Tumor markers carcinoembryonic antigen, tissue polypeptide antigen, and carbohydrate antigen 19/9 in liver diseases.

The tumor markers carcinoembryonic antigen (CEA), tissue polypeptide antigen (TPA), and carbohydrate antigen 19/9 (CA19/9) have been compared with regard to their sensitivity to liver diseases. All three markers have been found to be sensitive to liver diseases, but to a different degree: TPA rises the most; CEA the least. The sensitivities of TPA and CA19/9 are higher in a group of liver diseases than, for comparison, in a group of colorectal carcinomas.

Comparison of the tumor markers CEA, TPA, and CA 19-9 in colorectal carcinoma.

In 103 patients with colorectal carcinoma carcinogenic antigen, 19-9 (CEA), tissue polypeptide (TPA) and carbohydrate antigen (CA 19-9) were measured in serum. The values determined in these patients and in a control group have been converted into specificity-sensitivity diagrams. Comparison of diagrams of the three markers showed that CEA has the greatest sensitivity in colorectal carcinoma. Different ways to enhance the sensitivity by combination of the markers have been tried. By combination, however, the sensitivity of the single markers is not essentially enhanced.

[Diverticulosis of the jejunum and ileum].

Six cases of diverticulosis of the small intestine are reported in this paper. Described in some detail is the pathological pattern of diverticula in jejunum and ileum. There are chronic courses with recurrent complaints in the epigastric region and with malabsorption syndrome which may lead to megaloblastic anaemia, steatorrhoea, and dermatosis. Acute processes are caused by perforation, diverticulitis, and haemorrhage. Ileus can develop owing to strangulation or, in less frequent cases, owing to incarceration of enteroliths. Chronic pneumoperitoneum may be attributable to massive jejunal diverticulosis.

Reaction of the tumor markers CEA and TPA to a surgical trauma.

The level of the tumor marker tissue polypeptide Antigen (TPA) rises distinctly after a surgical trauma. The rise begins between 3 and 6 days after operation; the elevation lasts for more than 6 weeks. Carcinoembryonic antigen (CEA) shows no comparable reaction.

[Tissue concentration and effectiveness of ofloxacin in surgical patients].

Following a 400 mg preoperative oral dose, ofloxacin concentrations were measured preoperatively in serum and at the time of tissue removal in serum, gall bladder fluid, the gall bladder wall, liver tissue, muscle, fascia, subcutaneous fat and skin. Serum values were more or less constantly above 3 mg/l at all times. The highest intraoperative ofloxacin concentration was 11.85 mg/l in the gall bladder fluid. In the gall bladder wall, liver and muscle, concentrations were slightly higher than in the serum, whereas concentrations in the fascia, subcutaneous fat and skin were clearly lower than serum concentrations. Good activity was observed with regard to the MIC90 of the pathogens most commonly encountered in surgical wards.

[Diagnostic error: incarcerated hernia].

Reported in this paper are 19 patients on whom incarcerated hernia had been pre-operatively diagnosed, which proved to be wrong during the operation. Various types of inflammation, including appendicitis in the hernial sac, as well as accumulation of pus in the hernial sac and abscesses, were the disorders which had led to false diagnosis in ten cases. Tumours were recorded from four patients and cysts in the round ligament of the uterus from another three, while a funicular hydrocele was detected in one patient. In one case, painful swelling through haematoma in the inguinal region had been caused by ruptured aortic aneurysm. Ultrasonographic diagnosis is recommended for all dubious cases. Surgery should be considered a preferable option in situations of lasting uncertain diagnosis, in order to avoid incarceration from remaining unnoticed.
