Cell-mediated immunity to encephalitogenic factor (MMI test) in women with cervical dysplasia and carcinoma in situ: the effects of serum.

Lymphocyte sensitivity to encephalitogenic factor (EF) was measured with the macrophage migration inhibition (MMI) test in 60 women with dysplasia or carcinoma in situ of the cervix, in 10 women with invasive cervical carcinoma, and in 20 women with a variety of benign gynaecological conditions. Significant migration inhibition with EF (P less than 0.01) was seen with lymphocytes taken from 7/13 (54%) women with mild and/or moderate dysplasia, from 22/47 (47%) women with severe dysplasia and/or carcinoma in situ, from 6/10 (60%) women with invasive cervical carcinoma and from 3/20 (15%) women with benign gynaecological conditions. Autologous serum was seen to abrogate EF-mediated migration inhibition in 3/4 sensitized women with mild and/or moderate dysplasia, in 5/7 sensitized women with severe dysplasia and/or csrcinoma in situ and in 2/3 sensitized women with invasive cervical carcinoma. Autologous serum from 2 sensitized women with benign gynaecological conditions did not abrogate the response of their lymphocytes to EF.

Cellular and genetic control of antibody responses in vitro. II. Ir gene control of primary IgM responses to trinitrophenyl conjugates of poly-L-(Tyr,Glu)-poly-D,L-Ala--poly-L-Lys and poly-L-(His,Glu)-poly-D,L-Ala--poly-L-Lys.

The in vitro primary IgM anti-hapten responses to trinitrophenyl (TNP) conjugates of poly-L-(Tyr,Glu)-poly-D,L-Ala-poly-L-Lys (T,G)-A--L and poly-L(His,Glu)-poly-D,L-Ala--poly-L-Lys (H,G)-A--L were shown to be T-cell dependent and under autosomal dominant H-2-linked Ir gene control which mapped within the K or I-A regions of the H-2 complex. The in vitro response to TNP-keyhole limpet hemocyanin, while T-dependent, was not under demonstrable genetic control. The genes governing the in vitro primary IgM anti-hapten responses to TNP-(T,G)-A--L and TNP-(H,G)-A--L resemble the Ir genes controlling the in vivo secondary IgG responses to (T,G)-A--L and (H,G)-A--L in that they are autosomal dominant, map identically within the H-2 complex, and have identical responder and nonresponder haplotypes. It is concluded that Ir genes can govern the ability to generate an IgM response upon initial exposure to antigen.

Dynamic anatomy of the cervical epithelium.

The surface epithelium of the cervix undergoes change at many periods during life. The changes are principally due to the occurrence of metaplastic squamous epithelium within the transformation zone. This process seems to hold the key to an understanding of the origins of neoplastic development in the cervix. Cervical mucus secretion also seems to be dramatically influenced by this process.

A hypothesis: the role of a high-risk male in the etiology of cervical carcinoma: a correlation of epidemiology and molecular biology.

Certain impressive epidemiological evidence appearing in the last few years leads to the conclusion that some males are more prone to have a spouse with squamous cancer of the cervix than others. Advances in knowledge in other fields, especially at the molecular biological level, enable speculation on reasons for this curious conclusion. These readily testable theories, together with an outline of the evidence on which they rest, form the substance of this hypothesis.

Surgical treatment of mal perforans.

Mal perforans is a chronic foot ulcer commonly found in diabetics with neuropathy. Although it will often respond to conservative measures, recurrence is frequent and brings with it the risk of spreading infection and serious destruction of tissues. Surgical treatment of seven patients, based on excision of the distal metatarsal bone (five patients), or excision of the distal metatarsal and amputation of the first toe (two patients), gave satisfactory results and is indicated to achieve long-term healing. A special shoe, designed to remove pressure from the healing areas as well as from potential new pressure points, was used in the last four patients.

Painful leg syndrome (pseudo-phlebitis).

Lower extremity venography is essential to confirm the diagnosis of phlebitis when the clinical situation is not completely typical. 8 patients are presented, 6 of whom had been given prolonged prior anticoagulant therapy for phlebitis and pulmonary embolus. Normal venograms and reassurance led to resolution of symptoms in all cases.

Anomalous origin of the right coronary artery from the pulmonary artery.

Anomalous origin of a coronary artery from the pulmonary artery is a relatively rarer congenital anomaly which almost exclusively involves the left coronary artery. A previously healthy 25-year-old woman had a cardiopulmonary arrest. Following resuscitation, investigations revealed an even rarer anomaly, an anomalous right coronary artery arising from the pulmonary artery. Successful surgical repair was accomplished by reimplanting the right coronary artery into the aortic root. The patient is currently asymptomatic 2 years postoperatively. The pertinent literature is reviewed.
