[Sensitivity and specificity of sonography and sialography in relation to the diagnosis of parotid tumors].

The results of parotid sialography from a group of patients between 1980 and 1982, were subjected to retrospective evaluation, and compared with sonographic results from another group between 1983 and 1985. In the first period studied sialography showed 75% sensitivity and 100% specificity. Both sensitivity and specificity of sonography were 100% in the second group. Thus, sonography appears superior with regard to diagnosis of intraglandular tumours of the parotid.

[Diagnostic roentgen radiation exposure of clinical ENT patients].

The determination of permissible exposure to x-rays of our patients' during by diagnostic radiology was measured by ionometer, and compared with cosmic, natural and technical x-rays. The assessment of risk for the individual patient is difficult or only possible by using the genetic doubling dose. The exposure due to a single exposure is given in tables.

[Ultrasound diagnosis as a complement to sialography].

Over a period of one year, 82 patients with chronic inflammations, concrements, abscesses, cysts, Warthin's tumours, pleomorphic adenomas, lymphonodules and neoplasms of the salivary glands were examined by ultrasound and sialography. Morphological features are described, and the validity of diagnosis by ultrasound and sialography is discussed.

[Demonstration of normal and pathologic ossicle structures with a new direct roentgen magnification technic].

With a new l mu Focal spot tube, which allowed high grade direct radiographic magnification, we examined the structures of ossicles, taken from normal and pathological middle ears. Bony details that up to now could only be seen with histological or microscopic procedures, can be shown in the intact, untreated object. Some examples demonstrate this new technique.

[A new direct roentgen magnification technic and its possible applications in otorhinolaryngology].

The development of a new tube with a 1 mu focal spot size made it possible for the first time, to enlarge objects with a direct radiographic magnification technique in microscopic dimensions. Osseus structures that up to now could only be demonstrated by histological techniques, are visualised without any preparation and hence without destroying the object. A possible clinical application was investigated by means of a soft tissue phantom skull. Although clinical usefulness seems to be limited to special indications, this new technique shows some interesting and promising aspects for experimental research.

[Use and problems of high-resolution computed tomography of the petrous bone].

High resolution computed tomography of the petrous bone is not comparable to conventional tomography. Due to the different construction of both scanners the patient positioning techniques are different and conventional otoradiology projections are not applicable in computed tomography. We consider that high resolution computed tomography is only indicated in rare cases of temporal bone tumours and congenital anomalies.

[Artefacts in the CT scan caused by surgical clips and central venous catheters and the use of new materials].

When metal clips and venous catheters have been used the computerized tomography shows artifacts, which make it computerized tomography shows artifacts, which make it difficult to have a reliable and accurate diagnosis, because the X- raying of the metal clips and the venous catheter gives radiating lines, so called streaking. We have tested new materials for the metal clips and a new polymer to replace the substance of the venous catheter with the intention that the materials should have the same density as the normal tissues. With a new metal for the clips and a new polymer for the venous catheters, mixed with metal ions, the computerized tomography does not show streaking.

[Sipple syndrome as a differential diagnosis in recurrent laryngeal nerve].

Two cases of Sipple's syndrome with the typical signs of medullary carcinoma of the thyroid, pheochromecytoma and parathyroid adenoma are presented. Cushing's disease and marfanoid habitus were demonstrated. They are classified as multiple endocrine adenopathy type 2A and 2B according to the modern nomenclature.

[Bilateral skull crush trauma].

4 cases are presented of bilateral injury of the skull base with transverse fractures of the middle cranial fossa. The pathogenesis of these fractures is discussed, pointing out the typical patterns of these lesions.

[Clinical course and prognosis in medulloblastoma in infants and young children].

From 1962 until 1981 six children with histologically assured medulloblastoma younger than three years of age have been treated at Münster University Clinics. The interval from the onset of symptoms until the clinical admission lasted six weeks, a very short time compared with the admission interval of adults. The survival time after admission was only sixteen days in average. All children died because of central disregulation due to their disease.

[Value of sialography in the diagnosis of parotid and submandibular gland tumors].

40 patients clinically suspected of a tumor of the parotid gland, and 2 patients suspected of tumor of the glandula submandibular, were subjected to fractionated sialography at the Radiological Clinic of Münster University in the course of five years. In 31 cases (74%), the tumor was confirmed. In 11 patients (26%) no suspicion of tumour was possible even on retrospective examination of the x-ray films. Hence, the value of sialography is more in the direction of pre-operative exclusion of other types of disease, especially of stone formation. However, it is to be expected that the introduction of axial CT sialography will significantly improve the accuracy of tumour diagnosis, because this method enables examination and assessment of the glandular parenchyma besides an examination of the parotid duct.

Adenolymphoma↗

[Occurrence and prognosis of medulloblastoma in adult persons].

Between 1962 and 1981, 37 patients with histologically proved medulloblastomas have been treated at the hospitals of the University of Münster. 17 patients died immediately after operation or were, when admitted, in such a bad condition that any therapy was impossible. Since about 1972, the therapy of choice is the operation and post-irradiation of the neurocranium with saturating irradiation of the posterior cranial fossa as well as irradiation of the complete cerebrospinal system down to the second sacral vertebra, because the tumor tends to spinal formation of metastases. Among our patients, twelve were treated in this manner. Before this time, patients were only operated and submitted to an irradiation of the posterior cranial fossa. Eight patients were treated according to this incomplete therapy scheme, five of whom were younger than twelve and three older than fourty years. The tumor was situated in one side of the cerebellum in the three adult patients. The children had a shorter survival time than the adults, except one four years old child who survives already 228 months after the treatment and has to be considered as cured. Our medical records showed at the same time that the course of this disease is considerably worse in infants up to three years than in older patients.

[Computed tomographic diagnosis of epidural abscess, subdural empyema, meningitis and brain abscess].

Computerised tomography cannot be of great help in diagnosing meningitis. Examination of the cerebrospinal fluid remains essential. After the inflammation of the meninges has progressed to some stage of encephalitis, the formation of an abscess can be located via computed tomography. It is characterised by the ring-type abscess capsule. Computed tomography for diagnostic purposes is superior to cerebral scanning, which demonstrates enhanced activity, but does not show the formation of a membrane, so essential for differential diagnosis. Furthermore, computed tomography shows the adjacent anatomical structures and answers the questions of displacements and threatening invasion of the ventricle system. Epidural and subdural abscesses can also be located by computed tomography. Therapy can begin directly after computerised tomography, whereas in scintigraphy only a non-specific enhanced activity is present, which often does not allow differentiation between epidural and subdural location.

[CT diagnosis of glomus-jugulare tumours].

Glomus-jugulare tumours (chemodectomas, non-chromaffin paragangliomas) are semi-malignant tumours arising from the glomus structures of the jugular bulb. They are soft tissue tumours which usually expand the jugular foramen and destroy its bony margins. Conventional methods of examination are therefore very effective. However, they only demonstrate the bone destruction and are therefore an indirect method for showing the tumour. The actual size and extent of the tumour, which are important for the surgeon or radiotherapist, cannot be demonstrated by simple x-rays. Computer tomography is very effective in the diagnosis of soft tissue lesions in the skull. CT is therefore of great importance in the diagnosis of glomus-jugulare tumours.

[CT diagnosis of the carotid cavernous fistula and differential diagnosis (author's transl)].

One of the many urgent cases requiring immediate attention is the possibility of continuous bleeding into the sphenoidal sinus via a carotid cavernous fistula (CCF). Computed tomography offers a necessary and quick diagnostic method for such soft-tissue structures. Difficulty in CT diagnosis is enhanced by the limitations inherent in certain diagnoses. The article shows that safe diagnosis can be achieved in acute postoperative CCF with great shunt volumes. When diagnosing spontaneous cases we must consider the clinical data to differentiate CCF from other causes.

Arteriovenous Fistula↗

[Therapy and prognosis of medulloblastoma].

Between 1962 and 1981, 37 patients with histologically proved medulloblastomas were treated at the Radiologic Hospital and the Children's Hospital of the University of Münster. Because of the inadequate treatment over this long period, the collective was divided into three groups. The first group comprised 18 patients, 15 of whom died after having been operated, so that they could not be submitted to a sufficient radiotherapy. Two patients were unconscious when admitted; they died without therapy. One patient was submitted to a primary palliative irradiation with 10 Gy. The second group comprised seven patients who received only a postoperative irradiation of the skull. One of these patients is alive since 228 months without any recurrence. The other six patients had an average survival time of 9,2 months. The third group comprises twelve patients who were submitted to an irradiation of the skull and an additional irradiation of the whole cerebrospinal axis. Nine of these twelve patients were also treated by chemotherapy according to the SIOP scheme. Their average survival time is 28,4 months. Five patients are still alive and have no recurrences or metastases. The best survival chances actually seem to be provided by a radiotherapy of the whole cerebrospinal axis combined with chemotherapy.
