Pharmacokinetics and residual behaviour in milk of oxytetracycline in cows following administration of uterine pessaries.

The plasma kinetics and residual depletion in milk of cows treated by the intrauterine route with pessaries containing oxytetracycline (OTC) were evaluated. The antibiotic was administered to five healthy Friesian cows at a dosage of 3g/head in the early post partum phase. Blood samples were collected before and at different time intervals (3, 6, 12, 24, 48, 72, 84, and 96 h) after treatment. Milk was drawn before treatment and at 12-h intervals for 4 consecutive days. Samples were analysed by a high-performance liquid chromatography method and the pharmacokinetic parameters were processed using the minimum Akaike information criterion estimation (MAICE) test. The mean values obtained indicated a relatively low area under the concentration time curve (25.19+/-12.61 microg/mg per h) and maximum plasma concentration (Cmax) (0.549+/-0.278 microg/mL) with delayed time to Cmax (11.71+/-4.15 h) and elimination half-life (21.96+/-4.42 h). A similar pattern could be shown for milk, in which measurable residual levels are found in two out of five animals until the 72nd hour after treatment. Data obtained demonstrate that OTC administered as a solid form is poorly and slowly absorbed from the uterus of cows.

Use of alprazolam for relief of tinnitus. A double-blind study.

OBJECTIVE: To systematically test the effectiveness of alprazolam as a pharmacological agent for patients with tinnitus. DESIGN: Prospective, placebo-controlled, double-blind study. PATIENTS: Forty adult patients with constant tinnitus who had experienced their tinnitus for a minimum of 1 year and who resided in the Portland, Ore, metropolitan area. Twenty patients were randomly assigned to the experimental group and 20 to the control group. RESULTS: Seventeen of 20 patients in the experimental (alprazolam) group and 19 of the 20 in the placebo (lactose) group completed the study. Of the 17 patients receiving alprazolam, 13 (76%) had a reduction in the loudness of their tinnitus when measurements were made using a tinnitus synthesizer and a visual analog scale. Only one of the 19 who received the placebo showed any improvement in the loudness of their tinnitus. No changes were observed in the audiometric data or in tinnitus masking levels for either group. Individuals differed in the dosages required to achieve benefit from the alprazolam, and the side effects were minimal for this 12-week study. CONCLUSIONS: Alprazolam is a drug that will provide therapeutic relief for some patients with tinnitus. Regulation of the prescribed dosage of alprazolam is important since individuals differ considerably in sensitivity to this medication.

An implantable bone conduction hearing device.

A new implantable hearing device that transmits an acoustical signal from an external electromagnetic coil to a small magnet implanted in the temporal bone has been developed and implanted in more than 350 patients. Animal research indicates that the device serves as a high-fidelity sound source throughout the audio range. Especially notable is its high-frequency performance. Human experiments using a body processor indicate that the device compares favorably with conventional bone conduction hearing aids, and patients report excellent sound quality as well as improved ability to understand conversational speech. Further development has resulted in an at-the-ear (ATE) processor. Extensive testing of the ATE unit has revealed that current models provide approximately 10 dB less output than the body processors. Further research and development are continuing to address the need for increased power output from the ATE processor and to eliminate other minor problems.

The characteristics and natural history of tinnitus in Meniere's disease.

Four tests for the identification of tinnitus have been presented: pitch, loudness, maskability, and residual inhibition. The typical features of the tinnitus associated with Meniere's disease are as follows: 1. Usually the tinnitus is a low pitched noise. However, in the burned out condition it is often higher in pitch and tonal in quality. 2. The loudness match occurs at very low sensation levels. 3. Masking as a relief procedure is usually easily achieved, requiring only low levels of noise. 4. The masking recommended for relief of tinnitus is found to be acceptable as a substitute for tinnitus. 5. Masking of the tinnitus produces residual inhibition, which frequently is of longer than average duration.

Tinnitus: a new management.

Tinnitus Clinic was established at the University of Oregon Medical School and the result of treatment for 158 patients is discussed. Patients received three forms of masking as treatment: masking from hearing aids, masking from the Tinnitus Masker, and FM-Masking. Conditions leading to each kind of recommendation are discussed. The examination procedures used in the Tinnitus Clinic are presented. The phenomenon of residual inhibition is explained and discussed in relation to possible long-term or permanent relief of tinnitus. Future plans for the clinic and needs of tinnitus patients are presented.
