Hemangiopericytoma in the eyelid of a horse.

Hemangiopericytoma (HP) is a well-recognized neoplasm arising from vascular pericytes that has been reported only in the dog and man. In this study, we describe a 14-year-old female Arabian horse that was presented for surgical excision of a 2-cm-diameter expansile subcuticular mass in the right lower eyelid. Histologically, the mass consisted of loosely arranged interlacing streams and storiform bundles of spindle cells that often formed distinct whorls around a central capillary and bundles of collagen (Antoni A-like pattern). Immunohistochemical analysis revealed strong diffuse cytoplasmic immunoreactivity for vimentin and focal immunoreactivity for smooth muscle actin, whereas neoplastic cells did not stain for Factor VIII-related antigen, Glial fibrillary acidic protein (GFAP), or S100. On the basis of histomorphology and immunohistochemical reactivity, the present tumor was diagnosed as HP. To our knowledge, this is the first report describing a HP in a horse.

Concentration of methylprednisolone in the centrodistal joint after administration of methylprednisolone acetate in the tarsometatarsal joint.

REASONS FOR PERFORMING STUDY: The centrodistal (CD) and tarsometatarsal (TMT) joints are often injected individually with a corticosteroid to resolve lameness caused by osteoarthritis (OA). There are no data available regarding diffusion of methylprednisolone (MP) from the TMT joint to the CD joint. HYPOTHESIS: A therapeutic concentration of MP diffuses into the CD joint after methylprednisolone acetate (MPA) is administered into the TMT joint. OBJECTIVE: To measure the concentration of MP in the CD joint after MPA was administered into the TMT joint. METHODS: MPA was administered into a TMT joint of 16 horses. At different times, the ipsilateral CD joint of these horses was injected with a small amount of saline and recovered saline was measured for concentration of MP using high performance liquid chromatography. RESULTS: Six hours after administration of MPA into the TMT joint, a therapeutic concentration of MP was found in all 10 CD joints sampled at this time. CONCLUSIONS: Horses with pain arising from the distal 2 joints of the hock can be treated by administering MPA into the TMT joint alone. POTENTIAL RELEVANCE: Administering MPA into the TMT joint only, to treat OA of the distal 2 hock joints, reduces the difficulties and risks associated with centesis of the CD joint.

Abnormal brain hemodynamic responses during passive orthostatic challenge in panic disorder.

OBJECTIVE: Autonomic dysregulation and cerebral blood flow (CBF) abnormalities have been reported in patients with anxiety disorders and, more recently, in panic disorder. Variations in the middle cerebral artery velocity (measured by transcranial Doppler technique), heart rate, and blood pressure during a tilting-table test were used as nonanxiogenic procedures to explore these abnormalities. METHOD: Mean flow velocity in the right middle cerebral artery, heart rate, and blood pressure were monitored at rest and during a 70 degrees tilting-table test. Three groups of patients were studied: 11 patients (eight women and three men; mean age = 31.1 years, SD = 8.0) with a diagnosis of panic disorder within 6 months of the onset of the disorder, nine asymptomatic patients (two men and seven women; mean age = 35.0, SD = 6.3) with a previous diagnosis of panic disorder who were in full remission of symptoms and had been drug free for at least 6 months, and 10 normal comparison subjects (two men and eight women; mean age = 31.1, SD = 5.2). RESULTS: Both patients with acute panic disorder and patients with remitted panic disorder showed a significant percent reduction of right middle cerebral artery mean flow velocity compared with normal subjects following tilting to the upright position. No significant differences were observed for blood pressure and heart rate. CONCLUSIONS: Patients with panic disorder, both during the acute phase of the illness and after clinical recovery, show an exaggerated drop in CBF during tilting. Since these findings are similar, although attenuated in intensity, to those which are observed in dysautonomic illnesses, one possible interpretation is that of panic disorder as a subclinical form of autonomic dysreactivity.

201Tl myocardial perfusion in the management of the transplanted heart.

A semi quantitative method with thallium has been applied in the management of cardiac transplants. In all, 142 scans were performed in 20 patients and were arranged in 3 groups that represent respectively all controls performed to the transplants (G1), and a selection of the scans obtained during the 1st rejection episode in each patient (G2) and the 2nd if present (G3). A heart/lung index was calculated through early and late images (IA, IB), obtained 5 min and 4 h post injection of thallium. A decrease in IA was detected during acute rejection in all groups. This fall was statistically significant from the mild to moderate rejection degree in G1 (P less than 0.01), and the mild rejection in G3 (P less than 0.001). IB was not modified with rejection. Comparing the biopsies with and without edema, in the absence of rejection, it has been proved that the intra myocardial edema can produce a decrease in IA (P less than 0.001). We conclude that IA is a simple and sensitive index to evaluate the graft, although intra myocardial edema can reduce its specificity.

[Ulcerative lichen ruber planus of the feet, associated with complete loss of the nails and cicatricial alopecia of the scalp].

An unusual syndrome of lichen planus, characterized by ulcerations confined to the feet, permanent loss of toenails and a cicatricial alopecia of the scalp is reported, related to a 71 years old woman. Only eleven cases identical of this have been described as yet. Skin grafting to the ulcerous surfaces has been the treatment of choice, because offers the opportunity for a lasting therapeutic response.

201Tl myocardial imaging in a cardiac rejection episode.

Serial myocardial imaging using thallium Tl 201 was performed in the early follow-up of two patients with orthotopic cardiac transplantation. In one patient, non-homogeneous uptake, small defects and an irregular myocardial edge were observed during a moderately acute rejection crisis revealed by endomyocardial biopsy. The abnormal gammagraphic findings and histological changes were coincident and exhibited a parallel reversal. We emphasize the connection between these two events. The mechanisms which could explain these phenomena are discussed.

Scintigraphic demonstration of a diaphragmatic defect as the cause of massive hydrothorax in cirrhosis.

A 64-year-old man presented with a massive right-sided hydrothorax associated with cirrhosis of the liver. There was no clinical evidence of ascites nor other underlying disease. The usual complementary tests failed to demonstrate any causative abnormality. Chest and abdominal scintigraphy after intraperitoneal injection of 99mTc-human serum albumin disclosed early filling of the pleural space by the radiopharmaceutical and suggested a diaphragmatic defect as the cause for this rare association.

Ureteral fistula after renal transplantation: the significance of delayed images.

Urinary leakage after renal transplantation is a serious complication that can lead to diagnostic difficulties. A case is reported of urinoma secondary to ureteral fistula characterized in the routine study by a photon-deficient area. Only the image obtained at 6 h allowed a correct diagnosis. The causes for this are discussed and the need to include a delayed study in the follow-up protocol of these patients is emphasized.

[67gallium. Diagnostic possibilities and usefulness as a tracer].

As a consequence of its affinity to leukocytes and tumour cells, 67Gallium-Citrate is a good isotope for detecting infections and tumoral pathology. Its sensitivity is greater for lymphatic system malignancies, primary lung cancer, primary liver carcinoma and melanoma. It is a useful method with more sensitivity than diagnostic specificity.

[Indirect isotopic lymphography. Technic and clinical applications].

Interstitial lymphoscintigraphy is a diagnostic method useful in the staging and follow-up of several tumours as well as in the etiological evaluation of limb edema. It is a simple technique with minimal invasivity which can be performed even in those regions inaccesible to radiographic lymphography. Because of its great sensitivity and low specifity it requires careful interpretation.
