Varicella vaccination in children after bone marrow transplantation.

Herpes zoster (HZ) is one of the most common complications after bone marrow transplantation (BMT) in children. Apart from treatment with antiviral drugs, effective prevention by active immunization with varicella-zoster virus (VZV) appears to be possible. In this study 15 patients were vaccinated with a live attenuated VZV vaccine (Varilrix) 12-23 months after BMT. The vaccine was well tolerated without adverse reactions. Chickenpox or HZ were not observed for up to 2 years after immunization. Eight out of nine seronegative patients seroconverted and in six virus-specific IgG could still be demonstrated 2 years later. The incidence of VZV diseases in 133 non-immunized children after BMT was 26.3%. Infections usually occurred within 18 months after BMT.

Detection of varicella-zoster virus in congenital varicella syndrome: a case report.

BACKGROUND: We studied the possibility of detecting varicella-zoster virus in formalin-fixed tissue samples from a still-born infant with congenital anomalies in order to verify the relationship with maternal varicella. CASE: A girl with hypoplasia of extremities, skin lesions, and microphthalmos was stillborn at 34 weeks' gestation. Her mother had had chickenpox between the 13th and 15th gestational weeks. Varicella-zoster virus DNA could be detected in formalin-fixed tissue samples of lungs, spleen, adrenal glands, bulbus oculi, and placenta by polymerase chain reaction (PCR). The method, with the use of primers from gene 29 encoding the major DNA-binding protein, proved to be highly sensitive. In addition, varicella-zoster virus DNA/antigens were localized in some organs by in situ hybridization/monoclonal antibodies. CONCLUSION: The PCR method should be included in the diagnosis of congenital varicella syndrome. The varicella-zoster virus can be detected in formalin-fixed tissue samples using this technique.

[Varicella zoster virus infections in pregnancy].

Varicella in pregnancy are a rare event, but they may result in severe disease of child. During the first and second trimenon they may cause congenital defects, so-called congenital varicella syndrome. Primary infections with the varicella-zoster virus are also a risk shortly before or during delivery because of intrauterine transmission of the virus with subsequent connatal varicella. On the basis of own clinic-virological investigations there is reported on 27 women and their newborns affected with varicella during pregnancy. Measures for diagnosis, prophylaxis and therapy in the case of congenital varicella syndrome as well as connatal varicella are discussed.

[Detection of Epstein-Barr virus in the tonsils in infectious mononucleosis].

Tonsils of 50 patients with infectious mononucleosis were examined for the presence of Epstein-Barr virus nuclear antigens (EBNA) and in 11 cases for the presence of Epstein-Barr virus (EBV) nucleic acid sequences. In tonsillar tissue of 42 patients less than 1 to 40 per cent EBNA-positive cells could be demonstrated by anticomplement immunofluorescence. 10 out of 11 tonsils examined by in situ hybridization contained less than 1 to 50 per cent cells with EBV nucleic acid sequences. The histological examination indicated that cells labelled by in situ hybridisation are in the majority proliferating B-lymphocytes and to a small extent cells of tonsillar epithelium.

[Prevalence of varicella zoster virus in pregnant patients].

The prevalence of IgG antibodies to varicella-zoster (VZ) virus was estimated by indirect immunofluorescence test in a sero-epidemiological study in 295 healthy pregnant women at the age of 17 to 36 years. 93.2% were considered to be seropositive and consequently protected against varicella. No antibodies to VZ virus were detected in 6.8%. Owing to the lacking protective antibodies the occurrence of maternal varicella has to be taken into account.

[Microbiological and morphological findings in reactive lymphadenopathy].

An attempt was made by means of microbiological and histomorphological methods for diagnostic elucidation of reactive lymphadenopathies in 52 patients. The serological findings differed from those recorded from clinically intact individuals, in that they included not only increased prevalence of antibodies but also increased antibody titres to cytomegalovirus, influenza virus, and toxoplasma gondii. The dynamic of antibody titres against antigens specific of herpesvirus, influenza virus, and toxoplasma gondii were followed up and enabled positive detection in 11 of 18 patients of active infections with the above pathogens involved. Histologically, the lymph nodes affected were characterised by alterations reminiscent to Piringer lymphadenitis. While, in the course of bacteriological investigations, various bacterial species were isolated from lymph node tissue of 35 patients, direct morphological identification from lymph node sections failed to work. Immature sinus histiocytosis and an increase in phagocytosis activity were among the typical tissue findings.

Therapeutic effect of (E)-5-(2-bromovinyl)-2'-deoxyuridine, caffeic acid oxidation product, and trisodiumphosphonoformate on cutaneous herpes simplex virus type 1 infection in guinea pigs.

The influence of (E)-5-(2-bromovinyl)-2'-deoxyuridine (BVDU), caffeic acid oxidation product (KOP), and trisodiumphosphonoformate (TPF) on the course of the primary cutaneous herpes simplex virus infection was investigated by means of a guinea pig test model. The antiviral substances were applied in an ointment with 10% urea as a penetration mediator. When the treatment was initiated 15 minutes after virus inoculation, 3% BVDU effectively inhibited the development of herpetic vesicles and 0.1% BVDU prevented the appearance of herpetic satellites. Under the same conditions 1% and 3% KOP ointments inhibited the appearance of satellites; and 0.5% TPF ointment completely inhibited the development of cutaneous herpes lesions. Prophylactic drug administration given 24, 20, and 4 hours before virus inoculation was without any protective effect.

[Detection of Epstein-Barr virus nucleic acid sequences in non-Hodgkin's lymphoma].

Tumor tissue specimens of 25 patients with various entities of Non-Hodgkin's lymphomas were examined for the presence of Epstein-Barr virus (EBV) nucleic acids and EBV nuclear antigens (EBNA) by in situ hybridization and anticomplement immunofluorescence. In tumor cells of five patients EBV nucleic acid and EBNA were demonstrated. The histopathologic examinations revealed in these cases one Burkitt's lymphoma, two centroblastic lymphomas and each one lymphoplasmocytic and lymphocytic lymphoma.

Demonstration of Epstein-Barr virus in malignant non-Hodgkin's lymphomas.

Lymph nodes or tumor biopsies of 60 persons suspected of having a malignant lymphoma were examined for the presence of Epstein-Barr virus nuclear antigen (EBNA) by anticomplement immunofluorescence. In 8 cases the tissue specimens were also assayed for Epstein-Barr virus (EBV) DNA by nucleic acid hybridization. Serum samples of patients and controls were tested for EBV-related antibodies. The histological tests in 37 cases showed a malignant non-Hodgkin lymphoma, and in 23 cases a reactive lymphadenopathy. A Burkitt lymphoma of a European boy and a polymorphic centroblastoma contained EBNA and approximately 27 or 30 genome equivalents EBV DNA per cell, respectively. EBNA was also demonstrated in about 20% of the cells of a lymph node from a patient with recurrent reactive lymphadenopathy.

Comparative detection of herpesviruses in tissue specimens by in situ hybridization and immunofluorescence.

The conditions of in situ hybridization for demonstration of herpesvirus genomes in animal and human tissues were tested using the ORWO(R) K6 emulsion. It was possible to localize herpes simplex virus (HSV) genomes in infected mice organs (brain and liver) as well as Epstein-Barr virus (EBV) genomes in tonsils of patients with infectious mononucleosis and in tumour specimens of patients with nasopharyngeal carcinomas. Immunofluorescence (IF) revealed mostly corresponding results. The in situ hybridization is more favourable due to its higher specifity, but it is more time consuming and expensive. IF seems advantageous for screening of a great number of tissues.

[Antibody formation against specific Epstein-Barr virus antigens in infectious mononucleosis with tonsillectomy in the acute phase of the disease].

In the present study, the clinical course and the specific humoral immunological response to Epstein-Barr virus antigens have been investigated in patients affected with infectious mononucleosis after tonsillectomy in the acute phase of the disease, compared against conservatively treated patients. Clinical results confirm that tonsillectomy in the acute phase of infectious mononucleosis has a favourable effect on the course of the disease, and reduces the duration of the disease by about half of the usual time. Statistically significant differences - indicating a confined, delayed humoral immunological response or unresponsiveness to some extent - regarding the examined antigens after tonsillectomy in the acute phase of infectious mononucleosis, could be observed between the two groups of patients in respect of the production of antibodies against viral capsid and nuclear antigens of the Epstein-Barr virus and heterophilic antibodies. For this reason, tonsillectomy should be suggested only as therapy of infectious mononucleosis in anginous courses of the disease which appear life-threatening.

Antibodies against herpesviruses in patients with Hodgkin's disease.

The antibody titres to Epstein-Barr virus (EBV), cytomegalovirus (CMV), herpes simplex virus (HSV) and varicella-zoster virus (VZV) were determined in 57 adult patients with Hodgkin's disease and a group of age- and sex-matched controls. In the group of patients, the geometric mean antibody titres to EBV capsid antigen, CMV early and late antigens, and also VZV, were significantly higher than in the healthy controls. Antibodies to early antigens of EBV and CMV were found more commonly in patients than in controls. The raised antibody titres to herpesviruses were taken to be the result of the disturbed cellular immunity in Hodgkin's disease.
