present in T helper or in T cytotoxic/suppressor lymphocytes, whereas it was detected in tissue macrophages that reacted with different MoAb (74-22-15, C4, A7, and F2). A large number of cells strongly reactive with MoAb 74-12-4 (T helper lymphocytes) were found in the marginal zone in infected spleen. In infected lymph nodes, these intensely stained cells were found in small numbers. Cells reactive with MoAb 76-2-11 (T cytotoxic/suppressor lymphocytes) were less stained in infected spleen and lymph nodes than in non-infected organs.

Testicular amyloidosis in hamsters experimentally infected with Leishmania donovani.

Thirty hamsters were inoculated intraperitoneally with Leishmania donovani. Testes were examined grossly and histologically by light and electron microscopy. Progressive testicular atrophy developed. Spermatogenic cells of the seminiferous tubules showed vacuolar degeneration and decreased in number leading to a total azoospermia in the final weeks of the pathological process. Lymphoplasmocytic infiltrates with macrophages containing leishmanias appeared in the intertubular space. Amyloid deposits in the intertubular space and tubular basement membrane were identified by optical and ultrastructural methods. It has been suggested that testicular amyloidosis may have a pathogenic mechanism related to a dysfunction of plasma cells and stimulation of the reticuloendothial system, due to the antigenic character of the parasite.

Relationship between low bone mineral density and highly active antiretroviral therapy including protease inhibitors in HIV-infected patients.

PURPOSE: The objectives of this study were to determine the prevalence of osteopenia and the factors associated with its presence in HIV-infected patients under highly active antiretroviral therapy (HAART) and to assess the changes of bone mineral density (BMD) in a population followed prospectively. METHOD: BMD was assessed by dual-energy X-ray absorptiometry (DEXA) scans at the lumbar spine and at the femoral neck in 78 HIV-infected patients who had previously received HAART as the first antiretroviral regimen and in 11 antiretroviral-naive HIV-infected patients. BMD measurements were repeated in 70 treated patients who had completed 1 year of follow-up. RESULTS: Thirty-seven (42%) patients showed osteopenia at any localization. The prevalence of osteopenia in PI-naive patients was 23% versus 49% in individuals who had received PI at any moment [p =.001; adjusted odds ratio (95% CI) = 0.11 (0.02-0.48)]. The frequency of osteopenia was significantly higher among men than among women [50% vs. 17%; p =.016; adjusted OR (95% CI) = 12.1 (2.22-66.20)]. The level of plasma albumin was independently associated with osteopenia [adjusted OR (95% CI) per each g/dL of plasma albumin decrease 2.55 (1.18-10)]. In patients in whom a second DEXA was done, no significant changes in BMD were found. CONCLUSION: The prevalence of osteopenia in HIV-infected patients on HAART is high. Loss of BMD is associated with PI therapy, low plasma albumin level, and male sex. Osteopenia does not progress after 1 year of continued HAART.
