Liver metastases from extremity soft tissue sarcoma.

Liver involvement by soft tissue sarcoma is an unfavorable prognostic factor for survival. Complete resection of liver metastases can bring improvement in selected patients, but chemotherapy remains the only palliative treatment option for most. Anecdotal long-term survival of patients with unresectable liver metastases treated with systemic chemotherapy has been reported, such as the patient presented here.

Systemic lithium reabsorption from lithium-chloride-coated heat and moisture exchangers.

OBJECTIVE: To evaluate possible reabsorption and systemic effects of lithium released by lithium-chloride-coated heat and moisture exchangers (HMEs) during prolonged mechanical ventilation. DESIGN: Prospective study, including all patients mechanically ventilated for 5-30 days. SETTING: A 7 bed general-traumatological ICU in a University Hospital. PATIENTS: 27 consecutive ICU patients, admitted following trauma, neurosurgery and respiratory insufficiency, mechanically ventilated for at least 5 days, with a lithium coated hygroscopic HME in the circuit. MEASUREMENTS AND RESULTS: Serum lithium levels were measured daily, with a standard laboratory spectrophotometric method, from admission to discharge from the ICU, and showed a reabsorption of lithium in all the patients; in the adults, levels were 5 to 15 times lower than therapeutic range, while in a child therapeutic and even toxic levels were reached. CONCLUSIONS: LiCl coat enhances HMEs' performance greatly, but reabsorption and systemic action must be considered. In adults, serum lithium levels were lower than the therapeutic range, but lithium is effective at low concentrations and it has a narrow therapeutic range; moreover, toxicity can be observed within this range too. In children, the risk of toxicity is much greater. When lithium coated HMEs are used, the risk/benefit ratio between good performance and systemic reabsorption must be evaluated carefully.

[Auditory evoked potentials of the brain stem in brain injury. Prognostic value].

The study of brain stem auditory way in severe head trauma patients with auditory evoked potentials is very important for giving elements about functional and anatomic situation of analyzed neuronal substratum; ABR will be normal or altered in conformity with the level and/or the gravity of the damage. Authors have examined 20 post-traumatic comatose patients pointing out the existence of a strict relation between auditory brain stem responses and prognosis. Authors, in accordance with the ample literature in existence, agree to point out the auditory brain stem responses such as a very effective instrument for the prognostic valuation of post-traumatic comatose patients.

[Antibacterial chemotherapy in resuscitation. Our clinical experience].

Experience with antibiotic management in a resuscitation centre between 1974 and 1978 is described. An account is given of the incidence of bacteria in excreate, urine, blood, etc. cultures, and of the sensitivity of germs most commonly encountered in such centres to single antibiotics. It is shown that Gram-negative bacteria, particularly Pseudomonas aeruginosa, predominate, and the way in which their sensitivity to antibiotics varies in the course of time is examined. Preparation of an "overall antibiogram" based on assessment of the data made it clear that aminoglycosides and cephalosporins are the drugs of choice for Gram-negative forms, and cephalosporins for Gram-positive bacteria. A critical survey of the results leads to the suggestion that four changes should be made in the approach to antibiotic therapy: 1) elimination of the practice of "preventive" therapy in the form of "initial cover"; 2) withdrawals on admission for bacteriological determinations; 3) abstention or administration of a low-toxicity antibiotic (one of the penicillins) in the case of "minor" infection; 4) employment of potent bactericides of the aminoglycoside group or the polymixines only in cases of superinfection with evidence of septicaemia supported by an antibiogram. It is felt that the essential premiss to sound antibiotic management is the removal of other routes where by infection can enter. Drastic supplementary measures must be taken with regard to the architecture of the premises and the sterilisation of material. Staff training is also essential to prevent the spread of infection from hyperseptic patients.
