[Use of the Martin fixator for stabilization of bony fragments after resection of neoplasms].

The use of Martin fixator for bony fragments and graft stabilization after oncologic resection in 2 patients is presented. Good stabilization in both cases has been achieved. In one case due to the local recurrence of the tumor (sarcoma) an amputation was indispensable. In the other case (aneurysmal bone cyst) after 18 months complete union has been accomplished.

[Clinical heterogeneity of myelomeningocele as evidence of multisite closure of the neural tube].

Eighty-two patients treated surgically for myelomeningocele or myelodysplasia between 1945 and 1993 are presented. Family and environmental history has been studied. The presence of genetic markers (HLA B27, ABO, Rh) has been investigated. Heterogenicity of localization and type of defect in the series reviewed has been found. Associations between certain genetic features confirm multisite mechanism of closure of the neural tube. The relationship between site of neural tube defect and concurrence of developmental defects has been substantiated. A higher site often results in multiple defects.

[Companacci classification and result of treatment for aneurysmal bone cyst].

Results of treatment of 17 patients operated because of aneurysmal bone cyst have been assessed. Age at the onset of symptoms ranged from 4 to 39 years, 14 on an average. There were 6 aggressive and active cysts in this series according to the Campanacci classification. Treatment consisted of segmental resection of the bone in 5 patients, curettage of the lesion filing of the defect with autogenous bone in 11 cases. Recurrence of the cyst has been observed in 3 patients, only one of them had aggressive type of tumor.

[Thyroid function in patients during long-term treatment with amiodarone].

Thyroid function was evaluated in 31 patients with disturbances of heart rhythm both before and after 13 months of treatment with amiodarone. An increase in blood serum T4 concentration and transient increase (only during the first three months of treatment) in TSH concentration have been observed in almost all the patients studied. Clinically evident form of hyperthyroidism was found only in one female patient. In two patients the changes in hormone concentrations suggesting hyperthyroidism were not accompanied by clinical manifestations, and in four patients there was no clinical hypothyroidism despite hormone levels suggesting such a state. In addition to the determinations of T3, T4 and TSH also the results of TRH-TSH test played an important role in diagnosing both hypo- and hyperthyroidism.

Predictive value of various types of ventricular response to programmed ventricular stimulation: relation to Holter monitoring.

UNLABELLED: The study was performed to determine the predictive value of programmed stimulation for identification of pts with ventricular arrhythmias: 75 patients were studied by means of 24-hour ambulatory ECG (24 ECG) and programmed right (in some patients also left) ventricle stimulation at sinus and two or three pacing rates using two (standard) and three extrastimuli or burst stimulation (extensive protocol). Lown classes 0, 1-3 and 4a-4b were observed in 24 ECG in 35, 14, and 26 patients, respectively. In programmed stimulation 1-6 repetitive ventricular responses (RVR) were found in 56 pts, nonsustained ventricular tachycardia in 11 and sustained ventricular tachycardia in 21 pts. High incidence of induced VT was found in pts with complex ventricular arrhythmia in 24 ECG, 81% of this group, in all but six pts only standard protocol was used. The 1-6 RVR were observed in almost 40% of pts without any arrhythmia. CONCLUSION: Only VT induction is a useful index for high risk patients.
