Methylene blue as a means of treatment for priapism caused by intracavernous injection to combat erectile dysfunction.

OBJECTIVE: The authors observed priapism as a side effect occuring during the intracavernous treatment of erectile dysfunction. Earlier priapism had been treated with an intracavernous injection of sympatomimetics; unfortunately several complications and contraindications were found. PATIENTS AND METHODS: Methylene blue was applied in the treatment of five patients. First a corpus cavernosum punction was performed and some blood was aspirated from the penis. Finally 100 mg of Methylthionin Chlorati was injected into the corpus cavernosum. RESULTS: A sufficient detumescence was observed in all of these cases. There were no complications. The method was applied effectively in two cases after an unsuccessful punction. CONCLUSION: The autors recommend intravenous methylene blue for the treatment of priapism. According to their experience this method is free of complications and as effective as a sympathomimetics treatment. As they think, it can be recommended in any manifestations of priapism because its force of action appears to be both chemically and biologically clear.

[Adjuvant BCG immunotherapy in the management of superficial bladder tumors].

The net benefit of BCG immunotherapeutic prophylactic effect on recurrence of superficial bladder tumours was investigated. The BCG treatment group consisted of 121 stage Ta, T1 patients, while the control group, 49 patients, was treated only with transurethral resection. During 3-year follow-up recurrence rate in the control group was 55.1%, while in the BCG group in the two-and-a-half-year follow-up it was 23.9%. The yearly repeated 6-week cycles resulted in decrease of recurrence rate to 11.8%. The recurrence indexes were 2.2 and 0.6 in the control group and in the repeated treatment group respectively. A progression rate of 18.8% was recorded in the control group and 4.1% in the treatment group. Finally both clinical and investigative results were summarised and the necessity of repeated immunotherapeutic BCG courses were emphasized.

Long-term BCG immune therapy of superficial bladder tumours.

Long-term local BCG treatment of superficial bladder tumours is described and the 5-year results are reviewed. Complications of major significance in the course of immune therapy did not occur, loss due to death was not recorded. PPD skin test failed to furnish extra information regarding the biologic behaviour of the tumour. Annual repeats of the therapy promise better results than one single 6-week course.

Increased susceptibility of the heart to arrhythmia in response to QT-prolongation associated with unhomogeneous repolarization: further data concerning the pathomechanism of arrhythmias associated with long QT-syndrome.

The pathomechanism of arrhythmias consecutive to a prolonged electrical systole --the long QT syndrome--was studied by local hypothermia in the heart. Local increase in the QT-interval (long QT with unhomogeneous repolarisation) was induced at a circumscribed area of the right ventricle by perfusion of the mid- and end-sections of the right coronary with cooled blood. The effect of local prolongation of QT on the arrhythmic susceptibility of the heart was studied by means of programmed electrical stimulation. Induced local hypothermia of the heart was found to increase the QT-interval of the ECG-tracings, also if QT had been corrected for heart rate. The local ECG tracings showed a considerable increase of the QT-interval at the selected right ventricular area under the effect of induced hypothermia but in relation to QT (ERP/QT) it diminished. The long QT in association with an unhomogeneous repolarization went hand in hand with an increased irritability of the heart. Early ventricular impulses were found to elicit extrasystolic runs, ventricular tachycardia or ventricular fibrillation. The results indicate that the cause of tachyarrhythmias consecutive to the long QT-syndrome may be ascribed to the inhomogeneity of repolarization on the one hand, and to the shortness of ERP in relation to the QT-duration on the other.

Experimental studies on the mechanism of the nitroglycerin effect.

The regional decrease of the speed and extent of the contraction and the relaxation of the canine myocardium induced by coronary ligation was enhanced by intravenously administered nitroglycerin (GTN), simultaneously with the decrease of the left ventricular pressure, while the increase of myocardial fiber length caused by that procedure was partly abolished. The above changes could be reproduced by GTN applied directly onto the ischaemic myocardial surface even before the decrease of the left ventricular pressure. These changes induced by GTN might play a role in the decrease or abolishment of regional asynergic wall motion.

Significance of the apexcardiogram in non-invasive assessment of postextrasystolic potentiation.

The study revealed the usefulness of the apexcardiogram in the assessment of the contractile state of left ventricle in cases of ventricular extrasystolia. The pre-ejection period/left ventricular ejection time (PEP/LVET) ratio and isovolumic contraction period (IVCP) are good parameters for demonstrating the postextrasystolic potentiation, the lack of which occurs in severe heart failure. PEP/LVET and IVCP are R-R-interval dependent in parasystolia. In the establishment of the postextrasystolic potentiation, the authors contribute an important role to the ventricular filling during the compensatory pause.

[Prevention of reflux esophagitis after total gastrectomy].

The experience gained in animal experiments and on human material with the surgical modification used to ensure the prevention of reflux oesophagitis and improved functioning of the residual stomach, is presented. In animal experiments it was possible to prevent reflux oesophagitis and the weight gain was also more favorable than after oesophago-duodenostomy and Longmire's operation after which reflux-oesophagitis developed without exception. Seven such operations have been performed on man; one of the patients operated upon more than 7 years ago is still free from complaints. In order to assess the true value of the operation, conditions of absorption, conditions of absorption have to be studied in animal experiments and more experience has to be gathered on human material.
