Therapeutic intervention scoring system in medical intensive care.

A scoring system based on therapeutic intervention on critically ill patients called the therapeutic intervention scoring system (TISS) was used to assess the quantity of care provided in a medical intensive care unit. Besides observing the unit census, the severity of illness and the work load were studied. The survival rate was 77 percent. The non-survivors had admission TISS points higher than the survivors and their mean daily TISS was more than 20 points. The survivors at discharge had a mean TISS of five points. The work load showed that a nurse can effectively manage two patients who together may accumulate 24 TISS points per day. TISS points per patient rather than bed occupancy is a better indicator of the nurse's work load. Admission criteria and procedures before death certification are outlined.

Takayasu's disease.

A fourteen years old girl developed Takayasu's arteritis (pulseless disease) since six months prior to investigation. This unusual form of arteritis is common in Japan and Korea but has rarely been reported in individuals born in the United States. In Indonesian literature it has never been documented so far. The etiology is unclear. The literature currently but forward the hypothesis of an autoimmune basis and treatment with steroid. Although a tuberculin sensitization pathogenesis has been suggested, a close temporal relationship with the onset of a tuberculous process has not previously been documented. The likelihood of uncovering tuberculin sensitivity or active tuberculosis in patients with Takayasu' arteritis is substantially higher than in the general population in all countries analyzed. The natural history of this arteritis is highly variable. The adolescent described in this paper has demonstrated no response either to antituberculosis therapy, or to heparin and corticosteroid as suggested by Ishikawa, 1987. The patient died on the 59th day of hospitalization after getting syncopal attacks followed by shock.

Pericarditis and pleuritis caused by extramedullary plasmacytoma.

The following illustrates a case study of a 9 years-old girl with combined pericarditis and pleuritis caused by solitary extramedullary plasmacytoma. Pericardiocentesis and permanent thoracocentesis were performed, both yielded serohemorrhagic and serous fluid in succession. In the beginning etiological diagnosis was made on the basis of the clinical pattern for tuberculosis infection and growth of three species of bacteria for bacterial infections and candida species for candidiasis. The initial treatment was in accordance with the etiological diagnosis mentioned above. The final diagnosis was establish in the fourth month after the discovery of plasmacytoma in the pleural fluid and CT scan examination disclosing masses in the right lung. Accordingly, cytostatic therapy was started. The result of therapy was very good, exudation into the pleural and pericardial sac regressed gradually and eventually ceased completely. According to the literature the prognosis of these neoplasma is good.

Paroxysmal supraventricular tachycardia in children: the role of infectious diseases and its relationship to serum enzyme.

The records of 28 children whose first episode of paroxysmal supraventricular tachycardia occurred before 12 years (median age 10 months) were reviewed. There were 17 males and 11 females. In 17 cases the first attack occurred before the first year and in 11 of these it occurred after the first year. One case had congenital heart disease (ASD). The WPW syndrome was diagnosed in 3 cases. When first seen, most of the infants presented with signs of incipient or manifest congestive heart failure. In almost nine-tenth of cases there was an increased of serum enzymes (lactic dehydrogenase, creatine-phosphokinase and glutamic oxaloaccetic transaminase. Digitals was effective against congestive heart failure and when continued, might prevent failure during subsequent attacks. Antiarrhythmic agents other than digitals were not used. It is recommended to continue digitalis treatment for at least one year in all patients with SVT, whether or not the first episode terminated spontaneously.
