[Chronic lead poisoning caused by wine consumption: an epidemiologic study].

An epidemiological study was carried out after the observation of a case of chronic lead poisoning so as to investigate its possible sources. An environmental evaluation was performed, including an analysis of the waters and wines consumed by the involved family; blood samples were obtained to assess lead levels in the consumers of the possibly implicated wines, with a comparative analysis of controls living in the same area. The study revealed that the source of the poisoning were the claret and white wines, which made soluble the lead when passing through the outlet tubes of the storage casks. The need for a greater surveillance of alimentary products by the corresponding authorities is emphasized.

[Primary antiphospholipid syndrome: study of 27 patients].

The clinical and biological features of a series of 27 patients with the recently described primary antiphospholipid syndrome are reported. Most of them belonged to a cohort of 90 patients who were carriers of lupus anticoagulant, which had been detected in the systematic evaluation of prolonged activated partial thromboplastin times in our hospital. Since the diagnosis they underwent a prospective protocol of follow up, with a peak follow up period of 9 years. The mean age of the 27 patients was 40.8 years and there were virtually no differences between sexes. Venous thrombosis was the most common clinical finding (16 episodes in 14 of the 27 patients). The most prevalent laboratory findings were lupus anticoagulant and IgG anticardiolipin antibodies.

[Amebic liver abscess: presentation of 4 cases].

We report four patients with amebic liver abscess diagnosed in our hospital between January 1985 and January 1990. Mean patient age was 40 years. All patients had previously travelled to an endemic area. Fever, weight loss, right-upper-quadrant pain and hepatomegaly were the most frequent clinical manifestation. Two patients had a history of diarrhea. Diagnosis was made by epidemiology, clinical manifestation, ultrasonic scanning and computerized tomography and serologic test. Two patients received metronidazole exclusively and two patients required percutaneous catheter drainage under ultrasound guidance.

Fetal loss treatment in patients with antiphospholipid antibodies.

A group of seven young women with antiphospholipid antibodies, histories of recurrent fetal loss, and no live births is reported. Two patients had systemic lupus erythematosus, and the other five fulfilled criteria for the primary antiphospholipid syndrome. A false Venereal Disease Research Laboratory (VDRL) test was present in four of the patients, three had a previous episode of arterial or venous thrombosis, or both, and two had thrombocytopenia. Prednisone and acetylsalicylic acid were given, and monthly controls of lupus anticoagulant activity were carried out. The dose of acetylsalicylic acid was fixed while the dose of steroids was adjusted according to the degree of lupus anticoagulant activity. A fetal survival was obtained in 7/9 (78%) of the pregnancies. Three of the newborn infants had transitory lupus anticoagulant activity. A search for antiphospholipid antibodies should be carried out in patients with otherwise unexplained fetal losses, falsely positive VDRL tests, thrombosis, or thrombocytopenia as the treatment of such patients with prednisone and acetylsalicylic acid is highly effective.

[The ergonovine test: an episode of hallucinations].

The ergonovine maleate provocation test for the diagnosis of coronary spasm is widely used in patients with rest angina. A patient who developed an episode of psychotomimetic symptoms after the administration of intravenous ergonovine is presented. This side effect of ergonovine maleate had not been previously reported.

Further studies of trans-sobrerol metabolism: rat, dog and human urine.

The urinary metabolites of d, l-trans-sobrerol were investigated after oral administration to rats, dogs and humans. Structural analysis of the urinary metabolites indicates that allylic hydroxylation, oxidation and glucuronylation are the common interspecies modes of trans-sobrerol biotransformation. Comparison of the findings of trans-sobrerol with previous investigations of cyclic monoterpenes shows similarities in their metabolism in vivo, including the resistance to oxidation of the cyclic trisubstituted double bond.

Biotransformation of epomediol. I--Isolation and identification of main metabolites in the rat.

The identification of biotransformation products of the new choleretic drug epomediol in rat has been performed by means of mass spectral, proton magnetic resonance and infrared techniques. The drug was administered perorally or intravenously; urine, bile and faeces were collected and the metabolites isolated. In urine, three metabolites were found in addition to the unchanged drug, namely; 1,3,3-trimethyl-2-oxabicyclo[2.2.2]octan-6-hydroxy-7-one, 1,3-dimethyl-2-oxabicyclo[2.2.2]octan-6,7-hydroxy-10-methanol and 1,3,3-trimethyl-2-oxabicyclo[2.2.2.]octan-6-beta-glucuronide-7-ol. In bile only the glucuronide and the unchanged drug were found. Analogously, in faeces the glucuronide and the drug were also identified, but only in trace amounts. This fact may suggest the presence of an enterohepatic circulation of the drug.

New secondary metabolites of Aspergillus wentii Wehmer. The positive and negative ion mass spectra produced by electron impact.

The positive and negative ion mass spectra of three secondary metabolites of Aspergillus Wentii Wehmer, namely 15-acetoxyhexadecyl-, 15-oxohexadecyl- and 15-hydroxyhexadecylcitraconic anhydrides are reported and discussed. The positive ion mass spectra, which have been studied in detail by exact mass measurements and metastable ion detection using a defocusing method, show very weak molecular ions and complex fragmentation pathways, strongly influenced by the group on C-15. However, the negative ion mass spectra show very abundant molecular ions and little fragmentation, with the negative charge mainly retained by the anhydride moiety. The behavior of both positive and negative ions is in agreement with the charge localization concept, on the grounds of ionization energy and electron affinity considerations. positive and negative ion mass spectra are largely complementary, thus increasing their analytical value.

The electron impact induced fragmentation of Aspergillus amstelodami alkaloids and derivatives.

The mass spectral data of four isoprenylated indole alkaloids containing a piperazinedione group, neoechinuline, cryptoechinuline C, and A and echinuline, isolated from Aspergillus amstelodami cultures, and of two derivatives obtained by catalytic hydrogenation of neoechinuline and echinuline, are reported and discussed. Their fragmentation patterns are strongly dependent upon the presence of a single or a double bond betueen C-8 and C-9.

Sterol composition of Bulgarian soya and corn oils.

The free sterols, the sterol esters and the sterol glycosides of the raw soya and corn oils as well as those of the technical lecithin and the deodorizer distillated of the latter oils were isolated by preparative TLC. The composition of each of the isolated sterol derivatives was determined by GLC and MS. Sitosterol, campesterol, stigmasterol and an unknown sterol with a molecular weight of 428 are contained in almost all of the examined fractions of the soya oil and its refinement byproducts. Dehydrocampesterol is present in the free sterols of the raw soya oil and the soya lecithin. Stigmasterol is contained in the soya deodorizer distillate in high amounts. It was established that cholesterol was present in the sterol esters of the raw soya oil high amounts. Delta7-stigmastenol occurs only in the sterol esters of the latter oil. Sitosterol, campesterol and stimgasterol are the main components of all sterol fractions of the corn oil and its refinement products. Dehydrocampesterol and unknown sterols with molecular weights of 428 are present in the free sterols of the raw corn oil. Some sterol glycosides of the soya and corn lecithin are esterified with the same major fatty acid components of the glycerides, palmitic acid being the main one. The fatty acid compositon of sterol esters of the raw soya and corn oil roughly corresponds to the fatty acid composition of oils.
