Neurologic referrals to a psychiatric consultation-liaison service. A study of 199 patients.

The findings in a 199 neurology patients referred to a psychiatric consultation-liaison service are analyzed. Neurology referrals differ from general hospital referrals in reasons for referral and in patterns of diagnosis. The most common conversion symptoms seen are pain and pseudoseizures. The psychiatric diagnosis made in those patients with functional pain varies with the site of the pain. These two conversion symptoms, pain and pseudoseizures, have differing ages of onset and prognoses. A number of patients were given no psychiatric diagnosis and the reasons for this are discussed. The incidence of various neurologic diagnoses made in those patients referred to the consultation-liaison service is compared with the incidence of those diagnoses made in consecutive admissions to the neurology ward.

Liaison psychiatry in an Irish hospital: a survey of a year's experience.

A brief summary of the pattern of mental illness in Ireland is given and the work of a psychiatric consultation-liaison service in a general hospital in the southern part of the country is discussed in this context. The referral rates to the service mirror the referral rates in the United Kingdom more than those in the United States. The high rates of alcoholism and functional psychosis found in referred patients probably reflect the increased prevalence of these conditions in the general community. In Ireland, as elsewhere, the rate of referral of general hospital inpatients to a psychiatric consultation-liaison service is below the rate of psychiatric illness found in hospital inpatients.

Mechanism of late failure of the Alvarez disc valve prosthesis.

An Alvarez disc valve prosthesis had been implanted in the mitral and tricuspid positions in 15 patients between 1964 and 1967 in this hospital. Of these 15 patients, only three survived longer than 10 years, all three dying unexpectedly between 16 and 18 years postoperatively. Two deaths were a direct result of disruption of the Alvarez prosthesis. The third patient was not examined at necropsy, but in retrospect the history was highly suggestive of valve malfunction. Cross sectional and pulsed Doppler echocardiography made valuable contributions to the diagnosis in both cases of valve disruption and, when available, should be part of the primary investigation of suspected prosthetic heart valve failure. The Alvarez valve prosthesis is liable to erosive wear which may cause late detachment of the disc from the valve ring with abrupt and catastrophic haemodynamic consequences.

A study of discomfort and confusion among elderly surgical patients.

Acute confusion is a significant problem among elderly surgical patients, and it can impair the older adult's ability to localize, interpret, or communicate discomfort to care-givers. Discomfort is a common experience for hospitalized older adults, especially those recovering from trauma or surgery. In this study, 36 (78%) of the elderly (> 74 yrs.) patients on an orthopedic/trauma unit were found to be at risk for or acutely confused. Three measures of discomfort were administered to these 36 confused patients by research assistants during routine postoperative nursing activities, such as turning, transfers, or bathing. Acute confusion needs to be viewed as a priority problem for nurses working with elderly surgical patients. Self-report is not a reliable indicator of discomfort in elderly confused patients. It is recommended that health care providers focus on discomfort as they provide care to these elders, and intervene in a preventive fashion.
