[Channel selection for EEG-monitoring in anesthesia].

The conventional multichannel electroencephalogram is quite inconvenient for long-term monitoring in the operating theatre or intensive care unit. Recording of the EEG would be easier if a small number of channels was sufficient. Aiming at reduction of channels, leads from different regions of the scalp were analysed visually and with regard to their spectral content. METHODS. Electrode placements corresponded to the International 10/20 System (Fig. 1). EEG recordings were made with a conventional device (ES 12,000), a personal computer, and a spectral analyser. TWO-CHANNEL RECORDINGS. Retrospective analysis was performed on data from 392 patients (age 14-90 years) whose anaesthesia was induced with various anaesthetics/narcotics, for instance thiopental, ketamine, etomidate, halothane, and enflurane. The EEG was recorded using C3-P3 and Cz-A1. For each patient the changes of spectral parameters during the course of the induction were plotted and visually analysed. For statistical analyses a 30-s epoch of each patient was randomly selected from the first few minutes after the beginning of induction. TEN-CHANNEL RECORDINGS. In ten gynaecological patients (age 26-55 years) EEG recordings were performed during induction of anaesthesia with thiopental in combination with fentanyl, N2O and O2. The set of channels consisted of Fz'-Cb1, F3'-Cb1, Cz-Cb1, C3-Cb1, P3-Cb1, Oz-Cb1, Fz'-F3', F3'-C3, C3-P3, and P3-Oz. The electrodes Fz' and F3' were positioned on the forehead near to Fz and F3, respectively. These sites were chosen because they allow easy application of electrodes. The relationship between channels was calculated with Bravais-Person's coefficient of correlation for the power and the absolute power in the frequency bands delta (0.5-3.5 Hz), theta (3.5-7.5 Hz), alpha (7.5-12.5 Hz), and beta (> 12.5 Hz). RESULTS. In visual and statistical analyses of the two- and ten-channel recordings under the influence of anaesthetics/narcotics, similar changes of EEG activity could be observed in all channels. Although differences in the absolute power of the frequency bands were present, there was high conformity in the composition of the spectral content of the different channels. Classification of the EEG into stages of anaesthesia by means of a single channel led to consistent results for all channels. Alpha activity as leading feature of the awake state predominated occipitally. In channels including the region around the ears, contamination with EKG artifacts was observed. CONCLUSIONS. EEG patterns under the influence of different anaesthetics/narcotics are adequately represented by a reduced number of channels. For the choice of an appropriate set of channels the following aspects should be considered. Contamination with artifacts should be as low as possible, electrode sites should easily be accessible, and special features of the awake state should be identifiable. Experience with routinely conducted EEG recordings in the operating theatre and the intensive care unit showed that the channels C3-P3 or C4-P4 provide a sufficient basis for automatic staging of the depth of anaesthesia, which is implemented in the EEG monitor Narkograph.

A PC program for unbiased and predictive linear and quadratic discriminant analysis.

Discriminant analysis plays an important role in biological and medical research. The most popular methods of discrimination in practical applications are parametric methods like linear and quadratic discriminant analysis. However, there exist modifications of these approaches, namely unbiased and predictive discriminant analysis, which lead to reduced error rates in certain situations. In this paper a menu-driven, user-friendly PC program written in Borland Pascal 7.0 is introduced which performs unbiased and predictive linear and quadratic discriminant analysis.

A new system for descriptive classification of stones in the upper urinary tract.

A descriptive classification is proposed to stratify upper urinary tract stones by their number, size and location. The system considers the minimal but most important factors regarding the choice of surgical treatment and its success. Its principle is adaptable to more complex staging systems already existing. Practical use of the system has shown it to reflect clinical events, and its simplicity offers an opportunity for compliance in routine clinical study. It facilitates easy computerized stratification of stones in the upper urinary tract.

Isolation and characterization of a syncytium-inducing, macrophage/T-cell line-tropic human immunodeficiency virus type 1 isolate that readily infects chimpanzee cells in vitro and in vivo.

Fresh human immunodeficiency virus type 1 (HIV-1) isolates from patients with AIDS were screened for infectivity in chimpanzee peripheral blood mononuclear cells (PBMC) to identify strains potentially able to generate high virus loads in an inoculated animal. Only 3 of 23 isolates obtained were infectious in chimpanzee cells. Of these three, only one (HIV-1DH12) was able to initiate a productive infection in PBMC samples from all 25 chimpanzees tested. HIV-1DH12 tissue culture infections were characterized by extremely rapid replication kinetics, profound cytopathicity, and tropism for chimp and human PBMC, primary human macrophage, and several human T-cell lines. An infection was established within 1 week of inoculating a chimpanzee with 50 50% tissue culture infective doses of HIV-1DH12; cell-free virus was recovered from the plasma at weeks 1, 2, and 4 and was associated with the development of lymphadenopathy. Virus loads during the primary infection and at 6 months postinoculation were comparable to those reported in HIV-1-seropositive individuals.

The outcomes of elective laparoscopic and open cholecystectomies.

BACKGROUND: The demand for evidence of effectiveness for medical care has prompted the development of epidemiologic approaches to relating the outcomes of care to treatment. This study compares the outcomes of care for patients undergoing the newly introduced laparoscopic cholecystectomy with the results from conventional open cholecystectomies. METHODS: Consecutive cases of elective cholecystectomy from 35 hospitals (all of the metropolitan and selected rural hospitals in Minnesota) were enrolled in the study. Patients were interviewed on admission to establish baseline symptoms and functional status and to confirm risk factors. Their medical records were abstracted to yield information on risk factors, treatment, and hospital complications. To establish outcomes, patients were sent a questionnaire about their symptoms and functional status six months postoperatively. RESULTS: Of 3,448 patients studied, 2,490 (72 percent) had a laparoscopic procedure, including 195 cases that were converted to open cholecystectomies. Functional status data were obtained on 2,481 cases (76 percent). Laparoscopic operation was associated with more operative complications (odds ratio 3.02, p < 0.001), but with fewer general complications (odds ratio 0.32, p < 0.001). The mean time to return to work was 15 days for laparoscopic cases compared to 31 days for open procedures (p < 0.001). The only functional outcome difference between the two procedures was that patients who underwent laparoscopic cholecystectomies were more likely than those with conventional cholecystectomies to be able to perform their usual activities at follow-up evaluation (p < .001). There was evidence of a learning curve; the more laparoscopic procedures a surgeon performed, the fewer the operative (p < 0.01) and general (p < 0.0001) complications. There was no indication that the availability of laparoscopic operation was associated with more operations being performed. CONCLUSIONS: Laparoscopic operation seems to represent a significant advance in getting patients back to a normal life sooner. More attention needs to be given to which patients are most likely to benefit from cholecystectomy of either type. Epidemiologic approaches can be useful in assessing the effectiveness of care. Partnerships between providers and researchers can produce useful effectiveness data by supplementing available clinical records with more detailed outcome data.

Anaphase onset in vertebrate somatic cells is controlled by a checkpoint that monitors sister kinetochore attachment to the spindle.

To test the popular but unproven assumption that the metaphase-anaphase transition in vertebrate somatic cells is subject to a checkpoint that monitors chromosome (i.e., kinetochore) attachment to the spindle, we filmed mitosis in 126 PtK1 cells. We found that the time from nuclear envelope breakdown to anaphase onset is linearly related (r2 = 0.85) to the duration the cell has unattached kinetochores, and that even a single unattached kinetochore delays anaphase onset. We also found that anaphase is initiated at a relatively constant 23-min average interval after the last kinetochore attaches, regardless of how long the cell possessed unattached kinetochores. From these results we conclude that vertebrate somatic cells possess a metaphase-anaphase checkpoint control that monitors sister kinetochore attachment to the spindle. We also found that some cells treated with 0.3-0.75 nM Taxol, after the last kinetochore attached to the spindle, entered anaphase and completed normal poleward chromosome motion (anaphase A) up to 3 h after the treatment--well beyond the 9-48-min range exhibited by untreated cells. The fact that spindle bipolarity and the metaphase alignment of kinetochores are maintained in these cells, and that the chromosomes move poleward during anaphase, suggests that the checkpoint monitors more than just the attachment of microtubules at sister kinetochores or the metaphase alignment of chromosomes. Our data are most consistent with the hypothesis that the checkpoint monitors an increase in tension between kinetochores and their associated microtubules as biorientation occurs.

Congenital and acquired curvature of the penis treated surgically by plication of the tunica albuginea.

OBJECTIVE: To evaluate the effectiveness of plication surgery for patients with congenital and acquired curvature of the penis. PATIENTS AND METHODS: Forty-eight patients with congenital curvature of the penis and 57 with induratio penis plastica were treated by plication of the tunica albuginea. Their average age was 23 and 56 years, respectively. The operation was performed under local anaesthesia, using 2/0 Prolene and a specially designed suture. The average observation time in the two groups was 24 and 20 months, respectively. RESULTS: Nineteen per cent of patients in the first group and 8% in the second group reported normal intercourse pre-operatively. Post-operatively, the numbers increased to 81% and 82%, respectively, and all were able to perform intercourse. Although 18-19% experienced slight discomfort, mostly related to the cut ends of the suture, this minor problem was accepted by all. Five patients needed more than one operation. No impotence was recorded post-operatively and no patient complained of reduced sensibility of the skin. CONCLUSION: Satisfactory results were obtained with the plication technique in the first 105 patients. Since the follow-up study was completed more than 2 years ago, another 70 patients have been treated by the same procedure.

[Thymus-like expression and molecules accessory to division and of thymocyte-stroma interactions in thymomas: a prerequisite for the establishment of an abnormal T-cell repertoire?].

Costimulatory stromal receptors were identified in thymuses and myasthenia gravis (MG)-associated thymic epithelial tumors (TETs) by immunohistochemistry. ICAM-1 occurred in areas with medullary or cortical differentiation of TETs and thymuses. B7 and LFA-3, absent in the normal cortex, were hyperexpressed in cortical type TETs. VCAM-1, confined to endothelium and dendritic cells in cortex and medulla, was focally expressed by epithelial cells of a medullary thymoma. We suggest that aberrantly expressed costimulatory molecules may contribute to the pathogenesis of paraneoplastic MG by an intratumorous activation of mature T cells.

Cloning of a cDNA coding for the acetylcholine receptor alpha-subunit from a thymoma associated with myasthenia [correction of myastenia] gravis.

To investigate the role of the acetylcholine receptor (AchR) in the pathogenesis of paraneoplastic Myasthenia gravis (MG), we screened a cDNA library of a MG-associated thymoma with a DNA oligonucleotide coding for aa 371-378, i.e. for part of the very immunogenic cytoplasmatic epitope (VICE-alpha, aa 373-380) of the human AChR alpha-subunit. We isolated two cDNA clones. Analysis of these clones has identified an open reading frame of 1371 bp, coding for the AChR alpha-subunit. No point mutation, insertion or deletion could be detected. Since the thymoma did not contain thymic myoid cells, which normally express AChR, the origin of the AChR transcripts must be the tumor cells itself. These findings confirm former results, where AChR alpha-subunit sequences from MG-thymomas were amplified by PCR.

Distribution of molecules mediating thymocyte-stroma-interactions in human thymus, thymitis and thymic epithelial tumors.

Two findings in thymic epithelial tumors are correlated with the occurrence of myasthenia gravis(MG): (1) the expression of an acetylcholine receptor (AChR)-like-epitope in the neoplastic epithelium, and (2) the preservation of thymus-like features in the neoplasms, indicated by the presence of immature thymocytes. On this background it has been proposed that paraneoplastic MG may start with an intratumorous abnormal T cell selection due to aberrantly expressed AChR-epitopes (self-peptides). As appropriate thymocyte-stroma-interactions are prerequisites for thymocyte development in the thymus (and probably in MG-associated thymic tumors, too), we analyzed the expression of CD28/B7(BB1), CD2/:LFA3, LFA-1/ICAM-1 and VLA-4/VCAM-1 in human thymus and thymomas by immunohistochemistry. In normal thymuses and thymitis the stromal molecules were expressed at higher levels in the medulla than in the cortex. This was particularly true for B7(BB1) that was undetectable by immunoperoxidase techniques in the cortex. In contrast, cortical-type thymic epithelial tumors (cortical thymoma and well differentiated thymic carcinoma), known to exhibit the highest association with myasthenia, expressed the stromal molecules at almost medullary levels. The findings may be a clue to a functional difference between neoplastic and normal cortical epithelial cells: while we find the former to have the capacity to present soluble antigen to antigen-specific CD4+ T cells in vitro, normal cortical epithelium failed to do so. This altered microenvironment in thymomas might contribute to the autoimmunization by stimulating mature recirculating AChR-specific T cells.

Risk factors for suicide in Huntingtons disease: a retrospective case controlled study.

We reviewed 11 instances of suicide in HD families to determine what clinical and social characteristics might alert health professionals to increased suicide potential. The subjects were eight males and one female affected with HD, one female at risk for HD, and one unaffected female spouse, ranging in age from 24 to 65 years. Six of the nine individuals with HD who committed suicide were single or divorced. Duration of HD symptoms ranged from 1 to 14 years. The single most important risk factor for suicide in HD was having no offspring. Other suicides in the family, being unmarried, having contact with others affected with HD, living alone, and depression slightly increased the risk of suicide. The suicide of an unaffected spouse and an individual at risk, but unaffected, emphasizes the heavy burden of HD on other family members. Recognition of these risk factors should allow health care providers to assist families coping with HD and presymptomatic diagnosis.

Postural control in young and elderly adults when stance is challenged: clinical versus laboratory measurements.

The use of dynamic posturography (EquiTest) for the characterization of postural control biomechanics would be aided by specific knowledge of what the measured data imply about body segment movements. To investigate this issue, the biomechanics of a group of 15 healthy elderly subjects were compared to those of healthy young subjects by using both dynamic posturography and a laboratory movement and force measuring system. The results from EquiTest were analyzed by 1) routine clinical interpretation of data and 2) a clinical research interpretation by subjecting the EquiTest parameters to additional statistical comparison of mean performance of the young and elderly groups. The young-elderly differences from the 2 EquiTest analyses were then compared to the young-elderly differences derived from the laboratory protocol. The routine clinical interpretation of EquiTest data identified the same increases in sway shown by the laboratory study, but did not reveal the more subtle differences indicated by the laboratory study. When the EquiTest data were subjected to additional statistical analysis, the characterization of difference between young and elderly subjects was the same as that of the laboratory study, with the exception of issues of head versus trunk movement, a measure not made by EquiTest. This essential similarity in the characterization of elderly compared to young subjects by both systems suggests 1) that EquiTest is able to detect subtle differences in biomechanics of postural control between young and elderly healthy adult groups and 2) that implied movements of center of gravity, trunk versus lower limbs, and strength of reaction measures are consistently detected by both EquiTest and the laboratory kinematics and dynamics measurement systems.

The Minnesota Clinical Comparison and Assessment Program: a resource for clinical quality improvement programs.

UNLABELLED: Project Overview: MCCAP is a voluntary, statewide initiative of more than 50 hospitals designed to improve patient outcomes by reducing variation among healthcare providers. Elective cholecystectomy, both open and laparoscopic, has been one area of focus. KEY FINDINGS: Physicians are particularly interested in information about patient care that they do not routinely receive in a standardized format, such as the time required for patients to return to normal activities. Patients who did not meet guideline criteria but who were given prophylactic antibiotics prior to surgery experienced the same, minimal infection rate as patients who received no antibiotics prior to surgery. Participating hospitals are shifting their focus from looking at average occurrence rates (LOS, resource use by procedure) to identifying appropriate resource use. Although 82 percent of elective cholecystectomies met guideline requirements, 25 percent of all patients continue to experience their most distressing symptoms six months after surgery. Many physicians continue to use routine intraoperative cholangiography; however, MCCAP's guideline did a good job of differentiating patients who were more likely to benefit from the procedure from those who were not.

Postural control in young and elderly adults when stance is perturbed: kinematics.

Increased postural sway and falling are associated with aging and are likely related to problems with postural control in the elderly. We investigated the motions of individual body segments in 24 healthy young adults and 15 healthy elderly adults (mean ages 26 and 72) in response to four tasks: (a) standing with feet flat on an anteriorly accelerating platform (Flat Translation); standing on a narrow beam support that was (b) stationary (Beam Standing) and (c) accelerating anteriorly (Beam Translation); and (d) standing on a rotatable but otherwise stationary springboard (Springboard Standing). An optoelectronic camera system was used to measure rotations of body segments, particularly regarding their maximum excursions, time to first rotation response, direction of initial rotation, and time to first rotation reversal. In general, larger rotation excursions were noted in the elderly compared to the young group, particularly in the Beam Standing and Beam Translation tasks, but the magnitude of rotation difference was small. All rotation magnitudes were well within the available ranges of motion of the body joints. In both excursion magnitudes and directions of initial rotation, the elderly showed greater variability than the young. In the Beam Translation task, the elderly group, compared to the young, tended to rotate their upper body segments more than in the Flat Translation task. These data suggest that healthy elderly adults with no apparent musculoskeletal or neurological impairments have small but consistent differences in postural control kinematics, particularly when more challenging conditions are presented. Moreover, these data provide the basis for biomechanical analyses of joint torques and other dynamic requirements of these responses.

[Reduction of the number of recorded EEG channels for routine monitoring in the intensive care unit].

Monitoring patients in the intensive care unit with the aid of the conventional electroencephalogram employing a large number of recording channels is rather difficult, and can be laborious. This imposes limits on the routine application of this method. To investigate the possibility of developing a new monitoring device for easier application in the ICU, we aimed to establish whether the relevant information provided by a multi-channel EEG could be found in a subgroup of channels, thus reducing the number of channels required. Preferably those channels should be identified for use which are least contaminated by artefacts under routine conditions in the ICU. A total of 150 EEG recordings from the intensive care unit were inspected visually for the presence of artefacts. The derivations C3-P3 and C4-P4 proved to be least contaminated, at 35% and 39%, respectively. In these derivations visual assessment of the EEG was found to be impossible due to artefacts in only 4 and 5%, of all cases, respectively. A data set comprising 52 EEG segments with the fewest possible artefacts, was analysed using time series methods. On the basis of multivariate autoregressive processes, a measure was derived which describes the loss of information associated with a reduction in the number of EEG channels. The computation of the information loss for several channel combinations revealed that the derivations F3-C3, C3-P3 and A1-Cz represent a good compromise between information content, number of channels and frequency of artefacts. Practical experience shows that, at least for the control of sedation, a further reduction to a single channel should be possible.(ABSTRACT TRUNCATED AT 250 WORDS)
