Pleomorphic xanthoastrocytoma associated with von Recklinghausen neurofibromatosis.

The authors present a case of pleomorphic xanthoastrocytoma that occurred in a 14-year-old boy with von Recklinghausen's neurofibromatosis. The circumscribed tumor with a cystic component was located in the medial right temporal lobe. The designation of pleomorphic xanthoastrocytoma has been suggested for this neoplasm on the basis of its unique histological features. These include positive glial fibrillary acidic protein staining.

Magnetic resonance imaging in the diagnosis of idiopathic giant-cell granulomatous hypophysitis: a rare cause of hyperprolactinaemia.

Idiopathic giant cell granulomatous hypophysitis is a rare disorder of pituitary gland characterised by a chronic inflammatory process. It can also be an extremely rare cause of hyperprolactinaemia. In this paper, we present our experience with two cases of idiopathic giant cell granulomatous hypophysitis manifested by hyperprolactinaemia, and their neuroradiological evaluation including preoperative MRI studies in one of them, and discuss our findings in the light of the literature.

Cerebral phaeohyphomycosis.

A 14-year-old girl with a rare form of fungal brain abscess involving the dura and brain parenchyma is reported. No portal of entry of the infection was found. Histological findings and fungal culture both indicated that the causative agent was Xylohypha bantiana, an uncommon dematiaceous fungus. A review of the literature suggests that this infection, which affects primarily young male patients, exhibits distinct neurotropism. Despite therapy, the prognosis is generally poor.

A pontine tuberculoma mimicking an exophytic brain stem tumor.

A 21 year old man with a five month history of facial paresis, diplopia, and gait disturbance diagnosed with a brain stem tumor at another institution was evaluated. Computed tomography, magnetic resonance imaging, clinical and laboratory evaluations suggested a pontine astrocytoma, and the mass was excised. Pathological diagnosis was a tuberculoma confirmed by postoperative positive Mantoux testing. CT and MRI appearances of tuberculomas have been described, but can not differentiate them completely from other tumors such as astrocytomas. Because the incidence of tuberculomas has increased in recent years and they may mimic exophytic tumors of pons, it is important to include pontine tuberculoma in the differential diagnosis of pontine tumors.

The effect of iloprost and NDGA in ischemia reperfusion injury in rat liver.

In this study, the effects of iloprost (ZK 36374) and NDGA on warm ischemia and reperfusion injury in rat liver were investigated. Rats were given isotonic saline (control group), iloprost 25 micrograms/kg i.v. (group II) just before warm ischemia or NDGA 10 micrograms/kg i.v. (group III) 5 min before reperfusion or the same drugs were given together (group IV). Serum SGOT, SGPT, and LDH values and tissue malondialdehyde (MDA), glutathione (GSH), prostaglandin (PG)E2, and leukotriene (LT)C4 levels were determined after ischemia-reperfusion injury. Histopathologic examination of the liver was carried out under the light microscope. The serum SGOT, SGPT and LDH levels improved significantly in groups II, III, and IV when compared with the control group (p < 0.05). There was a significant decrease (p < 0.05) in tissue MDA levels and significant increase (p < 0.05) in tissue GSH levels in group I, when compared with group IV and the control groups. The values did not differ significantly in group IV when compared to controls. The LTC4/PGE2 ratio was low and histologic findings were worse in group III. In conclusion, iloprost was found to be beneficial in preventing the ischemia-reperfusion injury in the rat livers. NDGA, either by direct toxic effect or by shifting the arachidonic acid metabolism to the cyclooxygenase route, was not found to be as effective. Iloprost and NDGA did not exert a synergist effect.

Cytoprotective effect of iloprost on isolated cortical brain tissue grafts in rats.

The cytoprotective effect of iloprost was studied on isolated embryonic cortical brain tissue grafts of rats, using light and transmission electronmicroscopy. The brain tissue pieces were stored either in saline or 50 ng/ml iloprost solution for 30 minutes, 3, 6, 24 hours at +4 degrees C. It was demonstrated that iloprost significantly protected the neuronal integration of the tissue pieces compared with saline preserved pieces. Tissues preserved in iloprost showed only minimal dissolution of the tissue with minimal extracellular edema only in the later stages of preservation. The mechanism of action of the cytoprotective effect of iloprost is discussed.

Chondroma of the cervical spine.

Chondroma is a benign cartilaginous tumor and rarely seen as a spinal cord tumor. The authors report a case of cervical chondroma with cord compression signs. After the total surgical removal of the lesion neurological recovery was seen, but one year later recurrence occurred with no sarcomatous transformation.

Tumor in tumor: metastasis of breast carcinoma to intracranial meningioma.

This report presents magnetic resonance imaging (MRI) findings of a breast carcinoma metastasis in an intracranial meningioma with correlated pathological findings. MRI showed multiple foci of intense enhancement with hypointense surrounding areas. The described foci appeared to be metastatic disease from the patient's known breast carcinoma. In addition, this is the first study reported in the literature to have investigated the expression of a possibly common carcinogenic molecule in breast carcinoma metastatic to a coexisting meningioma: overexpressed c-myc oncogene was found both in the breast carcinoma compartment and in the meningioma component of the tumor.
