[Rexetin in the therapy of depressive states].

Rexetin was used in the dosage of 20-60 mg daily during 6 weeks in the treatment of 25 patients with depression. An efficacy of the therapy was evaluated by the Hamilton scales for Depression (HAM-D) and anxiety (HAM-A) and Montgomery-Asberg Scale (MADRS). Patients were considered as responders if they demonstrated the reduction of scores by 50%. Twenty-three responders (92.0%) were divided into 3 groups: the first--with immediate therapeutic response during 1-2 weeks (n = 8; 34.8%); the second--with positive dynamics at the 3rd week of the treatment (n = 9; 39.1%); the third group--with favorable results at the end of 5-6 week of the therapy (n = 6; 26.1%). The clinical predictors of different therapeutic responses during short-term therapy were as follows: severity of depressive episode, hypotimia of modality (anxiety, anxiety-sorrow); vitalization of verbal/ nonverbal sorrow affect; presentation of autonomic (somatic) appearances of generalized anxiety disorder; ranks of typical depressive symptoms in subjective perception of patients; character of sleep disorders, presence of atypical depressive symptoms, in particular a disturbance of interpersonal relations due to elevated irritability; localization and extent of generalization of pathological body sensations; dominant content of obsessive anxiety; a type of affective triad; panic attacks, with- or without agoraphobia, concomitant to depressive disorder. In the total group of responders, there was a simultant (harmonic) reduction of the total score on the depression and anxiety scales (65.2%) and the anticipating anxiolytic or thymoanaleptic effect of the drug was observed less often (34.8%).

[Hypochondriac schizophrenia in the involutional period].

A total of 101 patients suffering from slowly progressive schizophrenia with hypochondriac symptomatology and a manifestation or a relapse of the disease in the involutional age have been studied. Three variants of the disease course have been identified. The authors have considered the significance of sex, premorbid peculiarities of the personality, aggravated heredity, as well as the rate and character of exogenic manifestations in the patient's history as possible pathogenetic factors of a late manifestation (recurrence) of the disease. The incidence of adynamic-anergic component of depression in the involutional period is correlated with processes of accelerated mental ageing in schizophrenic patients with exogenously aggravated history. Cases with the first manifest attack in the involutional period have been referred to true late schizophrenia.

[Psychopathology of the late period of closed military craniocerebral injuries (non-psychotic forms)].

The author analyzes nonpsychotic disturbances at the late period of a combat closed craniocerebral injury (CCCI) in 97 veterans of the Great Patriotic War. Considerable differences were found in the incidence and structure of the organic decrease in mental activity, the psychoorganic syndrome as well as emotional-volitional and affective disorders, which depended on the clinical variant of the time-course at the late period of a CCCI. The author identified the following clinical variants: protracted decompensation of long-term sequels of a CCCI; cerebrovascular disease associated with residual manifestations of a CCCI; and a predominantly cerebrovascular disease in individuals with a history of a CCCI.

[Variants of the clinical course of the late period of military closed craniocerebral injuries].

As a result of a comprehensive psychoneurological examination of 160 war veterans who had sustained a cranicocerebral trauma of a mild or moderate degree during the Great Patriotic War, the authors identified 3 variants of the clinical course of its long-term period: (1) late (at the age of 45-59 years), protracted (of many years) decompensation of a complex (polymorphic) posttraumatic neuropsychic defect; (2) a comparatively early (by age) and accelerated development of cerebral atherosclerosis and arterial hypertension with unstable compensation of posttraumatic cerebral, predominantly vegetative-angiodystonic disorders; (3) the prevalence in the clinical picture of the disease of marked manifestations of chronic cerebrovascular failure or stable sequels of brain stroke in the presence of individual reduced psychopathological posttraumatic symptoms. The article also presents electrophysiological, biochemical and psychological changes characteristic of each variant of the course, which should be taken into consideration in making the diagnosis and predicting the clinical and employment-related outcome.

[Systematics of diseases with cenesthesiopathic-hypochondriac and algesic-hypochondriac syndromes].

The occurrence of cenesthopathic-hypochondriacal and algic-hypochondriacal syndrome in the climacteric period is inherent to certain age related and non-related groups of diseases. An exogenous (mainly infectious) age concept on the genesis of this protracted nonpsychiatric hypochondriacal symptomatology is formulated. An explanation is proposed concerning an earlier (up to 40 years of age) appearance of this disorder in adults.

[Pathologic hypochondriacal personality development (conditions of formation, clinical variants, type of course)].

The paper deals with some results of a complex somato-psycho-neurological study of 51 patients with a pathological hypochondriacal personality development which led to a stable restriction or a complete loss of working capacity. The author indicates a complex pathogenesis of this disorder: a low level of socio-working adaptation and psychopathic traits in the premorbid personality; psycho-traumatizing situation due to an underestimation by the physicians of the severity of the somato-neurological disorder which in some cases is supplemented by an unfavourable family situation.

[Protracted hypochondriacal states of cerebral-organic genesis].

On the basis of clinico-follow up studies of 67 patients with brain disorders the author distinguishes 4 types of the development of the process: long-termed sluggish, remittent, subacute-progressive without remissions and regressive. The author stresses the age significance, additional exogenic factor when sluggish and remittent types of development are replaced by continuous types with moderate progressiveness. The remote stage in the illness development is characterized by expressed mnestic-intellectual deterioration, mental and physical insufferability, chronic and periodic mood disturbances, sleep disturbances, increased sensory excitement, personality changes of a hypochondrical type.
