Iodine-induced hyperthyroidism--an epidemiological survey several years after institution of iodine prophylaxis in Poland.

In 1997, the obligatory model of iodine prophylaxis was introduced in Poland in order to correct the existing status of mild and/or moderate iodine deficiency. In order to monitor possible side-effects of increased iodine supply, studies on iodine-induced hyperthyroidism were initiated by establishing several regional registers of hyperthyroidism. In the present paper, the results of a two-year monitoring (2000-2001) have been summarized. There are no epidemiological data on hyperthyroidism prior to starting the iodine prophylaxis, but the obtained current data are comparable to observations in other countries, made after iodine supplementation. The incidence of iodine-induced hyperthyroidism did not exceed the acceptable level, thus confirming--together with previous observations on the effectiveness of iodine prophylaxis--the adequacy of applied dose of KI (30 microg/kg NaCl), used for salt iodization in Poland.

[The influence of treatment of progressive edematous infiltrative ophthalmopathy on intraocular pressure].

PURPOSE: To evaluate the influence of systemic steroid therapy and retrobulbar irradiation on intraocular pressure (IOP) in patients with infiltrative--oedematous Graves' ophthalmopathy. MATERIAL AND METHODS: We examined 76 patients divided into 3 groups: I--treated by irradiation only (15 patients), II--treated by irradiation and oral prednisone therapy (26 patients), III--treated by irradiation and intravenous methylprednisolone pulse therapy (35 cases). All patients underwent full ophthalmological examination (including IOP measurement, perimetry and gonioscopy) before, during, immediately after and 2-20 months after treatment. RESULTS: Increased IOP (21-31 mm Hg) was observed in 54 patients (71%) before treatment. The iridocorneal angle was open in all eyes. Changes in perimetry were not characteristic for glaucoma. IOP was higher in patients with more severe ophthalmopathy. We recorded transient increase of IOP during treatment in only 3 patients. Increased IOP immediately after therapy was observed in 16 patients with severe symptoms and signs of ophthalmopathy: in group I--4/15 (27%), in group II--4/26 (15%), in group III--8/35 (23%). Higher IOP was recorded in 10 patients two to twenty months after completion of treatment: from group I--4/15 (27%), from group II--1/26 (4%) and from group III--5/35 (14%). In 6 of these 10 persons we observed recurrence of ophthalmopathy, in 4 patients higher IOP was the only deviation, they needed local therapy. The mean values of IOP were lower in patients treated by steroid therapy in comparison to patients treated by irradiation only. The most rapid improvement of clinical status was observed in patients treated by methylprednisolone pulse therapy. CONCLUSIONS: The increase of IOP in patients with Graves' ophthalmopathy correlates with severity and duration of eye disease. Systemic steroid therapy is more efficient in reduction of IOP than irradiation of the retrobulbar tissue. Our results suggest that combined therapy is a preferable method of treatment of progressive ophthalmopathy, including cases with increased intraocular pressure.

The detection of somatic mutations of thyrotropin receptor gene in fine needle biopsy samples from thyroid nodules.

OBJECTIVE: To evaluate the detection possibility of TSH receptor gene mutation within the third cytoplasmic loop and the sixth transmembrane domain in the cytological material obtained by means of fine needle biopsy of autonomous and non-autonomous nodules. METHODS: The study has been carried out in 16 women with goitre showing no clinical signs of hyperthyroidism. According to the thyroid scintigraphy and serum level of thyrotropin (TSH) the patients were divided into two groups: 1. 6 patients with autonomous nodules; 2. 10 patients with non-autonomous nodules. Genomic DNA has been isolated from the cytological material and the peripheral blood nuclear cells in order to confirm possible somatic character of TSH receptor gene mutations. DNA has been amplified in polymerase chain reaction (PCR) with the use of a specific pair of primers. Purified PCR products have been subjected to further automatic sequencing. RESULTS: Among 6 autonomous nodules tested one heterozygotic somatic mutation of adenine for cytosine at 1804 nucleotide of TSH receptor gene was detected. This mutation resulted in the change of threonine (codon ACC) at 632 position of TSH receptor protein for proline (codon CCC). Among the non-autonomous nodules one heterozygotic somatic mutation of adenine for cytosine at 1870 nucleotide of receptor TSH gene has been detected. From this mutation followed the change of lysine (codon AAG) at 624 position of the polypeptide chain for glutamine (codon CAG) followed as a consequence. CONCLUSIONS: We emphasize the validity of fine needle biopsy in the detection of somatic mutations in the TSH receptor gene. For the first time the somatic mutation in the TSH receptor gene in a non-autonomous nodule has been reported.

Exaggerated natriuresis induced by sodium chloride infusion in essential hypertension is accompanied by an exaggerated urinary 3' 5' guanosine monophosphate excretion.

The effects of an intravenous infusion of physiological saline on plasma atrial natriuretic peptide (ANP), guanosine 3' 5' monophosphate (cGMP) concentrations, and on urinary cGMP and sodium excretion were studied in 13 patients with essential hypertension, class I according to WHO criteria, and in 10 healthy subjects. It was found that the groups did not differ as to basal and infusion-induced plasma ANP and cGMP and basal urinary cGMP and sodium excretion, but the sodium chloride infusion resulted in a significantly greater urinary cGMP and sodium excretion and creatinine clearance in hypertensive than in control subjects. The results of this study demonstrate that patients with essential hypertension respond to an intravenous sodium chloride load not only with exaggerated natriuresis, but also with augmented urinary cGMP excretion. The latter finding may in part be due to a greater glomerular filtration of cGMP, but increased renal contribution cannot be excluded. Apart from the possible stronger intrarenal effect of ANP on cGMP production in patients with hypertension, independent direct effect of volume expansion on cGMP excretion and modified activity of other cGMP generating systems may all be responsible for the higher urinary cGMP excretion in essential hypertension.

Iodized salt consumption, urinary iodine concentration and prevalence of goiter in children from four districts of north-western Poland (Szczecin coordinating center).

The study was aimed at evaluation of iodized salt consumption, urinary iodine concentration and incidence of goiter in children from four districts of north-western Poland: Szczecin, Koszalin, Słupsk and Gorzów Wlkp. The study was a part of the national programme: "Investigations of iodine deficite and iodine prophylaxis in Poland". The investigations were performed in ten schools randomly selected by Coordinating Centre in Kraków. Altogether 1793 children attending these schools (838 boys and 955 girls) of age between 6 ad 13 years, living in the cities and villages of coastal and lowland region were studied. The examination included interview in the form of a standard questionnaire, physical examination of the thyroid according to the WHO criteria, ultrasonographic evaluation of thyroid volume and determination of iodine concentration in single urine specimen. It appeared that only 11.2% of children used to consume iodized salt. Mean iodine concentration in urine was 76.2 micrograms/l both in children consuming and not consuming iodized salt, indicating dietary iodine deficiency. The incidence of goiter in country population of children (12.9%), indicated that the region of north-western Poland should be considered as an area of mild goiter endemy. These results suggest a need for iodine supplementation of edible salt in this region of Poland.

[Frequency of goiter in children and adolescents in the Szczecin region].

The study was carried out in 2153 children and adolescents (1066 girls and 1087 boys) aged 5-20 years. The examined group was chosen randomly, according to a simple drawing scheme. The aim of the study was to evaluate frequency, size and character of goiter in developmental age population in Szczecin's region. The goiter was found in 14% of examined population (18.4% in girls and 9.8% in boys). Frequency of goiter approximated to 37% in girls and 16% in boys during puberty spurt (between 10 and 16 year of age). Small degree of thyroid enlargement was predominant in examined population. Large goiter was present more frequent in girls than in boys. We did not find clinical symptoms of thyroid gland dysfunction in examined group. Frequency of nodular goiter was 7% of children's and adolescent's population with higher incidence in boys (9%) than in girls (6%).

[Effectiveness of iodine prophylaxis and frequency of thyroid enlargement (thyroid goiter) and clinical diagnosis of thyroid diseases in inhabitants of the Szczecin region after the Czernobyl accident].

The study, supported by program MZ-XVII, was carried on 4567 inhabitants of the area of Szczecin (2350 females and 2217 males). The population was chosen randomly, according to a simple drawing scheme. All subjects were clinically examined using standardised questionnaires. In 3468 persons (including 1807 girls and women, 1661 boys and men) apart form clinical examination, the assessment of thyrotropin, thyroxine and triiodothyronine in serum and frequency of antithyroglobulin antibodies and antithyroid membrane antibodies were evaluated. The data indicate that 94% of children in Szczecin's region received the prophylactic dose of iodine, mostly between the 1st and the 5th of May 1986. Only 17% of the adults received iodine. The most common preparation was Lugol solution given in a single dose. Among all persons who received iodine, only in 5% of subjects the side effects were noted (mostly in children), including symptoms of gastrointestinal tract (vomiting, abdomen pain) and occasionally intrathyroid side effects (thyroid pains). In examined population the high frequency of thyroid enlargement, mainly in women (up to 43-44% at the age group 30-50 years) was found. The frequency of clinical diagnosis of thyroid disease was higher in women than in man (most often the diffuse goiter, rarely the nodular goiter). The frequency of thyroid enlargement and clinical diagnosis of thyroid disease was not dependent on prophylactic iodine intake. The iodine prophylaxis did not influence on thyroid hormones and TSH serum levels and on frequency of antithyroid antibodies.

[Analysis of early results of combined treatment with glucocorticoids and telecobalt therapy for Graves' disease ophthalmopathy].

Early results of combined use of glucocorticoid administration and irradiation with radioactive cobalt for treatment of oedematous-infiltrative ophthalmopathy associated with Graves' disease have been analyzed in a group of 33 patients including 28 women and 5 men of age between 25 and 66 years (mean age 47.3 years). The combined therapy was a modification of the original method of Bartalena et al. which consisted in the gradual increase of the initial dose of glucocorticoids and prolongation of the period of administration of the drug. The ophthalmic lesions were assessed by thorough ophthalmologic examination and classified according to Werner. The ophthalmopathy index was calculated according to Donaldson. Satisfactory results of treatment have been obtained in 32 patients, with 9 patients being completely relieved from any objective or subjective ophthalmic symptoms (very good results), and 23 patients having still small afflictions originating from the soft tissues of the eye socket, exophthalmos, diplopia during marginal vision and a decreased visual acuity (good results). The clinical recovery was mostly connected with the improvement in the condition of soft tissues of the eye socket, cornea and external ocular muscles and, to a smaller extent, exophthalmos which persisted to some degree and acuity of vision not always attaining normal values. In one person the results of treatment were unsatisfactory despite some improvement. Very good and good results obtained in 97% of patients indicate that the administration of glucocorticoids combined with the irradiation of retrobulbar tissues with radioactive cobalt can now be regarded as the most effective method of treatment of the progressive oedematous-infiltrative ophthalmopathy.

[Frequency of thyroid enlargement (goiter) in residents of the Szczecin region].

The study has been carried out on 4,565 persons (2,349 girls and women also 2,216 boys and men) aged 4-64. The examined group was chosen randomly, according to a simple drawing scheme. The aim of the study is to evaluate frequency, magnitude and character of goiter in residents in Szczecin region. The goiter has been found in 19.5% persons in examined population, so it makes possible an endemic state. In consideration of a fact that goiter appears frequently among women and there are more cases of thyroid enlargement (O-B and I degree), as well as a small number of people with disturbance thyroid function only slightly endemic character is stated. Because the frequency of modular goiter among people with goiter is 20% it doesn't express extent of endemic state.

[Serum thyroxine and triiodothyronine levels after a single dose and after 2-month-long thiamazole treatment of Graves' disease with reference to drug's pharmacokinetics].

The performed studies covered 48 subjects, inhabiting the Western Pomerania, therein 40 patients with hyperthyroidism in the course of Graves-Basedow's disease (32 females and 8 males, aged 21-64 years) as well as 8 healthy individuals (5 females and 3 males, aged 23-36 years, with negative anamnesis towards thyroid diseases), who made up the control group at determining the pharmacokinetic parameters after a single oral dose containing 60 mg of thiamazole. On the basis of estimating the time of treatment with "full dose" of thiamazole, indispensable for attaining clinical euthyreosis, according to criteria provided by Crooks et al., the patients with hyperthyroidism during Graves-Basedow's disease were divided into 2 subgroups: 1). Subgroup IA included 22 patients, in whom the clinical state of euthryreosis was obtained in 28 days of therapy with "full dose" of thiamazole. 2). Subgroup IB encompassed 18 patients, in whom euthyreosis appeared after at least 35-day-long treatment with a "full dose" of thiamazole. The differing behavior of thyroid hormones and thiamazole pharmacokinetics+ in both subgroups of patients has furnished the basis for the following conclusions to be drawn: The patients with hyperthyroidism in the course of Graves-Basedow's disease, attaining rapidly the clinical euthyreosis during the treatment with "full dose" of thiamazole, are found to normalize the concentrations of thyroxine and triiodothyronine in serum after shorter time than it is done by patients requiring a longer drug application in a "full dose". In patients achieving euthyreosis both after short and long time of treatment with "full dose" of thiamazole, the normalization in concentrations of thyroid hormones in serum markedly exceeds, by about 2-4 weeks, the establishing of clinical euthyreosis. In patients, who readily attain the clinical euthyreosis, the concentrations of thiamazole in serum, after a single "full dose" as well as during two-month-long treatment, are significantly higher than in patients reaching euthyreosis slowly, in spite of the fact that there were no outstanding differences in absorbing the drug from the alimentary duct in both subgroups being compared. The cause that the clinical effect varies in the compared subgroups of patients with hyperthyroidism during the Graves-Basedow's disease is the difference in thiamazole metabolism in the organism, most likely in the liver.

Expression of stem cell markers on mononuclear cells derived from heparinized cadaveric organ donors before and after disconnection from the respirator.

We have attempted to evaluate the level of the earliest human hematopoietic cell marker expression (CD34, CD117, CD133, CD184) on cells obtained from heparinized cadaveric organ donors before and after disconnection from the respirator. Moreover, we compared various cell populations: (1) coexpressing CD34/CD117; (2) CD34/CD133; (3) highly enriched hematopoietic stem cells (CD34+CXCR4+CD45+); and (4) highly enriched tissue-committed stem cells (CD34+CXCR4+CD45-). Finally, we analyzed whether the level of hematopoietic stem cell marker expression depended on the age of the donor. The expression of the membrane receptors (CD34, CD45, CD117, CD133, CD184) was studied by flow cytometry. We observed that the proportion of mononuclear cells expressing these markers slightly decreased in bone marrow harvested after disconnection from the respirator compared with the samples obtained before disconnection. Moreover, the proportion of cells expressing CD117 antigen depended on age of the donor.

[Analysis of early and late results of progressive Graves-Basedow ophthalmopathy treatment with different methods].

The early (immediately after the end of treatment) and late (from 12 to 91, mean 46.3 months after the end of treatment) results of progressive Graves-Basedow ophthalmopathy treatment were evaluated in 71 patients (57 women and 14 men, aged 25-66, mean 47.3 years). In all patients the thorough ophthalmological evaluation was performed early and late after treatment and the abnormalities found were classified according to Werner's method and Donaldson's ophthalmopathy index. The patients were divided into 7 groups according to different methods of treatment. Groups I-V consisted of 31 patients treated with glucocorticoids or glucocorticoids with azathioprine (Imuran) in the first stage of medication (30 patients). Plasmapheresis (7 patients) or tele-cobalt irradiation (3 patients) was applied as the second stage of treatment when the first stage was ineffective. In one person plasmapheresis and tele-cobalt irradiation was applied without previous glucocorticoid therapy. Two patients were treated successively by glucocorticoids, plasmapheresis and tele-cobalt irradiation. Very good and good late results of treatment were found in 95% of patients out of 22 reexamined persons of groups 1-5. Group 6 consisted of 7 patients treated with cobalt irradiation alone, in all 6 patients evaluated late results were good or very good. Out of 33 patients of group 7 treated by combination of glucocorticoids and telecobalt irradiation 23 were evaluated late and the results were found good or very good in 96%. The results suggest that combined treatment of progressive endocrine ophthalmopathy with glucocorticoids and cobalt irradiation is the most effective method of treatment at present. The affectivity of stage treatment is comparable with combined treatment but lasts longer and usually is more expansive.(ABSTRACT TRUNCATED AT 250 WORDS)

[Titers of antibodies against thyroid membrane antigens in patients with Graves-Basedow disease and progressive infiltrative ophthalmopathy during treatment with glucocorticoids and retro-orbital tissue irradiation].

In 22 patients with progressive endocrine ophthalmopathy (classes 4-6 according to Werner's scale and ophthalmopathy index at least 4 and evidences of infiltrative changes in retro-ocular tissues in computerised tomography, after achievement of euthyroid state) the titers of antibodies against thyroid membrane antigens (ATMA) were determined by Gardas and all. method. Control group consisted of 26 patients with Graves-Basedow disease without clinical signs of ophthalmopathy who were investigated for ATMA before and after conventional methimazole treatment. The determinations of ATMA were performed before treatment, after 3-4 weeks and subsequently once a month. ATMA's titers exceeding 1:1000 were found in 68% of patients with progressive infiltrative ophthalmopathy and in 77% of patients with Graves-Basedow disease without ophthalmopathy. The incidence of elevated ATMA titers during treatment of ophthalmopathy with supervoltage retro-orbital tissues irradiation with high dose of prednisone, decreased markedly in 6th month of treatment with subsequent increase to average 55% at the end of treatment what suggests the influence of therapy on antibodies production. The changes in the incidence of elevated ATMA titers during conventional Graves-Basedow disease treatment were different. No relations between increased ATMA titers and intensity of infiltrative ophthalmopathy and the course of treatment in the patients with ophthalmopathy were found.
