Selective high-dose intra-arterial cisplatin as palliative treatment for incurable head and neck cancer.

OBJECTIVE: The purpose of the present study was to evaluate the palliative effect of selective intra-arterial chemotherapy in progressive unresectable head and neck cancer previously treated with radiochemotherapy. PATIENTS AND METHODS: 8 patients with advanced residual or recurrent squamous cell carcinoma of the head and neck were evaluated. These patients were included in the present study particularly because of progredient pain and recurrent bleedings due to tumor progression. In addition 6/8 patients suffered from unpleasant tumor-related smell. All patients received simultaneous infusions of cisplatin (150 mg/m2) intra-arterially to the tumor and sodium thiosulfate intravenously (9 g/m2) for systemic neutralization of cisplatin. The patients were treated by at most 4 cycles of selective intra-arterial chemotherapy via femoral approach. RESULTS: Tumor-associated pain, occurrence of tumor bleeding and tumor-related smell were reduced after at least 2 cycles of intra-arterial chemotherapy in all patients. Clinical and radiological assessment of the primary tumor site revealed a partial response in 4 patients while 4 patients were classified as nonresponders. Intra-arterial cisplatin treatment was well tolerated. CONCLUSION: Selective intra-arterial cisplatin therapy can be delivered safely for palliation of tumor-related symptoms of incurable head and neck cancer.

Peritonsillar abscess--critical analysis of abscess tonsillectomy.

The risk of secondary haemorrhage following abscess tonsillectomy is reported in the literature with differing rates. A retro- and prospective analysis of complication rates following abscess tonsillectomy was conducted in 142 patients (54 females, 88 males; mean age: 35 years). In 22% of patients, a secondary haemorrhage occurred. In half of these (11% of total), the haemorrhage had to be treated surgically. Secondary haemorrhage occurred most commonly on the 6th and 8th postoperative days. Reports in the literature are not in unison about the risk of secondary haemorrhage following abscess tonsillectomy and therefore allow no final judgement about an objective risk of this complication. This report strengthens the results of the 'Comparative Audit Service' analysis from 1997, which did show a high risk of secondary haemorrhage following tonsillectomy, as well as following abscess tonsillectomy.

Efficacy of ice packs in the management of epistaxis.

There are no uniformly accepted criteria for the management of epistaxis. The usefulness of ice application in the treatment of epistaxis as a first aid method is not generally accepted, but is widespread. In order to evaluate the effect of cold application on the blood vessels of the nasal mucosa, their blood flow and blood content were investigated on 56 healthy volunteers before and after exposure to cold in the neck area. Nasal mucosal microcirculatory blood flow was measured directly by non-invasive laser Doppler flowmetry in Kiesselbach's area. Changes in the nasal mucosal blood content were estimated using a conventional computer-aided rhinomanometer by measuring alterations in nasal airflow. After ice application in the neck area, no statistically significant effects on the blood vessels of the nasal mucosa were seen. These results do not support the usefulness of this manoeuvre in the treatment of epistaxis.

Aneuploidy of chromosome 9 and the tumor suppressor genes p16(INK4) and p15(INK4B) detected by in situ hybridization in locally advanced prostate cancer.

INTRODUCTION AND OBJECTIVES: The linked p16(INK4)/MTS1 and p15(INK4B)/MTS2 genes on chromosome 9p21 encode proteins that inhibit the cyclinD dependent kinases CDK4/6. Biallelic homozygous deletions involving this locus have been identified in a wide range of tumor cell lines, but in a lower frequency of primary tumors. As PCR based approaches analyzing for homozygous deletions could be confounded by unavoidable contributions of normal cells in microdissected tissue, we performed in situ hybridization (ISH) on primary prostate carcinomas to accurately evaluate p16 and p15 copy numbers on a cell-by-cell basis. MATERIAL AND METHODS: p16 and p15 loci were evaluated in 28 pT3N0M0 prostate cancer specimens. Of 28 patients, 15 (53%) were ascertained showing no recurrence (mean follow-up 61+/-17 months), 13 (47%) developed recurrences within 27+/-19 months. Tissues were provided for ISH analysis in a blinded fashion. Isolated DNA derived from P1 clone 1063 compromising p16 and p15 as well as a centromeric probe for chromosome 9 were used for hybridization. Signals were enumerated within 300 interphase nuclei per tumor specimen, and in 100 nuclei derived from 18 benign prostate tissues and 7 adjacent PIN regions. RESULTS: ISH detected aneuploid tumors in 12/13 (92%) patients with recurrence and in 5/15 (33%) without recurrence (p<0.0014). Whereas 3/7 PIN specimens associated with nonrecurrent PCA demonstrated euploidy, all 4/7 PIN associated with recurrent disease demonstrated the same aneuploidy for chr9 as the primary tumor. All benign tissues evaluated exhibited euploidy for chr9, p16 and p15. None of the PCA and PIN samples revealed homozygous deletions for p16(INK4)/MTS1/p15(INK4B)/MTS2; 2/28 (7.1%) PCA exhibited partial deletion for p16(INK4)/MTS1/p15(INK4B)/MTS2 and aneuploidy for chr9; both PCA derived from the recurrent group. CONCLUSIONS: Deletion of 9p21 was rare and therefore such genetic alterations may not play an important role in the pathogenesis of PCA. Analysis of the limited number of PCA examined suggest a strong association between chr9 aneuploidy and recurrenct disease. Aneuploidy in both PIN and PCA suggests that the clinical outcome of PCA might already be determined in the preinvasive PIN.

Expression of MMP-3, MMP-13, TIMP-2 and TIMP-3 in the VX2 carcinoma of the New Zealand white rabbit.

BACKGROUND: Squamous cell carcinomas of the head and neck region (HNSCC) are among the most common malignancies in this area. The VX2 carcinoma of the New Zealand white rabbit metastasizes lymphatically as is the case in HNSCC and therefore, potentially, could be used as a model for HNSCC. Since the family of matrix-metalloproteinases (MMPs) is involved in the process of HNSCC invasion, the aim of this study was to investigate the expression level of MMPs and their specific inhibitors (TIMPs) in the VX2 carcinoma to evaluate if they also play a role in VX2 tumor invasion as observed in human HNSCC. MATERIALS AND METHODS: The VX2 carcinoma was generated by tumor implantation in the rabbit's ear as previously described. Western blots were performed under standard conditions, utilizing antibodies against MMP-3, MMP-13, TIMP-2 and TIMP-3. Immunohistochemical staining was performed with the ABC-complex method. RESULTS: A positive immunohistochemical signal could be detected for MMP-3, TIMP-2 and TIMP-3 with no significant signal for MMP-13. In the Western blots immunoreactive bands could be observed for MMP-3, MMP-13, TIMP-2 and TIMP-3. CONCLUSION: MMP-3, MMP-13, TIMP-2 and TIMP-3 were found to be expressed in VX2 carcinomas of the New Zealand white rabbits. The VX2 carcinoma therefore resembles HNSCC tumors not only in its metastatic behavior, but also regarding the expression of MMPs and TIMPs, which are the probable keyplayers during the event of invasion. These observations further underline the significance of the VX2 carcinoma as a model tumor of human HNSCC.

Expression of MMP-2, -9 and -13 in cell lines and fresh biopsies of squamous cell carcinomas of the upper aerodigestive tract.

BACKGROUND: The expression patterns of the matrix metalloproteinases (MMP) 2, 9 and 13 have been examined in biopsies and cell lines of squamous cell carcinomas (SCC) of the upper aerodigestive tract to determine the association between MMP expression and the aggressiveness of the tumor. MATERIALS AND METHODS: The expression of MMP-2, -9 and -13 was analyzed by immuno-hybridization in 12 keratinocyte cell lines, 13 SCC cell lines and in 63 biopsies from SCCs of the upper aerodigestive tract. RESULTS: In biopsies a correlation was detected between T-status and the MMP expression [MMP-2 (p < 0.05), MMP-9 (p < 0.01), MMP-13 (p < 0.01)], between N-status and MMP expression [MMP-2 (p < 0.05), MMP-9 (p < 0.01)] and between UICC stage and MMP-9 expression (p < 0.01). CONCLUSION: The MMP-2, MMP-9 and MMP-13 expressions correlate strongly with pathological parameters. The determination of these tumor-biological parameters could help to identify high risk patients with increased frequency of metastatic spread.

Intravenous chemotherapy with cisplatin for regional lymph node metastases of auricular VX2 carcinoma.

INTRODUCTION: The VX2 carcinoma is well established as a useful model for studies on treatment of primary tumors of various locations including the rabbit's auricle; however, limited experience exists on the treatment modalities of lymph node (LN) metastases. In this investigation we studied the frequency and extent of lymphogenic metastatic spread of auricular VX2-carcinomas and their response to systemic chemotherapy. MATERIALS AND METHODS: Induction of a right-sided auricular VX2-carcinoma in 17 healthy New Zealand white rabbits was followed by ablation of the right auricle and intravenous application of 1 mg/kg KG CDDP (cisdiamminedichloroplatinum (II)) dissolved in 2 ml NaCl via the left-sided auricular vein in 10 rabbits (group 1), while 7 rabbits (group 2) remained untreated. After a 24-day follow-up period, animals of both groups were sacrificed and the regional draining LN as well as the lungs were isolated and examined histopathologically. RESULTS: Group 1. Following intravenous cisplatin therapy (ICT), 6/10 animals showed no vital tumor cells within LN metastases of the first draining LN station, while 4/10 animals had necrotic LN metastases limited to the parotideal LN. Group 2. All 7 animals showed necrotic LN metastases of the first and second draining LN station as well as pulmonary metastases. CONCLUSION: The auricular VX2-carcinoma, characterized by frequent lymphogenic metastatic spread and response of LN metastases to ICT, offers an excellent animal model for further studies on the optimised treatment of lymphogenic metastatic spread in HNSCC.
