[Resection of hepatic colorectal metastases].

Between October 1986 and March 1995, 35 patients had hepatic resections because of metastases of colorectal cancer: 24 (68.6%) metastasectomies were carried out and 11 formal resections. The surgical morbility was 34.3% (12/35 patients) and the mortality 5.7% (2/35 patients). The medium hospitalization was 13.1 days (range: 6-68). The medium follow up of our population was 30.3 months. From the 33 evaluable patients (2 out of the 35 patients were discarded from the analysis for intraoperatory death) 21 relapsed. The medium disease free survival was 18.9 months; 19/33 patients died (all from progressive disease). The medium survival for the 33 patients was 26.6 months. The 5 years survival was 42.4%, comparable with other international series. We consider that factors related with the strict selection of the patients contributed to achieve these encouraging results. The multidisciplinary work (that made easier the selection of the better candidates for surgery) involving surgeons, anesthesiologists, oncologists, radiologists and internists, were made possible in a public hospital.

[Dermatophytes isolated in our clinics. 5-year-study in Zaragoza].

BACKGROUND: This review summarizes the different species of dermatophytes isolates in our laboratory between 1991 and 1995. We describe the clinical forms and establish the distribution over this period of time. METHODS: Retrospective survey of samples from outpatients of the Dermatology Service in Miguel Servet Hospital where mycologic cultures are required. The extraction of samples is made by scrapes with a carpet or scalpel and they are cultured on Saboureaud agar with chloramphenicol and dermatophytes agar for 3 weeks. All plates were incubated at 28 degrees C. The identification of isolated strains is made by means of morphologic and physiologic criteria; the doubtful strains were identified in national referral center of Majadahonda CNMVISS. RESULTS: 4004 samples were analyzed from 3934 patients and 543 strains of dermatophytes were isolated. The frequencies were as follow: Microsporum canis (44%), Trichophyton mentagrophytes (31.4%), Trichophyton rubrum (18.6%), Epidermophyton floccosum (2.6%), Microsporum gypseum (1.4%), Trichophyton tonsurans (0.7%), Trichophyton verrucosum (0.7%), Trichophyton violaceum (0.2%) y Microsporum audouinii (0.2%). The most frequently observed dermatophytoses were Tinea corporis (54.8%), followed by Tinea unguium (12.6%), Tinea capitis (12.5%), Tinea pedis (8.3%), Tinea manuum (6.3%), Tinea cruris (4.7%) and Tinea barbae (0.7%). CONCLUSIONS: The zoophylic species are the most prevalent in our area and we have observed a raise of Microsporum canis in recent years. It is important to perform mycologic survey in every suspected lesion in older to determinate the true incidence of human dermatophytoses.

Nucleotide sequence of the ampC-ampR region from the chromosome of Yersinia enterocolitica.

The nucleotide sequence of a 3.1-kb region from the chromosome of the Yersinia enterocolitica O:5b strain IP97 containing the gene for an inducible chromosomal cephalosporinase has been determined. The cephalosporinase gene was homologous to other enterobacterial chromosomal cephalosporinase genes, and it was accompanied by a gene homologous to the regulatory ampR gene. The arrangement of genes in the Y. enterocolitica ampCR unit was identical to that in the Enterobacter cloacae and Citrobacter freundii ampCR units.

Multiple antibiotic susceptibility associated with inactivation of the prc gene.

A Tn5 insertion which led to increased susceptibility to multiple drugs, including tetracycline, chloramphenicol, nalidixic acid, erythromycin, spectinomycin, norfloxacin, and novobiocin, was identified in Escherichia coli. Cloning and sequence studies showed that the insertion was in the previously identified prc gene at min 40.4. The prc product is known to function as a protease linked to processing of penicillin-binding protein 3 and lambda repressor and when absent to allow some leakage of periplasmic constituents. Complementation studies with the prc gene on plasmids showed complete recovery of parental levels of susceptibility to all drugs except chloramphenicol, with which only partial reversion to wild-type levels was observed.

Nucleotide sequence of a new class A beta-lactamase gene from the chromosome of Yersinia enterocolitica: implications for the evolution of class A beta-lactamases.

The nucleotide sequence has been determined of a 1400 bp fragment from the chromosome of Yersinia enterocolitica containing the gene for beta-lactamase I. An ORF of 882 bp was identified, which could code for a polypeptide of 294 amino acids, closely related to other beta-lactamases of molecular class A. Amino acids 1-30 could constitute a signal peptide. The mature protein would be 264 amino acids long with a calculated pI of 6.2. Alignment of the amino acid sequence of the class A beta-lactamases suggested the existence of two subgroups in the same class, and this is discussed in the context of the evolution of the enzymes.

Cloning of chromosomal beta-lactamase genes from Yersinia enterocolitica.

Two beta-lactamase genes present in the chromosome of Yersinia enterocolitica have been cloned individually into the plasmid pACY184 and expressed in Escherichia coli. The gene for broad-spectrum beta-lactamase I ('A') was cloned from a strain belonging to the O:3 serotype, and the gene for (cephalosporinase) beta-lactamase II ('B') was cloned from a strain of the O:5b serotype. The properties of the beta-lactamases expressed in E. coli are similar to those previously described in Y. enterocolitica.

Plasmid profiles of Legionella pneumophila strains isolated in Spain.

An assay for plasmid DNA content was carried out in 100 strains of Legionella pneumophila of distinct serogroups and isolated from various sources (clinical, environment). The strains were isolated from different geographic regions in our country. The presence of plasmids was proved in one of the 11 clinical isolates and in 68 of the 89 isolated of environmental origin studied. In the strains belonging to serogroup 1 and isolated in our region (Cantabria), three plasmid profiles were observed, whereas in strains of the same serogroup from other geographic regions, two profiles were shown which exhibited differences compared to the former ones. Analysis by means of restriction endonucleases suggested that plasmids of similar size in serogroup 1 strains of different source, were related. The results obtained do not appear to reveal any correlation between plasmid profile and source of isolation or serogroup.
