[Hyperkalemia--life-threatening early complication of asphyxia in premature infants].

16 premature infants with hyaline membrane disease all under mechanical ventilation developed severe electrolyte disturbances during the first 48 hours. The serum potassium concentration increased over 8 mmol/l. After a delay of several hours the serum calcium concentration decreased to 1.0-1.4 mmol/l. The metabolic-respiratory acidosis (pH 6.92-7.19) was present in nearly all patients. The typical hyperkalemic cardiac arrhythmia appeared in 7 patients; six of them died. The quick shifting of potassium from the extracellular into the intracellular space by the glucose-insulin infusion was indispensable for the survival of our patients. The serum potassium concentration decreased by 2.9-5.1 mmol/l in 4 hours. When the cardiac arrhythmia is present calcium gluconate and bicarbonate must be infused immediately followed by the glucose-insulin infusion.

Allergic asthma and NBT dye reduction by granulocytes.

The nitro-blue tetrazolium dye reduction test according to modified Park's et al. method in asthmatic children with pollen allergy during remission and status asthmaticus was performed. In patients with status asthmaticus high rate of spontaneous NBT reduction by peripheral phagocytes was found. The routine investigations allowed to exclude bacterial infection as a cause of asthma attacks. It is suggested that allergic reaction may change cellular metabolism providing to increase of NBT reduction.

[Reversible bactericidal defect of leukocytes. Pseudomonas meningitis in a child with diabetes mellitus (author's transl)].

A 3-year-old boy with diabetes mellitus contracted meningitis caused by Pseudomonas aeruginosa. The disease lasted for several months in spite of antibiotic treatment. Diabetes mellitus was poorly controlled and hypoglycemia often occured. Immunoglobulin levels were normal. Investigations of leukocyte functions showed (no bactericidal activity), decreased phagocytic activity and very low NBT dye reduction by neutrophils during phagocytosis. Under the administration of polymixin, leukocyte transfusions and stabilization of glycemia the pathological clinical and laboratory findings disappeared within three weeks. Serial investigations of leukocyte function showed a gradual recovery of normal activity.
