Atherosclerosis in the pediatric age with special reference to initiation, progression and regression of atherosclerotic lesions.

Fibromuscular intimal thickening begins to occur early in the pediatric age, with or without lipid deposition. Fatty streaks also appear first in infant. Fatty streaks occur almost always in close association with intimal thickening. Some fatty streaks remain unchanged as those seen in the ascending aorta. Intimal thickening and lipid deposition generally increase with age. The prevalence of those lesions is very high in infants and children. The degree and extent of those lesions display wide individual variability. Finally, intimal thickening is considered to be an integral part of atherosclerosis and to represent the first stage of atherosclerosis. Atherosclerotic lesions, at least a part of them, are derived from thrombosis in association with endothelial denudation. Hyperlipidemia accelerates the atherosclerotic process. Lipid-rich atherosclerotic lesions may regress to some degree under a long normolipidemic state but leave intimal fibrous scarring. Therefore primary prevention of atherosclerosis should be directed as well as secondary prevention.

Correlative evaluation of angiography and pancreatoductography in relation to surgery for cancer of the pancreas.

The results of preoperative angiograms and pancreatoductograms in 37 patients with cancer of the pancreas were correlated with each other, with the findings at laparotomy, and with survival times. Tumors which could not be detected by arteriography and/or pancreatography were resectable, except for those located in the uncinate process. Among the resectable tumors, those which were identified only by pancreatography were confined mostly to the parenchyma of the pancreas and were nearly 2 cm in size. Those patients were expected to have a long survival after total pancreatectomy. In contrast, those tumors which were diagnosed only by arteriography arose from the subcapsular portion of the gland or the uncinate process and showed macroscopic invasion to the capsule of the pancreas. These tumors, recurred within 1 year, despite total pancreatectomy. The resectable tumors showing both vascular and ductal invasion were more than 3 cm in size and extended far beyond the capsule of the gland, and patients with these tumors died within 1 year after operation. In this group no difference in survival time was seen among those treated by the Whipple procedure, those by total pancreatectomy, or those by resection of the total pancreas and the portal vein.

Scanning and transmission electron microscopic study of human pulmonary pneumocystosis.

Histopathologic examinations were performed in 13 autopsy cases with Pneumocystis pneumonia, five of whom were examined with scanning as well as transmission electron microscope. Findings by scanning electron microscopy were compared with those by light microscopy and transmission electron microscopy. By scanning electron microscopy, two principal structural varieties of Pneumocystis organisms, smooth-surfaced organisms and rough-surfaced organisms, could be classified. It was indicated that rough-surfaced organisms and smooth-surfaced organisms correspond respectively to trophozoite-type organisms and thick-walled cysts, recognized with transmission electron microscope. Intraalveolar honeycombed material was mainly composed of dilated trophozoite-type organisms, containing thick-walled cysts scatteredly.

Perinephritis hypertension in monkeys. I. An increase of plasma renin activity associated with increased permeability of retinal vessels and angionecrosis.

The renal parenchyma of 5 crub-eating monkeys (Macaca irus) was wrapped by cellophane, and plasma renin activity, blood pressure and vascular permeability of ocular ground were measured in comparison with 5 unoperated control monkeys. The results demonstrated that increases of systolic arterial pressure, plasma renin activity, and permeability of the retinal vessels were found in 4 operated monkeys. There was no such abnormal finding in the unoperated control monkeys. Generally there was a rough parallelism among levels of plasma renin activity and systolic blood pressure, an increase of permeability of retinal vessels and fibrinoid angionecrosis and/or necrotizing angitis similar to polyarteritis nodosa.

Intralobar pulmonary sequestration; with special emphasis on bronchial communication.

The radiologic and pathologic features of eight cases of intralobar pulmonary sequestration are reported. Special attention was given the communication between the cystic areas and the normal bronchial tree. In three of five surgical specimens, probing revealed such communications. Microscopically, the transition from the normal bronchus to the cysts revealed no inflammation to suggest fistulous communication secondary to infection. The possibility of preexisting congenital bronchocystic communications within the pulmonary sequestrations was therefore suggested.

Lipid profile in the evolution of experimental atherosclerotic plaques from thrombus.

Nonocclusive white mural thrombosis was induced in the abdominal aortae of normolipidemic rabbits by insertion of a polyethylene catheter into the abdominal aortae. The thrombus subsequently organized into intimal thickenings which resembled fibrofatty type atherosclerosis seen in man, showing large numbers of foam cells containing stainable lipid, fatty necrotic centers, cholesterol clefts, calcification, and fibrous caps. The lipid composition of the thrombus and lesions was followed at serial time intervals from 4 hours to 60 weeks. Lipid analysis showed significant concentrations of lipid in the early lesions and with time these lipid concentrations increased and later decreased. These studies demonstrate that the fibrofatty lesions derived from a white thrombus have significant amounts of the same lipids that characterize the atherosclerotic lesions of man, however, there is a lower proportion of cholesterol to the other constituents and a higher proportion of phospholipids. The free cholesterol and cholesteryl ester of the 4-day lesions were much greater than that of platelets alone indicating that significant amounts of plasma are trapped in a thrombus when it forms.

Renal subcellular fractions producing angionecrosis and increased vascular permeability.

In order to define the subcellular localization and characters of substances in the rat kidney which increase vascular permeability and produce angionecrosis, the following investigations have been undertaken: (1) subcellular fractionation by use of differential centrifugation and osmotic shock treatment with enzyme profile determination; (2) chromatographic separation of lysosomal contents with concanavalin A affinity column. Lysomal contents contained substances that induced an increase of vascular permeability of the rabbit skin and angionecrosis in the pancreas of the bilaterally nephrectomized rats and the spontaneously hypertensive rats. Lysosomal contents treated at 60 degrees for 30 min showed no renin activity and yet produced angionecrosis. Non-affinity fraction from concanavalin A column chromatography showed no renin activity but produced angionecrosis and an increase of vascular permeability of rabbit skin.
