Monitoring of fracture calluses with color Doppler sonography.

PURPOSE: Fracture callus formation is closely associated with vascular invasion, and the use of color Doppler sonography has been suggested as a means to monitor, earlier than gray-scale sonography, the first stages of the healing process. We report the findings in a series of patients with tibial fractures in whom both gray-scale sonography and color Doppler imaging were employed to monitor new bone formation at the fracture site. METHODS: Twenty patients with tibial fractures treated with external fixator frames were examined sonographically about 10 days after surgery and then about every 25 days until radiographic demonstration of consolidation. RESULTS: Eighteen of 20 patients had a well-developed callus, while the remaining 2 patients showed delayed fracture healing. In patients with normal callus development, color Doppler imaging demonstrated the progressive formation of new vessels until about 100 days from the surgery; at subsequent examinations, flow signals decreased, and bone remodeling was confirmed by conventional radiography and gray-scale sonography. The resistance indices in these patients tended to decrease in the early weeks after surgery and then slightly increased. In contrast, lack of development of flow signals and persistence of high resistance indices were observed in the 2 patients with delayed fracture healing. CONCLUSIONS: Color Doppler sonography seems to have the capability to predict whether the development of fracture calluses will be normal or delayed.

The effect of N2O on the oxyhemoglobin dissociation curve is mediated by CO2.

In order to test whether the previously reported effect of N2O on the oxyhemoglobin dissociation curve (ODC) depends upon the presence of 2, 3 DPG or CO2, the blood of healthy subjects was balanced in microtonometers at 37 degrees C with gas mixtures containing N2O but CO2 depleted. Samples of the same blood were exposed to the above gases after incubation at 37 degrees C for 24 hours in order to deplete them of 2, 3 DPG. After a 10 minute tonometry, the hemoglobin saturation and the pH were measured. The pH was corrected into 7.4.P50 increased 5.7% in fresh blood and 5% in 2, 3 DPG depleted blood after samples exposure to N2O. No effect of N2O on P50 was found in samples exposed to gas without CO2. The authors conclude that the effect of N2O on the ODC is mediated by CO2.

[Estrogen receptors in the lower uterine segment].

The presence and quantity of specific cytoplasmatic receptors for E2 were analysed in the smooth muscle cells of the lower uterine segment in 14 patients (mean age 29.5 years, mean parity 2.9) who had a normal pregnancy and delivered at term. Myometrial tissue (greater than 150 mg) was taken from the median zone of the front wall of the lower uterus during cesarian section with a transversal incision on the lower segment at the level of the vescico-uterine fold. Cesarian section was electively chosen by 10 patients before the onset of labour and was used in 4 cases due to complications arising during labour. In order to carry out the receptor assay 100 ml of cytosolic fraction were incubated for 24 h with increasing concentrations of 0.38 microCu/ml to 2.5 microCu/ml of E2 tired in presence or absence of approximately 100-fold higher concentrations of unlabelled E2. Recorded cpm were compared to a standard reference curve. There was no significant difference between the radioactivity levels of the cytosolic fraction measured as total bound or with non-specific bound. Results are fully discussed in the paper.

[Evaluation of the liberation of urinary necrosis enzymes (NAG-AAP) after administration of plasma expanders: study of dextran 40, hydroxyethyl starch and polymerized gelatin].

We studied the 24-hour urinary elimination of enzymatic markers of renal tubular necrosis (NAG-AAP) in 21 patients (mean age 34.6 years old) who were treated with plasma expanders before peridural anesthesia. The patients were divided into three groups of seven subjects each: - 14 ml/Kg -1 of dextran 40 was administered to group 1 - 14 ml/Kg -1 of gelatin was administered to group 2 - 14 ml/Kg -1 of hydroxyethyl starch was administered to group 3. Urinary elimination of N-acetylglucosaminidase and of alanine aminopeptidase was determined in the 24-hour urine the day before surgery (controls), the day of surgery (G1) and the day after surgery (G2). The values of the samples, taken after plasma expander administration, did differ significantly from the control values (G1, G2). Therefore the administration of 14 ml/Kg -1 of dextran, gelatin or hydroxyethyl starch does not affect the renal tubular epithelium.

[Hormonal and psychophysiologic responses under conditions of experimental stress in subjects with essential arterial hypertension].

Two sex balanced groups of 6 subjects each (hypertensive patients vs normotensive subjects) were monitored for both biochemical and psychophysiological variables under stress (PRA, Adrenal Corticosteroid, Aldosterone, HR and Blood Pressure). A computed controlled RT procedure was employed as stress stimulation. The results suggest that different coping mechanisms are present in hypertensives and normotensives. Hypertensives were more emotionally involved and less efficient in stimulus habituation. Biochemical mechanisms have been hypothesized to explain these differences.

Parity and sexual behavior in pregnancy.

Correlation between parity and sexual behavior during pregnancy was studied in 106 pluriparous and 69 primiparous patients. Sexual desire, frequency of coitus, frequency of orgasm, which partner initiated sexual activity, and level of sexual satisfaction were analyzed in the year before pregnancy and during pregnancy. Sexual desire, frequency of coitus, and sexual satisfaction were qualitatively equal in both groups. The frequency of coitus in particular decreased progressively in the three trimesters of pregnancy and more evidently in the pluriparous. The partner who initiated sexual activity most frequently was the male in the pluriparous group.

Immune therapy of osteoarthritis: an open assessment of clinical results with heterologous antibodies to articular tissue ('Serocytol').

Forty-three out-patients received treatment (6 months in 13 cases of spondylarthrosis, 12 months in 30 cases of gonarthrosis) with heterologous antibodies to articular tissue, given as 1 rectal suppository once every other day for periods of 4 weeks out of every 5. The treatment produced complete freedom from symptoms and signs of the illness in 5 (12%) patients and overall positive therapeutic results in 36 (84%), equally distributed in the disorders of the vertebral column and of the knee. This was accompanied by a significant improvement, both in terms of percentage improvement and the percentage of patients improved, of all monitored symptoms. The percentage improvement was similar for all components of the illness: pain, mobility and inflammatory-reactive component. No local or systemic side-reactions were recorded throughout the observation period, and there were no adverse effects on haematology, haematochemistry or renal function. A high percentage of patients reported a remarkable improvement in their quality of life, an overall measure of the therapeutic benefit-risk ratio as experienced by the individual patient.

LM 427, a new spiropiperidylrifamycin: in vitro and in vivo studies.

The spiropiperidylrifamycin LM 427 (4-deoxo-3,4-[2-spiro-N-isobutyl-4-piperidyl]-(1H)-imidazo-(2,5-dihydro) rifamycin S) displays a broad spectrum of potent antibacterial activity in vitro. In vivo it is particularly effective in the therapy of experimental tubercular infections of mice. Three schedules of treatment were employed and the best results were obtained when intermittent administrations were used (ED50 of LM 427; 7 times lower than rifampicin). LM 427 is well distributed in tissues of mice and rats, with lung concentrations 10 to 20 times higher than plasma levels.
