Enzymic acylation of methyl D- and L-glycopyranosides: influence of the 3-hydroxyl group.

Porcine pancreatic (PPL), Candida cylindracea (CCL) and Pseudomonas cepacia (LPS) lipases, suspended in organic solvents, were used to regioselectively acylate methyl 6-O-butyryl-alpha-D- and L-allopyranosides and methyl 6-O-butyryl-3-deoxy-alpha-D- and L-ribo-hexopyranosides. Both the D- and the L-3-deoxy sugars showed a complete regioselectivity, while the reactions of the allosides proved to be less regioselective. This indicates that the presence of the hydroxyl group at C-3 is an unfavourable factor for the action of the lipases.

[Pneumothorax in multiple trauma. Radiologic and CT study].

This study was aimed at evaluating the necessity to perform chest Computerized Tomography (CT) in multiple traumatized patients to diagnose pleuropulmonary lesions and, particularly, pneumothorax: the correct identification of this condition, although minimal, is important especially in prevision of long anesthesias and/or positive end-expiratory pressure (PEEP) therapy. This assisted respiratory technique improves arterial oxygenation but causes a barotrauma which may cause some complications; particularly, a small undetected pneumothorax can suddenly increase so as to cause pulmonary collapse with sometimes dramatic symptoms. Chest X-ray films and CT scans, performed in rapid succession on patient's admission in Emergency Ward, were compared in 21 subjects. CT is indispensable in case of severe chest parietal lesions which can mask the radiological evidence of pulmonary or pleural conditions, as it occurred in 3 of our cases. Moreover, CT resulted more reliable than chest X-rays (18 versus 10 correct diagnoses) especially in the detection of small antero-inferior pneumothorax flaps, in which direct radiological signs cannot be identified, in default of radio-geometrical assumptions. Indirect radiological signs of pneumothorax must be recognized but critically considered in order to avoid over-staging.

Quantitative photoacoustic spectroscopy of cataractous human lenses.

Quantitative photoacoustic spectra of the nuclei of cataractous human lenses with various degrees of colouration and opacification were measured in the spectral range 250-600 nm. The lens nuclei were obtained from 20 cataractous patients through extracapsular cataract extraction. These measurements yield the light loss per unit path length in the nucleus of cataractous lenses.

[Buildings for mental patients: from the architecture of isolation to the architecture of integration].

Traditional psychiatric hospitals designed for several thousand patients are very far from contributing to rapid recovery of the patients. Current investigations have demonstrated that such hospitals should be replaced by small rehabilitation centers designed for a minor number of patients (several dozens). The architecture of new psychiatric hospitals should fully comply with the treatment purposes. Thus, the hospital should have little comfortable wards, a large number of rooms for recreation purposes as well as provide accommodation for meetings with relatives and friends. Psychiatric hospitals should represent small institutions of the familial type.

Simultaneous determination of pyrimethamine and mefloquine in human plasma by high-performance liquid chromatography with ultraviolet detection.

A rapid, sensitive and selective method was developed for the simultaneous determination of pyrimethamine and mefloquine, two of the active ingredients of Fansimef, in human plasma. The procedure involved extraction of the compounds and the internal standard nitrazepam from basified plasma with dichloromethane and chromatography on a C18 column (microBondapak, 300 X 3.9 mm I.D.) with acetonitrile-phosphate buffer as the mobile phase and UV detection at 222 nm. The limit of quantification was 10 ng/ml for both substances, using a 1-ml plasma specimen. The mean inter-assay precision was 2.8% for pyrimethamine and 4.7% for mefloquine up to 800 ng/ml. The practicability of the method was demonstrated by the analysis of more than 1200 plasma samples from several pharmacokinetic studies involving single-dose administration of Fansimef to both patients and volunteers.

Posterior chamber intraocular lens implantation in patients with retinitis pigmentosa.

20 eyes of 12 Retinitis Pigmentosa patients underwent extracapsular cataract extraction followed by I.O.L. implantation. No unusual intra or post operative complications were encountered. All pseudophakic eyes showed improvement in visual acuity whereas the V.F. remained unaltered. Subjectively all 12 patients reported a benefit from surgery despite the Visual Field impairment.

Synthesis of 14 beta-cholesta-5,7-dien-3 beta-ol.

5,7-Cholestadien-3 beta-ol was transformed into 14 beta-cholesta-5,7-dien-3 beta-ol in six steps. The inversion of the stereochemistry at C-14 was obtained by a selective protection of the delta 5 and the elaboration of the delta 7 double bond.

The impact of Psychiatric Reform on inpatient services in a metropolitan area of Southern Italy: the Naples case.

Six years after the Psychiatric Reform, phasing out of Mental Hospital and implementing a community based Mental Health System, i.e. the main goals of the Reform, are not yet performed in Southern big cities of Italian peninsula. In Naples metropolitan area this new Mental Health policy faces with a dramatic lack of community services. This paper summarizes data concerning the trends in psychiatric admissions to some inpatient public settings of Naples before and after the Reform, with the intent of pointing out the short-time outcomes of this law in a highly populated area. The Authors stress some positive results of the Reform in the study area, as well as the factors which have contributed to the inadequate application of the new law.

[Proposal of an echotomographic method of guided portal catheterization].

To identify the frontal plane on which to find the right branch of the portal vein in 33 patients who underwent transhepatic percutaneous portography, we used a real-time ultrasonic scanner improved with an original device conceived for this specific need. The suggested method reduced the number of the ineffectual punctures of the liver, allowing in nearly all cases the cannulation of the right branch on the first attempts.

Endopolygalacturonase from Rhizoctonia fragariae. Purification and characterization of two isoenzymes.

An electrophoretically homogeneous preparation of endo-polygalacturonase (poly(1,4-alpha-D-galacturonide)glycanohydrolase, EC 3.2.1.15) from culture filtrates of Rhizoctonia fragariae, a pathogenic agent in strawberry plants, was resolved into two isoenzymes when subjected to isoelectrofocusing in a narrow pH range. The isoelectric points of the two isoenzymes were 6.76 +/- 0.03 and 7.08 +/- 0.05. The two polygalacturonases exhibited similar substrate specificity, pH optimum and pattern of degradation of sodium polypectate. The two enzymes consisted of a single polypeptide chain which had an apparent molecular weight of 36 000 as determined by gel filtration on Sephadex G-100.
