[Prevalence of rheumatoid factor in a single sampling of the urban population in Croatia].

The prevalence of rheumatoid factor (RF) in urban population was examined. A random sample was taken according to the register of voters and included 5% of the population. Among them 60% suffered from various rheumatic symptoms. Out of 500 persons invited to an examination, 231 examinees responded. None of the examinees in that moment showed signs of rheumatoid arthritis. RF is determined in serum according to Waaler-Rose and it was positive in 12 out of 231 examinees (5.2%) about equally in men and women (P > 0.05). The majority of examinees was over 50 years old. Titar RF increased with the age of the examinee, but difference is not significant (P > 0.05). RF is not only a characteristic of RA, but it could also be found during the other diseases which are accompanied by, first of all, hypergammaglobulinemia and fibrosis.

[Socioeconomic aspects of rheumatoid arthritis].

This study investigated the financial parameters of the hospital treatment and of the rehabilitation in the Centers for Rehabilitation for the patients with rheumatoid arthritis (RA). The control group consisted of patients who were ill or had been ill from some other diseases which were controlled at the physician of the primary health service. The average duration of the treatment of the patients with RA in hospital was 28 days (ranging from 21 to 35 days), while the average duration of the treatment of the patients from the control group was 18 days (ranging 7 to 45 days). 45 patients with RA were hospitalized and 22 from 65 patients from the control group (P greater than 0.05). The duration of the hospital treatment of 45 patients with RA was 2548 days and it was 1818 days for the patients from the control group (P = 0.039). The cost of one stay of 28 days in the hospital amounted 56.000 din per one patient with RA and it was 36.000 din per one patient from the control group (according to the price-list from 31. 7. 1991.). 49 patients with RA went for several times to the rehabilitation into the Centers for Rehabilitation and 11 from 65 patients from the control group (P less than 0.0001). The average duration of rehabilitation for the patients with RA was 23 days while it was 26 days for the patients from the control group (P greater than 0.06). The rehabilitation for the patients with RA lasted 4278 days and 832 days for the patients from the control group (P less than 0.0001).(ABSTRACT TRUNCATED AT 250 WORDS)

Evaluating the magnitude of socio-medical problems of rheumatic diseases in adult urban population.

In the course of 1982 and 1983 a study was carried out on the prevalence of rheumatic symptoms and the identification of rheumatic diseases in the urban population of the Zagreb commune of Medvescak. The representative sample consisted of 5% of the adult population of both sexes, amounting to 1999 examinees. Rheumatic symptoms were found in 59.6%; in 41.6% of women and 18 of men. The prevalence of the most frequent rheumatic disease and the influence of age and sex on the frequency of the onset of symptoms were determined. By random sampling, of the 1191 surveyed with symptoms (42%), 500 were invited for a rheumatological examination; 238 responded (47.6%). Persons with rheumatic, symptoms who did not respond to the invitation were also interviewed in order to find out the reason for their not coming to the examination. Rheumatic changes were found in 86% of the examined. Most frequently diagnosed were different locations of degenerative changes of the joints (48.3% of all diagnoses). The magnitude of the problem of rheumatic diseases was assessed, the estimates being drawn on the basis of complex probability. Based on the results and estimates, it was concluded that 35% of the observed adult population suffered from diseases of the locomotor system and that they needed treatment but that only half of them were being treated (16%). The overall magnitude of the problem of rheumatic diseases appeared to be within the expected limits, with a high percentage of degenerative diseases of the joints.

After the clinical and radiological examination of sacroiliitis joints, sacroiliitis on a various development level was found in 12 examinees (3.2%). Evidently, sacroiliitis is not rarity, but the diagnosis is still stated late.
