Hypersensitivity syndrome caused by amitriptyline administration.

Adverse cutaneous manifestations are among the most common side effects associated with psychotropic drugs. Skin reactions due to amitriptyline (a tricyclic antidepressant agent) include rashes and hypersensitivity reactions (for example, urticaria and photosensitivity) as well as hyperpigmentation. Hypersensitivity syndrome is a specific severe idiosyncratic reaction causing skin, liver, joint, and haematological abnormalities, which usually resolve after the discontinuation of the implicated drug. A case of a 24 year old woman who experienced hypersensitivity syndrome three weeks after the initiation of amitriptyline is reported.

Immunohistochemical expression of retinoblastoma gene product (Rb), p53 protein, MDM2, c-erbB-2, HLA-DR and proliferation indices in human urinary bladder carcinoma.

Archival biopsy specimens from transitional cell bladder cancers (n=88) were analysed immunohistochemically for the expression of the retinoblastoma (Rb) gene protein, p53, mdm2, c-erbB-2, HLA-DR antigen and proliferation indices. An altered nuclear expression of Rb, p53 and mdm2 was observed in 55.2%, 33.3% and 18.2% of tumors respectively. Cytoplasmic membrane immunoreactivity (>25% tumor cells) for c-erbB-2 was detected in 14.1% of tumors and aberrant HLA-DR antigen cytoplasmic staining (>5% of tumor cells) in 22.2% of the cases. P53 overexpression was associated with higher tumor grade and stage. Aberrant HLA-DR antigen expression and PCNA were also correlated with the grade of differentiation and tumor stage. MIB1 was statistically correlated with stage. pRb scores and HLA-DR antigen expression were correlated with proliferation activity as determined by PCNA and MIB1 immunostaining. p53 protein was also strongly correlated with the proliferation index PCNA. A strong correlation between PCNA and MIB1 (p<0.0001) was also found. In addition a statistically positive correlation between p53 and HLA-DR antigen expression was observed. Our data show that, although pRb and p53 protein expressions are not associated between them, they may contribute to the growth fraction of the bladder cancer. In addition, p53 and HLA-DR antigen expression could be indicators of aggressive behavior of bladder cancer.

p53 and c-jun expression in urinary bladder transitional cell carcinoma: correlation with proliferating cell nuclear antigen (PCNA) histological grade and clinical stage.

OBJECTIVE: To investigate p53 and c-jun oncoproteins and proliferating cell nuclear antigen (PCNA) in transitional cell urinary bladder carcinomas (TCCs) and to determine their relationships to tumour grade, stage and survival. MATERIALS AND METHODS: The expression of p53, c-jun and PCNA was studied using immunohistochemistry in formalin-fixed, paraffin-embedded tissues in a series of 110 TCCs. RESULTS: 58% of our cases were positive for p53 and 88% for c-jun. A statistically very significant correlation (p < 0.0001) was observed between p53 and c-jun (r = 0.781), p53 and PCNA (r = 0.772), c-jun and PCNA (r = 0.831) as well as between each of the two oncoproteins and the histological grade and clinical stage (p < 0.001). There was no correlation of either p53, PCNA or c-jun with clinical outcome in terms of patients survival. CONCLUSION: p53 and c-jun proteins' overexpression are strongly related to rapid tumour cell proliferation and hence with aggressive growth in urinary bladder TCC. PCNA score remains an important prognostic index in transitional cell carcinoma of the bladder.

Functional assessment of the rat sciatic nerve following intraoperative expansion: the effect of recovery duration on behavioural, neurophysiological, and morphological measures.

The purpose of this study was to investigate the feasibility of rapid intraoperative elongation of the rat sciatic nerve with the use of tissue expander and to assess its functional recovery. Out of 51 rats 43 had their right sciatic nerve expanded with a 5-ml intraoperative expander over 1 hr and 8 were sham-operated controls. The functional recovery of the nerve was assessed at intervals up to 4 months using the Sciatic Functional index (SFI), neurophysiological indices, and histology. Intraoperative expansion elongated the rat sciatic nerve by about 13%. SFI decreased on the first postoperative day and started to recover by Day 7, reaching almost preoperative values by Days 14 and 30 according to De Medinaceli and Bain-Mackinnon-Hunter formulas, respectively. Latency and motor conduction velocity demonstrated a deterioration after expansion which peaked on Day 1. Recovery started by Day 7 and reached preoperative levels by 60 days. The histological findings indicated minor aberrations immediately after expansion and maximal demyelination with minimal axonal disruption on Day 1. The reparative process started by Day 7 and continued till Day 120 when almost no histological changes were observed. In conclusion, intraoperative nerve expansion successfully elongates the rat sciatic nerve. It also causes functional and morphological abnormalities which are of low to moderate degree, have a short duration, and are reversible. Intraoperative nerve expansion might be a valuable solution in the treatment of short nerve gaps, but its clinical application still needs to be evaluated.

p53 expression in hepatocellular carcinoma in Greece. Correlation with epidemiological and histopathological data.

Localization of p53 oncoprotein was investigated in 60 hepatocellular carcinomas (HCCs) from patients resident in the Northwest and Central Greece. The streptavidin-biotin complex immunoperoxidase method was performed in archival formalin-fixed, paraffin-embedded material, using the monoclonal antibody DO-1. The aim of our study was to correlate p53 expression with histological and epidemiological data. p53 overexpression in patients with serological hepatitis B or C was greater (47%) as compared to that observed in patients without these markers (p < 0.01). Morphologically normal liver tissue (NLT) and liver cell dysplasia (LCD) was recognized adjacent to HCC developing on non-alcoholic cirrhotic livers in patients with "NonA, NonB hepatitis" from between 1975-1986. NLT and LCD and p53 oncoprotein was expressed in 10% of the cases. No relationship was observed between p53 expression and tumor histological grade, patients' age and sex. These results suggest that in Northwest and Central Greece, p53 oncosupressor gene may be involved in some HCCs; it may be associated with viral chronic infection disease (HBV or HCV), and as yet with uncharacterized viruses which remain to be determined.

Hepatobiliary cystadenoma with mesenchymal stroma (CMS) in association with unilateral mucinous cystadenoma of the ovary (MCO): case report and review of the literature.

The case of a patient with a cystadenoma with mesenchymal stroma (CMS) of the liver in association with asymptomatic unilateral mucinous cystadenoma of the right ovary (MCO) is reported. Immunohistochemical analysis of both lesions showed an identical profile. CA19-9 was expressed in the epithelial lining of both, intrahepatic CMS and MCO, without a parallel serum elevation. A probable common pathway of synchronous histogenesis for these lesions is hypothesised. The etiology of CMS remains obscure suggesting a multifactorial process implicating sex-related (hormone-dependent?) synergic and environmental factors, which probably act in different stages of organogenesis. Partial hepatectomy and right salpingo-oophorectomy were performed. Four years after surgery, the patient remains well with no evidence of residual or recurrent disease.

Enteric type villoglandular papillary adenocarcinoma of the uterine cervix associated with in situ squamous cell carcinoma. Case report and review of the literature.

A very rare case of enteric-type villoglandular papillary adenocarcinoma of the uterine cervix associated with in situ squamous cell carcinoma in a 21-year-old nulliparous caucasian woman is reported. Adjacent to the infiltrative component of an enteric-type lesion, cervical intraepithelial glandular neoplasia (CIGN) was also noticed. Histochemical detection of acetylated sialomucins demonstrates the enteric-type nature of this neoplastic (via metaplasia) process. The patient was treated by conization. Follow-up was uneventful one year after diagnosis.

Effect of cadmium pretreatment on liver regeneration after partial hepatectomy in rats.

Cadmium is a rare element that is nevertheless widely distributed throughout the biosphere and its toxic effects are becoming potentially more serious due to industrialization. Liver regeneration can be considered as a spectacular example of controlled tissue increase. In this study we examined the effect of cadmium pretreatment, administered 24 h before partial hepatectomy, on the liver regenerative process in rats, at different time intervals. The rate of 3H thymidine incorporation into hepatic DNA and the activity of the enzyme thymidine kinase were used as indices of liver proliferative capacity. Thymidine kinase, the rate-determining enzyme of DNA biosynthesis, was suppressed during the first hours following partial hepatectomy in the liver of cadmium pretreated animals. DNA biosynthesis was also strongly decreased in cadmium pretreated animals, by delaying the first peak of liver regeneration, compared with the partially hepatectomized ones. Biochemical parameters, mitotic index and proliferating cell nuclear antigen staining were also coestimated. The above data suggest that cadmium pretreatment suppressed the hepatic regenerative process, probably due to the inhibition of thymidine kinase.

Effect of cadmium on liver regeneration after partial hepatectomy in rats.

Cadmium is a nonabundant element that is widely distributed throughout the biosphere and its toxic effects are becoming potentially more serious due to industrialization. It has been reported that cadmium might interact with nucleic acid biosynthesis. In this study we examined the effect of cadmium administration, either 24 hr before or simultaneously to partial hepatectomy, on the liver regenerative process in rats, at different time intervals. The rate of DNA synthesis was suppressed markedly in the cadmium pretreated group and the first peak of liver regeneration was delayed, compared to the simply partially hepatectomized one. The administration of cadmium simultaneously to partial hepatectomy, caused a marked decrease of the rate of DNA biosynthesis, compared to the pretreatment. The rate-determining enzyme thymidine kinase was suppressed in the liver of both cadmium-treated groups. Biochemical parameters and histological findings were also coestimated. The above data suggest that either pre- or simultaneous administration of cadmium, suppressed the liver regenerative process, probably due to the inhibition of thymidine kinase.

Proliferating cell nuclear antigen (PCNA) in medullary thyroid carcinoma.

We investigated the expression of proliferating cell nuclear antigen (PCNA) in paraffin sections from 20 cases of medullary thyroid carcinoma (MTC). Follow-up data were available in eleven cases. PCNA index positively correlated with the degree of cellular pleomorphism (grade) of the tumor (p < 0.01), the pathologic stage (p < 0.01) and the poor clinical outcome (p < 0.05). These findings suggest that PCNA may be of prognostic significance in MTC.

Silver-binding nucleolar organizer regions in benign and malignant prostatic lesions.

Thirty-six transvesical or transurethral prostatectomy cases were selected from the histopathology files of the Laiko General Hospital. Among the 36 cases, there were 10 with benign prostatic hyperplasia (8 patients having distinct areas of adenosis) and 26 cases of prostatic adenocarcinoma (6 grade I, 13 grades II and III and 7 grade IV carcinomas). From each case, silver-binding nucleolar organizer regions (AgNORs) have been counted in sections of routinely processed paraffin-embedded tissue blocks. The mean AgNOR count per case was calculated. For the cases of prostatic hyperplasia, the mean AgNOR count was found to be 2.95 +/- 0.42, for adenosis 3.45 +/- 0.56, for grade I adenocarcinoma 4.97 +/- 0.74, for grades II and III 7.31 +/- 0.81 and for grade IV adenocarcinomas 11.41 +/- 1.68. This difference in the mean AgNOR count was found to be of statistical significance (p < 0.001) between adenosis and grade I adenocarcinomas and between grade II and III and grade IV adenocarcinomas. It appears that AgNOR counting may prove to be of benefit in differentiating between some benign and malignant prostatic lesions and that it might provide information concerning the biological behavior of prostatic adenocarcinomas.

Comparative assessment of proliferating cell nuclear antigen immunostaining and of nucleolar organizer region staining in transitional cell carcinomas of the urinary bladder. Correlation with other conventional prognostic pathologic parameters.

The expression of two cell proliferation indices, the proliferating cell nuclear antigen (PCNA), using the monoclonal antibody PC-10 in the immunoperoxidase method, and the nucleolar organizer regions (NORs), using the colloid silver nitrate staining technique, was assessed in formalin-fixed paraffin-embedded material of 50 transitional cell urinary bladder carcinomas. A relationship was found between the histologic grade and each of the two indices used. A significant difference was observed, particularly between carcinomas of grade II and III (p < 0.001). A relationship was also demonstrated between each of PC-10 and AgNOR scores and the clinical stage, but it was attributed mostly to the close correlation of the latter with the histologic grade of these tumors. The linear correlation coefficient between PC-10 and AgNOR scores was 0.757 (p < 0.001). Our results suggest that PC-10 and AgNOR scores may be important prognostic indices in urinary bladder cancer.

Expression of proliferating cell nuclear antigen (PCNA) and nucleolar organizer regions (NORs) in multiple myeloma.

We have studied 72 cases of multiple myeloma (MM) bone marrow trephine biopsies using the monoclonal antibody PC-10 and the colloid silver nitrate method for identification of proliferating cell nuclear antigen (PCNA) and nucleolar organizer regions (NORs), respectively. PC-10 and AgNOR scores were statistically significantly different between low versus intermediate and low versus high grade of malignancy (P < 0.001), whereas their difference was not significant between intermediate and high grade. A strong correlation was identified between these two proliferation-associated indices in each histological grade. There is evidence that the proliferative state of myelomas belonging to the intermediate grade is approximately the same as that observed in high grade (blastic) ones.
