[Results of the operative treatment of severe second degree stress incontinence in 425 cases (author's transl)].

The results of the operative treatment of 425 women with severe stress incontinence grade II and grade III according to Ingelman-Sundberg are reported. The following types of operation were used: diaphragmaplasty (DP), urethrovesicosuspension Marshall-Marchetti-Krauz (MK), lyoduraslings (LDS), and puborectalisplasty of Franz-Ingelman-Sundberg (PRP). The choice of the operative method and the evaluation of the results of the operation were done by history, clinical examination and radiological examination. The urodynamic investigation with a microtransducer which we used since 1977 prior and after each operation for stress incontinence does have little influence on the choice of the operation. The value of urodynamic studies on a quantitative evaluation of the type of operation is at present doubtful. Our definitive evaluation of the result of the operation was done not earlier than one year following the operation in a prospective stress incontinence clinic. Because of our individual treatment plan from the onset by different methods 89% of the 425 patients were cured or markedly improved.

[Markers of viral replication (HBeAg and DNA polymerase activity) in chronic uremia, HBsAg positive, hemodialysis patients].

The contagiousness of hepatitis B virus (HBV) carriers with end-stage renal disease undergoing chronic hemodialysis has been ascribed to an immunologic tolerance for HBV antigens, especially hepatitis B core antigen, supporting persistently high levels of virus replication. In this context hepatitis B e antigen and core-associated DNA polymerase (DNA P) activity have proved to be distinct markers of HBV replication. In order to evaluate the potential infectivity of these subjects, thirty-five HBsAg positive hemodialysis patients were studied for the presence of HBeAg/anti-HBe system correlating the results with serum DNA P activity. Twenty out of 35 patients were HBeAg positive (57%) and 21 DNA P positive (60%). A highly significant correlation (P less than 0,001) was recorded between detection of HBeAg and presence of serum DNA P activity. These findings confirm that the majority of hemodialysis patients carrying HBsAg show high levels of virus replication so that the determination of HBeAg and DNA P activity other than HBsAg is required for the identification of patients highly infectious.

[Surface antigen (HBsAg) and core antibody (anti-HBc) in chronic uremia during periodic hemodialysis treatment of hepatitis B patients. Correlation with viral replication markers (HBeAg and DNA polymerase activity)].

Sera of thirty-five hepatitis B surface antigen (HBsAg) positive hemodialysis patients and fifty-three asymptomatic HBsAg chronic carrier were studied to assess the relationship between markers of virus activity (hepatitis B e antigen and core-associated DNA polymerase activity) and titers of HBsAg and of antibody to hepatitis B core antigen (anti-HBc). All sera were tested by solid-phase radioimmunoassay methods. HBeAg was detected in 20 (51%) hemodialysis patients and in 14 (26%) asymptomatic carriers, whereas DNA P activity was present in sera of 21 (60%) and 17 (32%) respectively. The highest titers of HBsAg and anti-HBc expressed as P/N ratio between positive and negative controls, were detected in the majority of hemodialysis patients, whereas asymptomatic carriers showed low titers of these markers. These data suggest that in HBsAg positive hemodialysis patients a more active viral replication occurs and further underline the difference between these patients and other categories of HBsAg carriers in terms of infectivity.

[Invalidity after radiotherapy of the cervix carcinoma, stages II and III (author's transl)].

At least five years after combined radiotherapy of the cervix carcinoma, stage II and III, 67 patients were examined in order to find out whether they were invalid or not. Despite of conventional percutaneous radiotherapy there were no cutaneous damages. About half of all patients had late lesions of the bladder and the bowel. The capacity for work was limited only in a third of all patients. The lesions of the vagina and the consequences of the partly precocious postmenopause were judged to be the most serious damages. Only one quarter of all patients had a normal sex life.

[Serodiagnosis of acute hepatitis A].

Hepatitis A antibody (anti-HAV) was detected by specific radioimmunoassay (RIA) method in sera from 10 patients with acute icteric hepatitis. Anti-HAV was detectable in many subjects very early before the onset of jaundice, but the diagnosis of type A hepatitis in all patients was confirmed by the demonstration of seroconversion during convalescence. Since the initial antibody detected by RIA is predominantly IgM, while IgG specific anti-HAV appears later reaching peak levels within 1 to 2 months, we treated serum specimens of these patients with 2-mercaptoethanol (2ME) in order to differentiate acute-from convalescent-phase hepatitis A sera. Inactivation of IgM fraction with 2ME produced a significant reduction of anti-HAV titer only in acute-phase sera, so that this procedure may be used for early diagnosis of acute type A hepatitis.

Ultrasonic detection of "hourglass membranes" with funic presentation.

Two cases of prolapse of the amniotic sac through a partially dilated cervix, the so-called "hourglass membranes," are presented. In both cases, ultrasonography revealed the umbilical cord in the prolapsed sac. Both fetuses were grossly immature, and efforts were made to delay labor. In neither patient did rupture of the membranes occur. Fetal distress developed in 1 case, necessitating an emergency cesarean section shortly after the initiation of tocolytic therapy. A 27-week, 800-g infant was delivered who subsequently died. The second case resulted in a 3-week delay in the delivery of a viable fetus who did well. It is concluded that when confronted with a very immature fetus, aggressive measures to delay delivery are justified.

[Granulosa cell and thecacell tumors in 25 cases (author's transl)].

Granulosa cell tumors are derived from specific ovarion stroma and have the ability to produce hormones. In the 25 cases of granulosa cell tumors no concommitant endometrial carcinoma was found. Because of the varying degree of malignancy tumors which contain granulosa cells must be differentiated from pure theca cell tumors. Granulosa cell tumors are frequently malignant. Theca cell tumors are benign. The treatment of granulosa cell tumors is fundamentally the same as that of epithelial cancers of the ovary. The treatment of theca cell tumors is conservative.

Fetoscopy in the practice of perinatology and obstetrics.

The clinical indications for fetoscopy in perinatal medicine are encompassing an ever widening circle of inherited diseases and teratologically induced disorders that can now or potentially be diagnosed in utero. Fetoscopy, which has evolved over the past 20 years into a clinically accepted procedure, is not without risk to the pregnant mother and the fetus. When the decision is made to perform the procedure, its merits should outweigh its inherent risks and it should be performed by a perinatologist experienced in the technique. Fetoscopy is appropriate when the fetus is at risk for teratologically induced malformation, inherited blood dyscrasias, sex-linked or autosomal abnormalities with gross malformations, or neural tube disorders that cannot be diagnosed by amniocentesis. Technical improvements should result in increased refinement and wider applicability of the technique in the future.

[The adenocarcinoma of the cervix: study of 81 cases (author's transl)].

The present study deals with 81 primary adenocarcinomas of the uterine cervix diagnosed and treated in the departement of gynecology and obstetrics of the University of Zürich from 1950 through 1977. Clinical and morphological data such as patients age, character and duration of symptoms, clinical and anatomical stage, histological differentiation of the tumor and therapy were recorded and analysed in relation to patients survival. Of all these factors the stage of the tumor at time of diagnosis proved to have the highest prognostic significance.

Christmas disease (haemophilia B) in a girl with deletion of the short arm of one X-chromosome (functional Turner syndrome).

A I-year-old girl with severe Christmas disease and a factor IX content less than I% of normal is described. The family history was negative and coagulation studies on her relatives were normal. Genetic investigation showed an XXp-karyotype with deletion of the short arm of one X-chromosome, a cytogenetic variant of Turner syndrome. The transmission pathway of the haemophilia gene is discussed.

Bilateral renal artery aneurysm: percutaneous treatment with stent-graft placement.

A 51-year-old man with an 8-year history of hypertension (170/115 mmHg with two drugs) and altered renal function (5.6 mg/dl serum creatinine, 101 mg/dl BUN) was referred to our Department to evaluate the renal arteries and rule out renovascular hypertension. Doppler ultrasound and magnetic resonance angiography revealed significant bilateral renal artery stenosis and the presence of bilateral renal artery aneurysms. A self-expandable polytetrafluoroethylene (PTFE)-covered nitinol stent-graft was deployed in each renal artery to treat the stenoses and to exclude the aneurysm. Postprocedural digital subtraction angiography confirmed the resolution of the renal artery stenoses and the complete exclusion of the aneurysms. At the 6 month follow-up, color Doppler confirmed normal patency of the renal arteries with complete exclusion of the aneurysms and significant reduction of the blood pressure (130/85 mmHg with one drug) and serum creatinine levels (2.1 mg/dl).

Effect of information campaign by the mass media on hysterectomy rates.

The annual frequency of hysterectomy was monitored in the Canton Ticino, Switzerland, from 1977 to 1986. From February to October, 1984, there was a public information campaign in the mass media about rates of and need for hysterectomy. After the start of the campaign and during the following year the annual rate of operations per 100,000 women of all ages dropped by 25.8%, whereas in the reference area (Canton Bern), where no information was given to the public, hysterectomy rates increased by 1%. In the same period the hysterectomy rate per 100,000 women aged 35-49 declined by 33.2%, and the number of hysterectomies performed annually per gynaecologist decreased by 33.3%. In Canton Bern these rates were unchanged. The decline began 2 months after the start of the information campaign. The reduction in the number of hysterectomies was greater (p less than 0.001) in non-teaching hospitals (31.9%) than in teaching hospitals (18.1%). Information on regional rates and on the need for hysterectomy given through the mass media to the general population can change professional practices.
