[Physiopathology in accommodation and presbyopia, clinical and surgical approach].

Accommodation is a complex of phenomena by which the refracting power of the eye changes for focusing on near vision. Accommodative amplitude declines progressively with increasing age. Until today, this phenomenon called presbyopia was explained by Helmholtz's hypothezis which involves only ciliary muscle. It seems that in accommodation are involved, besides the ciliary muscle, the lens and extraventicular elastic components, biophysical dynamics of the lens capsule, vitreous, iris. According with the role of these factors and theirs pathophysiological implications, new surgical technique has developed to reverse presbyopia in the human eye (Surgical reversal presbyopia, Anterior ciliary sclerotomy) and offers a potential for understanding ocular hypertension and treatment of primary open-angle glaucoma.

[Lingual hemangioma--research and treatment limits and possibilities (a clinical case)].

The vascular tumours represent a special pathology in the otorhinolaryngologic practice, taking, many times, an interdisciplinary aspect. The authors present a case of a lingual hemangioma, that has been surgically treated in the ORL section of the Sibiu County Hospital, thus bringing forward, with reference to the presented case, the pathology of the vascular tumours in the ORL domain, that is the vascular tumours of the buccal cavity. As the conservatory methods of treatment include the possibility of a relapse, we consider that, in some cases, surgery is to be preferred.

[The physiopathology of primary cataract].

The alteration of the lens with the age are mainly due to the diminishing of the energetic metabolism, whose consequence is the diminishing of the synthesis of poliphosphate nucleotides and RNA, decreasing also the protein synthesis. From a biomolecular point of view alterations of the DNA appear, because of the increase in the activity of ADP-ribosyl transferasis with the age. The progress of the cellular biology and the knowledge of the physiopathological mechanisms implied in the opacification of the lens, opens new prospects in the prevention of this form of cataract.

[Extracapsular extraction of the crystalline lens].

From June 1, 1990 to September 1, 1995 we have operated within our department o Ophthalmology 759 eyes at 714 patients using the method of Extra-Capsular Extraction (EEC). From these 759 eyes, 242 (31.88%) were subjects of an artificial lens implant, 239 eyes were subjects of posterior chamber implant and 4 of anterior chamber. The used technique was the classical EEC one. The patients' age was between 8 and 93 years, with the maximal frequencies at the groups of 60-70 years (43.83%) and 70-80 years (35.57%). Considering the type of cataract we have operated 12 (1.58%) traumatic cataracts, 81 (10.67%) pre-senile cataracts and 621 (81.81%) senile ones. The most frequent complications were: capsule breaking and zonular dialysis, 8 cases (1.05%). vitreous out going, 12 cases (1.57%), post-surgical oedema of cornea, 132 cases (17.5%). increase of intraocular pressure, 158 cases (20.81%), irido-ciliary inflammation. 45 cases (5.9%), endophthalmitis, 3 cases (0.39%), oedematous-bubbling keratopathy, 6 cases (0.79%), macular cystoid oedema, 2 cases (0.26%), posterior capsule opacification, 52 cases (6.8%). The results we have obtained are good: 530 patients (69.8%) have a visual acuity of 0.5 or better. The rest of the patients have a lower visual acuity because of intra- or post-surgical complications as well as exfoliation syndrome, diabetes, glaucoma, myopia, pigmentary retinopathy and Fuchs' heterochromia. The artificial lens implant has increased, reaching in 1995 a total of 78.61%.

[The exfoliative syndrome. The risk factor in extracapsular surgery of the crystalline lens].

The exfoliative syndrome is a risk factor of the extracapsular surgery of the crystalline. Thus, on a lot of 868 operated eyes for cataract through EEC +/- IOL, 10 were founded presenting the exfoliative syndrome (1,15%). On these cases it was possible to mark the increased incidence of the intra- and postoperative complications, such as the insufficient dilatation of the pupil, the tearing of the posterior capsule, the loss of the vitreous, the modifications of the corneal endothelium, the increasing of the postoperative intraocular pressure, as well as the more frequent opacification of the posterior capsule.

[Diabetes and extracapsular extraction of the crystalline lens].

The present study proposes itself to display the behaviour of eyes proposed for operation of cataract in case of the existence of diabetes. Therefore on a lot of 868 operated eyes for cataract using the EEC +/- IOL method has been found that 28 eyes of having diabetes required antidiabetic treatment. From these, 9 didn't present diabetic retinopathy, 15 had diabetic background retinopathy and 4 presented proliferative diabetic retinopathy without rubeosis iridis or neovascular glaucoma. From these, 9 were operated through EEC and 19 through EEC +/- IOL. The intra- and postoperative complications were more frequent at the patients having diabetes. There weren't marked differences regarding the implant of the artificial crystalline. The operation of cataract brings to the appearance or to the increasing of the gravity of ocular phenomena of the diabetes. Thus, the diabetes retinopathy grows progressively worse after the surgical intervention, inflammatory phenomena of the anterior segment of the eye are more frequent, endophthalmitis appears more frequently due to the low immunity of the sufferings of diabetes. The method to be elected for solving the cataract of the sufferings with diabetes mellitus remains EEC +/- IOL.
