Avoiding pitfalls when implementing local area networks in hospital environments.

This paper is intended to outline some of the most commonly encountered, but yet still underestimated pitfalls during the implementation of computer systems networks in hospitals and health care institutions and to give information technology planners and responsibles some practical hints for avoiding them. Pitfalls encountered during the difficult process of achieving consensus among all groups concerned on the necessity for electronic data processing in health care institutions will not be addressed here [1], though the authors believe that some major risks for project failure stem from shortcomings in this field. Instead, those pitfalls encountered during project initialization and project realization with the phases of analysis, design, contracting, installation, and maintenance will be discussed. The paper concludes with the authors' opinion that only the application of industry-proven project management and realization techniques will allow health care administrators to cope with the steadily increasing risks for failure of information technology projects in health care.

Induction of squamous cell carcinomas in the mouse lung after long-term inhalation of polycyclic aromatic hydrocarbon-rich exhausts.

A 10-month inhalation study using newborn female mice was performed to determine pulmonary tumorigenicity of polycyclic aromatic hydrocarbon (PAH)-enriched exhausts. At two exhaust doses containing 50 micrograms/m3 (group II) and 90 micrograms/m3 benzo(a)pyrene (BaP) (group III) a significant increase of lung tumors was observed and the incidence of malignant lung tumors was dose-dependent. Bronchiolo-alveolar adenomas were observed in all mice of groups II and III (40/40 each) as compared to 5/40 in the control (group I). Bronchiolo-alveolar adenocarcinomas developed in 10/40 and in 33/40 mice of groups II and III, respectively, but were absent in group I. Single or multiple squamous cell carcinomas, which showed variable degrees of differentiation were observed exclusively in 6 mice of group III. One adenosquamous carcinoma was seen in an further animal of this group.

[Mercury concentrations in the urine of children with and without amalgam fillings].

Studies on adults documented that most of the urinary concentration in occupationally non-exposed persons is determined by the number and extent of their amalgam fillings. Very few data concerning the mercury exposure of children are available. In this study the mercury concentration in the urine of 3-15-year-old children with and without amalgam fillings was determined. The mean urinary mercury concentration for the children without amalgam (n = 93) was 0.17 microgram/l, and for the children with amalgam fillings (n = 86) it was 0.70 microgram/l. The creatinine adjusted values were 0.19 microgram/g and 0.64 microgram/g, respectively. This difference was significant (p < 0.001). Also, a significant correlation was found between the number of amalgam points (each amalgam surface had been given 1 to 3 points depending on its extent) and the urinary mercury concentration (r = 0.69).

Solvent damping of internal processes in myoglobin studied by specific heat spectroscopy and flash photolysis.

We address the question of dynamic coupling between protein and solvent by comparing the enthalpy relaxation of the solvent (75% v/v glycerol-water) to internal ligand binding in myoglobin. When the solvent relaxation is slow compared to intramolecular events we observe decoupling of protein motions from the solvent. In the opposite limit there is a significant contribution of the solvent to internal friction. The solvent enhances the apparent activation energy of transitions in myoglobin. This result is discussed in terms of a generalized Kramer's law involving a dynamic friction coefficient.

[Oral health in children after dental care under anesthesia].

Carious morbidity, oral hygiene, periodontal health, use of fluoride and therapeutic compliance were investigated in a clinical study of 41 children who had previously required insufflation anaesthesia for dental therapy. All of the children underwent postoperative treatment willingly; anaesthesia was not necessary in any of the cases. Half of the children still had a raised risk of caries; gingivitis was present in all of the children as a result of insufficient oral hygiene. For these reasons, all children that have undergone dental therapy under anaesthesia must subsequently be integrated in a programme of selective intensive prophylaxis.

Conformational substates and motions in myoglobin. External influences on structure and dynamics.

Myoglobin, a simppe dioxygen-storage protein, is a good laboratory for the investigation of the connection between protein structure, dynamics, and function. Fourier-transform infrared spectroscopy on carbon-monoxymyoglobin (MbCO) shows three major CO bands. These bands are excellent probes for the investigation of the structure-function relationship. They have different CO binding kinetics and their CO dipoles form different angles with respect to the heme normal, implying that MbCO exists in three major conformational substates, A0, A1, and A3. The entropies and enthalpies of these substates depend on temperature above approximately 180 K and are influenced by pH, solvent, and pressure. These results suggest that even a protein as simple as Mb can assume a small number of clearly different structures that perform the same function, but with different rates. Moreover, protein structure and dynamics depend strongly on the interaction of the protein with its environment.

[Indications for root canal treatment in patients with clotting disorders].

In a prospective study the indications for root canal treatment and the complication rate were evaluated in 28 patients (51 teeth) with hemorrhagic diathesis. If strict precautions are taken, root canal treatment may be performed in patients with clotting disorders. The effort required to this end, however, seems to be unusually great, and long-term success cannot be guaranteed.

[Epidemiologic studies in 14 to 16-year-old pupils in Lippstadt].

In a cross-sectional survey we obtained figures on the current state of periodontal health of 14 to 16-year-old pupils who attend different types of schools in Lippstadt, a town with a balanced social structure. A rate of 82% of gingivitis morbidity was found, including an average CPITN of 0.49. Nevertheless, only 43.8% of the examined periodontia showed signs of gingivitis. Analysis of the individual values showed marked differences related with age, sex and type of school.

[Concept of dental restoration using general anesthesia for uncooperative children].

The indication of treating uncooperative children with the help of general anaesthesia is discussed, the prior conditions and the clinical course are debated. Personal clinical experience in the period from 1981-1988 prove that the above mentioned indication has to be ventilated with greatest reluctance. As there are comparatively high dmf-s values (34.1 in the average) at the early age of 3-4 years, it seems to be most important to stress the value of a balanced lactovegetabile nutrition, a sufficient oral hygiene and the indispensable caries prophylaxis by fluorides. This is the only way to avoid extremely high efforts and the limited possibilities of a dental care with the help of general anaesthesia.

[Epidemiological studies in 14-16-year old pupils from schools in Lippstadt. 1. Caries occurrence and oral hygiene].

In a cross-sectional examination we received a current state of the prevalence of caries of 14-16 year old pupils, who attend different types of schools in Lippstadt, a town with a balanced social structure. An average DMF-S-Index of 14.6 (DMF-T: 7.8) was found. The particular results show a clear influence of age, sex and type of school. Comparing our results with former examinations a tendency of reduced caries-prevalence can be derived from the low annual caries-increment.

[Filling cysts with type 1 bone collagen].

The good properties of bovine bone collagen type 1 for filling of bony defects was demonstrated in 56 cysts of the alveolar bone in 52 patients. 49 cysts (approx. 87%) healed without irritation. 8 months postop. complete reossification of the defects had been achieved in 20 (approx. 36%) of the cysts. Further 32 defects (approx. 57%) were markedly reduced. The discussion will compare the advantages of bone collagen and other materials.

[Lipopigment in the adipose tissue of slaughtered swine].

After feeding rests of slaughtered poultry together with whey, tapioca and residues of stark production to pigs 70% of the slaughtered pigs were refused by the meat inspection because of yellow fat and yellow hair. The occurrence of bile pigment, carotenoids and synthetic colours could be excluded while acid fast pigments were demonstrated in the fat tissue, liver and skeleton muscle. This could be confirmed by the peroxide number, acid number and the content of malondialdehyde and vitamin E. The results were discussed in the view of meat inspection laws.
