A new diagnostic approach to congenital stridor using a laryngeal mask airway and rigid endoscope.

Neonates with symptoms of stridor from birth, present a difficult diagnostic problem. We have demonstrated that by the use of a laryngeal mask airway in an anaesthetized baby breathing spontaneously, we are able to reach a diagnosis. This is accomplished by the introduction of a rigid fibre-optic endoscope through a Portex swivel connector and visualizing the glottis and larynx.

[Evaluation of the usefulness of standard strains sets from the senior collection for typing Proteus mirabilis strains].

The object of our study constituted 651 P. mirabilis strains, isolated from samples of clinical material, which were typed by the standard strain set from the B.W. Senior collection. One hundred and sixty seven types of reaction (P/S) were determined basing on their ability to produce proticine++ or on their susceptibility to proticine++. No typing strains (PO/SO) made up 10.4%. On the other hand, strains that typed by the presented method could be classified in separate groups of clinical material within the range of 44.2%-73.5%. Particular P/S types were looked for, such that would detect so-called hospital strains. Among the strains typing in all groups of clinical material (apart from autopsy), the production of proticine++ 2 and 3 was dominated. Susceptibility of P. mirabilis strains to given proticine slightly differed in separate groups of clinical material. The majority of strains were susceptible to proticine++ 9. P. mirabilis strains produced one or several proticine and were also susceptible to one or several of them. Thorough analysis did not establish the dominance of whatever P/S type in respective groups of clinical material or of particular P/S types characteristic of a given disease. In the case of mixed infections no correlation was established between the P/S P. mirabilis type and the occurrence of accompanying microorganisms. The discussed method of typing P. mirabilis strains by the standard strain set from the B.W. Senior collection makes it possible to obtain a higher percentage of strain differentiation as compared with findings reported by other authors. Hence our recommendation to use this set as well as the modified typing method in epidemiological analysis.(ABSTRACT TRUNCATED AT 250 WORDS)

[Usefulness of plasmids as epidemiologic markers for typing of Klebsiella].

The study was aimed at evaluation of utility of plasmids as epidemiological markers for interspecific differentiation of 200 strains of Klebsiella isolated from patients with generalized infections and hospitalized at intensive care ward of the Gdańsk Medical Academy. Strains isolated from the ward environment were also investigated. In the first phase of this study classical methods of typing were used, such as determination of the biochemical, bacteriocin (susceptibility to 8 bacteriocins) and phage types, and MIC for 9 antibiotics. Basing on these methods it was found that 41 types of Klebsiella pneumoniae, 13 types of Klebsiella oxytoca and 1 type of Klebsiella ozenae occur. Six groups of bacteria were differentiated in the group which had the same type. These strains were investigated for plasmid and restrictive patterns. Identity of types occurring in individual groups was detected. For one of them, a restrictive pattern was determined by cleavage of plasmid DNA by a restrictive Eco RV enzyme. In remaining groups such pattern was not found, both after treatment with Eco RV and Hha I enzyme and this may be associated with modification of the plasmid DNA. Strains belonging to one of these groups did not contain plasmids in their cells.

[Mycoplasma pneumonia].

Two cases of mycoplasma pneumonias are presented. Initially this etiology was not suspected. Prior to hospitalization they were treated with cefamandole and carbenicillin and cephradine with gentamycin without any improvement in the clinical state of the patients. After serological tests finding a high titer of serum antibodies (1: 2560) the therapy was altered in the first patient to minocycline (in a dose of 200 mg per day for 16 days) and in the second to doxycycline (in a dose of 200 mg per day for 10 days). The first patient improved clinically after two days of altered therapy, the second after 10 days. Both patients were discharged fully recovered after 32 and 36 days of hospitalization.

[Hemagglutinating properties of Proteus mirabilis strains isolated from clinical specimens].

Haemagglutinating properties of 345 P. mirabilis strains isolated from various clinical samples were determined. Red blood cells of different origin as human group 0, bovine, horse, sheep and rat were used for the study. For the detection of MS and MR/P haemagglutinins the haemagglutination reaction was run with and without D-mannose. On the other hand, for the detection of type MR/K haemagglutinins tanned human and bovine erythrocytes were used. The majority of tested strains (90.14%) was polyhaemagglutinating i.e. showed simultaneously the presence of two or three haemagglutinins. Only three strains of P. mirabilis (0.87%) did not agglutinate any of the erythrocytes used for the study. The majority of strains (95.83-100%) in specific groups of clinical materials showed the presence of MR/K+ while MR/P+ 45.45-93.75% of strains and MS+ 45.83-73.1% of tested strains. Out of P. mirabilis strains isolated from urine, faeces and blood the highest percentage possessed at the same time all three haemagglutinin types (MS+, MR/K+, MR/P+) or pattern MR/K+, MR/P+. Bronchial isolates had usually pattern MR/K+ (31.82%) and strains isolated from skin possessed haemagglutinins of pattern MR/K+, MR/P+ (50%) and MS+, MR/K+, MR/P+ (43.75%). Among strains expressing MR/P+ at 37 degrees C a great differentiation of spectrum activity against tested erythrocytes was seen. Undoubtedly, the majority of MR/P+ strains from specific groups of clinical materials (with the exception of urine) agglutinated sheep and horse erythrocytes with and without D-mannose. Brainstem auditory evoked potentials to different stimulus rates in parkinsonian patients.

Twenty-five Parkinson's disease (PD) patients were studied by brainstem auditory evoked potentials (BAEP) with increased stimulus rate (ISR) and compared to a control age and hearing matched group. A second comparison was made between L-dopa-treated and untreated PD patients. The results of the study suggest that there is subclinical involvement of the auditory brainstem in PD patients, possibly due to the dopamine influence upon the blood vessels. Our results also indicate that dopamine is probably not involved in the synaptic transmission along the auditory pathway of the brainstem.

Unusual sites of schwannoma in the head and neck.

Two cases of schwannoma originating in unusual sites in the head and neck are reported. To the best of our knowledge ours is the first case of schwannoma in the nasopharyngeal area to be described. Schwannomas in the parotid area must be kept in mind in the differential diagnosis of parotid tumours. The pertinent literature is reviewed.

[Microbial contamination of ABO antigens in bone tissue].

Fragments of human bones were stored in different media for two years and then expression of the ABO antigens was indicated. Simultaneously, microbiological investigations were performed. In almost all cases, different ABO substances were detected in putrefied and fresh bones taken from the same person. Blood group antigens found in putrefied bones were compared with serological activity of bacterium cultured from these tissues. Attempts were made to remove unspecific reactions. The authors assume that bacteria are responsible for nonspecific serological reactions, not only as a carrier of blood-group-like substances, but also as a source of enzymes responsible for changes in the structure of ABO antigens in putrefied bones.
